Growing Pains Remain a Mystery: MI Podiatrist
 
According to Conway McLean, DPM, "Growing pains are defined as recurrent, self-limiting episodes of limb pain in children, usually affecting the lower extremities. It is a benign condition, causing no complications, occurring usually during early childhood. Often experienced at night, various designations have been applied. Some examples include ‘benign nocturnal limb pain of childhood’ and ‘recurrent limb pain of childhood.’ Regardless of the name, this appears to be a more frequent ailment than once thought, affecting one in three youths."

"The question remains unanswered: are growing pains a problem with bone health? Or is it the result of poor skeletal alignment leading to poor function, the all-important body  biomechanics? Clearly the act of weight-bearing is a critical component in this discussion since it is experienced almost exclusively in the legs. Numerous studies have been performed and sufficient data gathered. Conservative intervention in the form of in-shoe bracing, in conjunction with an appropriate physical therapy regimen, is recommended for the short-term and long-term management of growing pain in the presence of any abnormality of motion or mechanics. And yet why do foot supports help even when there isn’t a problem with function? Like so many other questions in medicine, it may for some time remain a mystery," says Dr. McLean.

BUSPM Student Establishes AAPSM Liaison Position
 
Recently, second year student doctor Ms. Madalyn Werd, American Academy of Podiatric Sports Medicine (AAPSM) Class of 2026 Delegate for the Barry University School of Podiatric Medicine (BUSPM), took the initiative to establish the newly-created AAPSM liaison position.

Madalyn worked closely with AAPSM's executive director Rita Yates to draft a resolution, which was passed at the House of Delegates annual meeting in August 2023 at APMA National in Nashville. Ms. Werd ran for the position of liaison and was appointed during the ACFAS2024 conference in Tampa, Florida.

Sacramento CA Podiatrists Would See Higher Taxes if Bill Passes
 
Close to 2,000 Sacramento professional license holders' business taxes will more than double if voters approve Measure C on March 5, the same day the city’s primary election is held. The proposed tax increases would raise the maximum rate by as much as 400% or up to $125,000 a year. Sacramento podiatrist Le Phong, DPM, the incoming president of the California Podiatric Medical Association, said the tax increase will be hard on new podiatrists.

“I guess somewhat established practices should be able to absorb the cost, but for some of the new people coming in, it’s rough,” he said. He said podiatrists already pay thousands of dollars yearly in various fees, taxes, and charges, from maintaining their state license to fees associated with operating x-ray machines and other radiological equipment. Phong said that is in addition to malpractice insurance and paying off student loans.

TX Podiatrist Rides Motorcycle 8 Hours to Protest Border Crisis
 
Protesters trekked in a vehicle convoy to Texas this week alongside American and Texas flags  rallying against illegal immigration near the U.S.-Mexico border on Saturday.

Rob Brace, DPM, a 79-year-old retired podiatrist, rode his motorcycle alone eight hours from McAllen, Texas, to greet the convoy, saying he felt compelled to come because of the high levels of illegal immigration. “People just seem to ignore the fact that our country is in a very bad situation,” he said.

It is with great sadness that I read of the passing of Dr. Vincent Muscarella. “Vinny” stood out as a classmate in so many ways. He was that rare combination of being a great student, while also being that classmate who always delivered on a fun experience. 
 
While proficient in the martial arts, he also loved to dance the hustle to Donna Summers! I will forever remember his huge smile, as well as his patience when trying to teach me to dance the hustle! Yes, he will be missed by many. He passed way too soon.

RE: The Passing of Vincent J Muscarella, DPM

Vince was a dedicated educator and clinician and friend. Some years ago, Vincent asked me to present a one day seminar as a fund raiser at Temple. I agreed to do so with no honorarium or reimbursement for any expenses. I asked only for one thing. Masaharu Morimoto had just opened one of his first restaurants in the United States in Philadelphia. At the time, there was quite the wait for a reservation. 
 
As a fan of the original Iron Chefs from Japan, my stipulation was that he obtain for me a dinner reservation at Morimoto’s. Not pay for the meal, just secure me a reservation. Not only did he do so, but he arranged for me to meet Morimoto himself. I still have and treasure my signed menu from the great chef. Thank you once again Vince. Rest in peace.

I wonder about the remaining 13% of patients with peripheral neurological involvement who ostensibly go undiagnosed or with a delayed diagnosis. What is the cost to them in time, advanced imaging, neurological consults, EMG, and potential degeneration of their health?
 
Do practitiners fret about recommending orthotics even when the patient must pay out-of-pocket? If not, is it because we believe that we are giving them our best advice? It is unfortunate that the discussion of abuse rises to the top in more than a few podiatry conversations.

The last response to the query by Heather Kaufman, DPM was quite the character assassination of Ivar Roth, DPM. The thoughts expressed by Dr. Kaufman, "The hypocrisy and arrogance that drips from every word of his posts like slime down a wall is nauseating" was off base and extreme. Although on the surface, Dr. Roth's treatment plan of turning a $125 debridement service into a $3,000 plus visit seems extreme, I'm assuming a detailed treatment plan to address the patients fungal nail condition is incorporating medication and probably the use of a series of laser treatments. 
 
The bottom line is that his patients are paying cash. I'm sure he gives his patients a detailed explanation of the course of treatment along with other options. Seeing that he is practicing in a liberal state like California, he is not holding the proverbial gun to his patients' heads forcing them into their decisions. I am sure Dr. Kaufman is an asset to the podiatric community. Going off on a fellow podiatrist like she did is not helping our profession.

I would like to acknowledge Heather Kaufman, DPM and Richard Manolian, DPM for their professional integrity in addressing concerns related to recent PM News posts. Their decision to voice these concerns demonstrates a commitment to upholding ethical standards within the podiatry community. It has come to my attention that certain podiatrists' representations of themselves as mentors and authorities in the field have raised significant concerns among peers.

Please note that this message is intended to encourage professional dialogue and should not be interpreted as a definitive statement on the conduct or practices of any individual. It is merely an acknowledgment of the ongoing conversations within our community and a call for continued commitment to excellence in our field. The method of loss leader bilking of patients is unseemly, if not fraudulent. A podiatrist's portrayal as a sole gatekeeper for podiatric privileges in local hospitals, based on the claim of being the only podiatrist with a three-year residency, has been a subject of contention. These representations impact the professional advancement of other podiatrists and bring into question the integrity of such actions.

The hyper-promotion of a surgical procedure, advertised extensively in the newspapers as a universal “orthotic solution” for all ages, leads to considerable professional scrutiny. A lack of understanding of adjunct procedures can lead to post-interventions by orthopedists and podiatrists, ultimately resulting in the procedure being excluded from local hospitals. Again, this brings our profession into scrutiny. These situations highlight the importance of professional integrity and evidence-based practice and the ethical responsibility of medical professionals to prioritize patient welfare over personal or professional gain.
