FL Podiatrist Publishes His Seventh Novel
Stephen Meritt, DPM, a practicing podiatrist in Jacksonville, Florida, has just published his seventh novel. His books were written for early readers and inspired by school stories written by his granddaughter. His granddaughter is listed in the credits of all the books and in the copyrights.

The books are a series involving the same three main characters in various worldwide adventures. The characters include a fictionalized version of his granddaughter, her next door neighbor, a wolf, and an earth spirit. Meritt’s books have received a positive reception and they are available through Amazon books in electronic version. Hard bound versions are also available through the publisher.

NY Podiatrist Takes Polar Bear Plunge for Make-A-Wish
Braving near freezing winter air temperatures, podiatrist Dr. Andrew Shapiro, from Lido Beach, NY, took the plunge on Super Bowl Sunday into the frigid Atlantic Ocean waves in Long Beach, NY. This was the 10th year in a row he participated in this event for the Make-A-Wish Foundation.

Cheered on by Long Beach residents, more than 5,000 people jumped into the Atlantic Ocean for the 20th annual Long Beach Polar Bear Club Dip. Organizers expect to raise well over a half a million dollars from this event.

2. An employee lies to you about something. It may be an inconsequential thing but remember, that is only the lie you caught. If an employee lies once, they probably lied about other issues as well. Patients' lives are at stake here and you simply cannot afford to keep an employee you cannot trust.

Query: Discriminatory Scope Issues
The State of New York currently has a discriminatory podiatry scope law that distinguishes the right to practice based on certain pathways. There is a pathway via The American Board of Foot and Ankle Surgery. This pathway gives one the ability to practice both podiatric surgery above the ankle as well as podiatric medicine above the ankle. Yet, those board certified in podiatric medicine have no pathway to practice only podiatric medicine. This is counter to the APMA and Council of Podiatric Medicine Education’s policy that there are two governing bodies in podiatry: ABFAS for podiatric surgery and ABPM for podiatric medicine. I wonder if New York is the only state with such discriminatory laws or are any colleagues in other states having similar troubles?

Query: Billing CPT 11721
I am billing CPT 11721 with ICD-10 L60.3 (nail dystrophy) as the primary and M79.671 (pain in right foot) as the secondary. I can’t get this claim to pass the scrubbing process. However, these ICD-10 codes are listed as billable for Medicare. Can anyone help with this problem?

Response: Since you are in Alabama, you would be billing Cahaba Medicare, as are we. The primary diagnosis code for CPT 11721 should be B35.1, onychomycosis.
Since you are using pain as the secondary code, and you are billing for 6 or more nails, then you should include pain for both right and left feet if that pain exists. We generally use M79.674 and M79.675 for pain in toes right and left. This combination should get the procedure paid. As always, make sure your notes support your claim.

RE: OSHA Bloodborne Pathogen Standard

In contrast to what we were required to do many years ago when the Bloodborne Pathogen Standard became law, OSHA now provides a great deal of help in developing a model Exposure Control Plan. Here is the link.
Although I keep a record of hazardous chemicals and the accompanying MSDS sheets, I will admit that I did not know that we are also required to have an Exposure Control Plan for the Hazard Communications Standard. A model for that plan is also included in this link. For those of my colleagues who don't think this is still important, think again. All you need is one disgruntled former employee (to complain).

I did not attend PCPM, so I did not know Leon as a professor. We practiced in the same community. I knew him as a friend and colleague. I cannot speak of his academic achievements, but I can tell you that he was one the finest gentleman I have ever known. Early in my practice, I had two serious life events occur. The doctor I was working for died suddenly at age 59. He was a friend of Leon's as were all of the podiatrists at that time. A year or two later, my only employee's son was killed in a car accident. In both cases, the first person who called me was Dr. Leon Kehr and his wife, Thelma.
Leon helped me run the practice when Dr. Roelofs suddenly died. His wife, Thelma, actually came to my office and worked as my assistant when my employee mourned her son's death. I remember asking Leon why he was being so kind and generous to someone he barely knew at the time. He said that podiatry is a small profession and we must learn to take care of each other. A good lesson for all of us today. Rest in peace, Leon.
