NY Podiatrists Disagree on "Foot Facelift" Surgery

People are going under the knife to better fit into their favorite pair of heels. The procedure, nicknamed the foot facelift, is growing in popularity. "A lot of times, patients want to get into fashionable shoes, and things of that sort, or it is the result of wearing fashionable shoes too long," Dr. Oliver Zong said.

Drs. Oliver Zong and William Spielfogel

The surgery does have its critics. Dr. William Spielfogel, the director for the Foundation of Podiatric Medicine, said the foot facelift is unnecessary surgery and poses health risks. "Doing surgery just to fit into a pair of shoes, I don't think really is a wise decision," Dr. Spielfogel said. "There is possibility of getting an infection or pain, and there is swelling."

AT THE COLLEGES

Scholl Students Volunteer at Joint 35th Annual Midwinter Seminar

The two-day CE program, “A Step Ahead: The 35th Annual Midwinter Seminar” was co-sponsored by the Illinois Podiatric Medical Students’ Association (IPMSA) and the Dr. William M. Scholl College of Podiatric Medicine, Rosalind Franklin University of Medicine and Science (RFUMS), and was held on January 25-26, 2013 at the Westin Hotel in Chicago. The Midwinter Seminar is the IPMSA’s largest fundraiser for the year. Proceeds help support podiatric student activities and student scholarships. A total of 94 Scholl College students served as volunteers throughout the event.

(L-R) Seminar Planners Maliha Tahniyath, Laura Lasecki, Laura Reitz, David Schweibish, and Ali Malik.

This year, 140 podiatric physicians and other healthcare providers attended the seminar that featured topics in the areas of surgery, special patient populations, technology and imaging, and sports medicine. This seminar was an excellent way to learn from a variety of experts within the podiatric profession, as well as an opportunity to earn continuing education credits.

INTERNATIONAL PODIATRISTS IN THE NEWS

Aussie Podiatrist Now Practices as a Chiropodist in Canada

Long before CSI became a hit with television viewers, Australia-born Anthony Short set his sights on a career in forensic pathology. "I had always been interested in the sciences and health-related fields," says Short. "But my uncle, who is a podiatrist, suggested I follow his path because there would always be good job security."

Anthony Short (Photo: J.P. Moczulski)

"For the first time in my school life, I had found something that I really enjoyed. There is a lot more anatomy in podiatry so I made the switch," he says. Short attained a bachelor of health science (podiatry) from the Queensland University of Technology in Brisbane. Then in 2011, he left Australia to marry his Canadian sweetheart. He qualified and gained admittance to the College of Chiropodists of Ontario (COCOO), and now practices in his adopted home.

MI Podiatry Practice Receives Low Interest Loan For Digital X-Ray Machine

The Department of Environmental Quality (DEQ) officials today announced a $38,000 pollution prevention loan awarded to Shelby Foot & Ankle, PLLC to fund the purchase and installation of a new digital x-ray system, thereby eliminating wastes from its former conventional x-ray system.

(L-R) Drs. Anthony Giordano and Ronald Martin

The podiatry office received the low-interest loan through the DEQ's Small Business Pollution Prevention Loan Program. This project eliminates the need for radiographic film, lead-lined film containers, and the chemicals used to develop the film. The new equipment also reduces water and energy usage compared to conventional x-ray equipment. The DEQ and the company's local lending institution, Citizens Bank, each contributed half of the loan proceeds.

QUERIES (NON-CLINCAL)

Query: Source for Custom Dress Shoes

I am looking for a vendor who makes custom made shoes, preferably in the New York area; not for diabetics or for any foot pathology, but for stylish dress shoes...the kind you would go out and buy in a regular shoe store. I have a patient who has a hard-to-fit foot, and money is not necessarily an issue. Do these types of vendors still exist?

Query: Alternative to Onycol

We can no longer order Onycol, which I believe was a superior product for application to mycotic toenails. What are others using?

RESPONSES / COMMENTS (DME) - PART 1

RE: Medicare Rescinds AFO Height Requirement
From: Paul Kesselman, DPM

Medicare has announced that the height restrictions on AFOs have been rescinded. The policy article stipulating that AFOs come to somewhere near the fibular head has been revised to remove this requirement.

One should note that no changes have been made to medical necessity requirements or PDAC verification requirements for HCPCS 1906.

Congratulations is due the rest of the DME Subcommittee as well as Drs. Harry Goldsmith, Marc Block, Michael King, and Scott Haag (APMA staff) for a job well done. It is my hope that APMA will  be invited by CMS to assist in the development of a new AFO policy sometime in the future.

RE: Diabetic Therapeutic Shoe Bill (Dale Feinberg, DPM)
From: Josh White, DPM, CPed

I resent the implication by Dr. Feinberg that he follows SafeStep protocols. He has never used our WorryFree DME service to process a single Medicare order, and so I am not surprised at his difficulty complying with Medicare documentation requirements.

Since the recent posting by Dr. Johnson, it has been very satisfying to hear from other DPMs who have shared with me stories of their successfully passing scores of Medicare shoe audits by utilizing our WorryFree DME compliance service.

RE: Etiology of Hallux Varus (Jordan Sheff, DPM)
From: Dennis Shavelson, DPM

This does not look like a met adductus but instead a severe plantar plate injury and/or neurologic pathology with a windswept set of digits as the result. I believe an MRI is the starting place, not an orthotic.

RESPONSES / COMMENTS (PM JURY VERDICT REPORTER)

RE: Alleged Lack of Informed Consent and Causing of DVT
From: Adam Cirlincione, DPM

I was wondering if there is a source for podiatrists to read about these cases in their entirety, including statements made by expert witnesses. I am curious in this specific case as to why a podiatrist was alleged to have caused a DVT? With the incidence of DVT following foot and ankle procedures reported to be very low, my curiousity in this case revolves around an accusation of a physician being associated with the development of one.

I understand there may be more accusations made in this case, and this is why I ask if we can read more about it. I know there has been previous discussions in this forum on expert testimony on both sides, but I think it would be a valuable learning tool to be able to read what was said under oath by the experts.

Adam Cirlincione, NY, NY,  pittpod@gmail.com

Editor's comment: Full court testimony is public record and can be read by going to the court in which the case was tried. In the past, these transcripts were recorded by a court stenographer and often were costly to obtain. These days, most transcripts are digitally recorded and more accessible. PM News is currently negotiating with a service that will supply these transcripts, which we intend to post on our website.
