MI Podiatrist Discusses Heel Pain
 
According to Conway McLean, DPM, "Numerous treatments are available for plantar fasciitis. Yet diagnosing the problem correctly is a key first step. And when the reason is less obvious, healthcare providers unfamiliar with lower extremity mechanics and physiology may be unfamiliar with the possibilities. As should be expected, the location of symptoms and their character (sharp, dull, aching, etc.) varies greatly, in part because so many issues have the potential to lead to pain. One of the most frequently misdiagnosed, is called “Heel pad syndrome,” referring to the compartmentalized fat protecting the heel from pressure and impact. This syndrome develops when this specialized padding thins over the years.

"How does someone get a precise diagnosis for their heel pain? Seeing a specialist for care is often the way to go. Regardless, living with heel pain isn’t good for you, even if it isn’t severe. It doesn’t take a rocket scientist to know that pain from the heel with pressure is going to lead to less activity. The take-home message is there is a solution to everyone’s heel pain, although a good “first step” is an accurate diagnosis. Treatments work better when they are treating the right problem," says Dr. McLean.

American Podiatrists Volunteer in El Salvador
 
Marc Benard, DPM (CA) and Amol Saxena, DPM, MPH (CA) performed surgery at National Hospital Benjamin Bloom in San Salvador as part of Operation Footprint. Drs. Benard and Bhavesh Shah, DPM (TX) have been doing surgical brigades there via invitation from Dept. of Pediatric Orthopedics Chief, Odir Amaya as well as in Honduras and India.

Other participants included Neeta Hasmukh, DPM (TX), Matt Roberts, DPM (OK), and Maria Cifone (NY). The team operated on 13 feet which included neglected clubfoot, vertical talus, and deformities from cerebral palsy.

Congress reversing Medicare reversing the physician pay cuts for 2025 seems more like a slap in the face and doesn’t even come close to addressing the real problem. For years and years, Medicare has failed to increase physician payments anywhere close to the rate of inflation. A Medicare bunion reimburses $500-600 AND has a 90-day global period. It’s not sustainable. All the risk of surgery, all the training and investment of time, and 3-4 no charge follow-up visits, assuming nothing goes wrong. Yes, the patients are thankful, but a “thank you” doesn’t pay the bills.
 
We do more and more and get paid less than less. It’s absolutely ridiculous. I see that the AMA is on board with this absurdity. Physician payment increases should be put back on track to where they were 20 years ago, with adjustment for yearly inflation. What should the reversal actually consist of? 30% 50%? 80%? Don’t get me started on MIPS deductions and tracking.
 
Physicians are routinely undervalued, under-appreciated, and underpaid. But that’s “OK” because doctors are “rich” and “privileged” and “make enough money already”. If you want to know why we have low applicants for podiatry school, it’s because physicians are not valued in the United States. And with Medicare leading the way, it’s only going to get worse.

Dr. Rubin has hit this nail on the head. All we need is a group of “thought leaders” to come up with some solutions here. I do not feel it is the APMA that we should turn to. If they have not done this to date, there is a reason. I believe we need some real “in the trenches” DPMs with experience in life, and I for one, would volunteer to help out. We have so many problems going on concerning our future. 
 
I recently spoke with a recent residency graduate who did a three-year surgical program, and he said no one from his program ever did a case skin-to-skin in the three years of training. How is this possible? He said he is too afraid to say something as he is early on in his career and does not want to make any problems. I am just dumbfounded that no one has spoken up. If others reading this post are moved to be part of the “thought leaders” let’s hear from you and hopefully we can do something positive together.

Dr. Jacobs makes some poignant and comical statements as usual. In my humble opinion, he is one hundred percent correct. I have always introduced myself as a podiatrist. Being an expert does very little in the arena of managed care unless you are well known as “the” expert. Patients are more than willing to accept insurances that don’t cover their needs and are, more often than, not oblivious to the fact they are receiving sub-par medical care in the hands of alternative providers.
 
With all that is happening to our profession, both in our hands and the perceptions of the medical community, Dr. Jacobs has further made the point for us to obtain a medical degree with a sub-specialty in foot and ankle.

In the midst of all our downhearted comments about why our colleges are not doing well, a young college student called my office and asked to visit because she is interested in becoming a podiatrist. I welcomed her with open arms and will also arrange for her to shadow other colleagues who are more surgically oriented. Based on my conversation with her, I am positive that she will be a student at NYCPM.

Our profession has lost a great surgeon, teacher, and innovator. In my opinion, Dr. Isham was the most under-appreciated person in our profession. His contributions and innovations in MIS surgery have enabled thousands of surgeons in podiatry and orthopedics to improve the outcomes and lessen recovery times following foot and ankle surgery for hundreds of thousand off patients worldwide. Thank you Dr. Isham for your generosity in sharing your great wealth of knowledge and innovative techniques for these many years. A posthumous Podiatry Hall of Fame induction should be considered, way overdue.

RE: The Passing of Stephen Isham, DPM (Brant McCartan, DPM)

Stephen was a true gentleman and a trailblazer with his skill set and innovative procedures that he pioneered. He was always there to give advice and consultation on my cases when I needed it. I always enjoyed going to the yearly cadaver lab and lecture series of the Academy of Minimal Invasive Foot and Ankle Surgeons (MIFAS) where Stephen would work his way around the lab helping new practitioners and seasoned veterans in MIS technique and procedures. Steve was one of the several true pioneers of MIS surgery, not only in North America (Canada, USA, and Mexico) but he reached out to introduce and advance MIS techniques in Europe and South America. He will be sadly missed. Rest in Peace Stephen.
