IL Podiatrist Discusses How to "Age-Proof" Your Feet
 
Unfortunately, as we get older, so do our feet. In fact, foot problems are often the first signs of aging. "Ligaments, tendons, and muscles become weaker and thinner and less able to hold the joints in proper alignment. Joint cartilage wears down as bones move out of alignment resulting in toes that are no longer straight or parallel to one another. The skin loses its ability to produce sweat and oil, resulting in peeling, flaking, itchiness, discoloration, thickening, hardness, and cracking," says podiatrist Dr. John Sigle.

Sigle suggests some simple ways to age-proof your feet. "Be a little more proactive and inspect your feet regularly. Look for a change of color, temperature, nail discoloration, cuts or cracks in the skin, scaling, peeling, or any new growths. Practice good hygiene. Wash and dry your feet regularly; and exfoliate and moisturize dry flaky skin."

WinterTrax, the handy gadget that turns stilettos into the perfect snow shoes, are nifty little devices that fit over the soles of a high-heeled shoe or boot to give them a snow-proof grip. They use coils, rather than spikes, to gain traction and allow you, says one anonymous user online, "to walk confidently on packed snow and ice."

Celebrity podiatrist Margaret Dabbs disagrees. "The problem is that high heels throw your weight forward, so your balance is compromised already, even before snow and ice. I think these look really good: wearing them would definitely make a fashion statement. I wouldn't be surprised if a designer copied this onto a permanent shoe or boot."

NY Podiatrist Takes Icy Plunge for Make-A-Wish
 
Braving near-freezing, 34 degrees air temperature, podiatrist Dr. Andrew Shapiro, from Lido Beach, NY, took the plunge on Super Bowl Sunday into the frigid Atlantic ocean waves in Long Beach, NY. This was the 8th year in a row that he participated in this event for Make-A-Wish Foundation. Cheered on by Long Beach residents, more than 5,000 people jumped into the Atlantic Ocean for the 18th annual Long Beach Polar Bear Club Dip. Organizers expect to raise well over a half a million dollars from this event.

The Super Bowl Sunday tradition started in 1998 with two friends who began collecting funds for Make-A-Wish Foundation to honor the memory of their friend's 4-year-old son, who died of cancer. To date, they've raised more than $3.5 million to grant wishes for kids with life-threatening medical conditions.

In a packed room overlooking Joplin High School's shiny new football practice field on Wednesday, Joplin football players Walker Easton and Donovan Harns signed national letters of intent to continue their careers at the next level. Easton, a highly regarded long snapper and offensive lineman, is headed to the University of Louisiana-Monroe.

Query: Erroneous MU Payment?
 
Our office successfully attested in late 2012 and received our incentive payment for $18K. We did not attest in 2013 for technical reasons. In late 2014, we again successfully attested for what we believed to be Stage 1, Year 2 and expected an incentive payment of $12K. Today, we received notification of a transfer into our account of $7,840 ($8,000 less 2%). We had already appealed the withholding of the 2% we were notified about recently, as we had met the measures needed. Should the Stage 1, year 2 payment be $12K as we expected, or is their $8K payment to us correct? If the payment is incorrect, how does one appeal the payment to get the additional $4K?

Why Your Patients Aren't Always Taking Their Meds
 
High drug prices are having a profound impact on patient adherence. Escalating generic drug costs and high co-payments can make it difficult for even insured patients to afford their medications. Newly approved drugs pose an even greater challenge. 
 
In a Consumer Reports Health survey of more than 1,100 U.S. adults who currently take prescription medications, more than one half (57%) had taken potentially dangerous steps in the past year to curb their drug spending. Among respondents with a pharmacy benefit, 19% skipped filling a prescription because of cost; 17% skipped a dose without professional approval; and 7% cut pills in half, again without approval. For respondents without a pharmacy benefit, such cost-cutting behavior was even more common: 37% said they had neglected to fill a prescription, skipped doses (33%), or cut pills in half (18%).

Query: Medicare "Dark" Days
 
I have not gotten paid in about two months from Medicare, and my billing service keeps telling me about these "dark" days that Medicare has. Can someone please explain this to me in plain English? This has happened a couple of times before. How can I run my practice not having gotten paid in two months?

Response: Because of fee scheduling issues, CMS instituted a hold on all payments for services rendered during the first two weeks of January. However, payments should have been re-started after January 15th. The term "dark days" are otherwise used by carriers during times when they are updating their systems. Usually, these occur during the weekend or non-business hours. Since you are continually getting the same answer from your biller and you are having significant payment issues, you might want to call your carrier yourself and be sure that there is not some other issue(s) going on.

We have had great success treating cases of idiopathic pruritis using Sarna lotion, which is available OTC. With regard to prescribing gabapentin 300 mg bid, it may be efficacious if the problem is neuropathic in origin. You must,  however, forewarn the patient that when initiating this drug or even raising its dosage, it will be temporarily but highly sedating. You may opt to start the patient on 100 mg at bedtime for three days and see how he reacts to it. Then gradually raise the dosage and only give it during the day if the patient fully understands that he cannot drive or use delicate machinery until his body acclimates to the drug.
 
Lastly, find out if the problem is due to winter dryness. Is this something that comes only in the late Fall or Winter when the heat goes on? You might advise that the patient install a humidifier in his room in addition to the use of Sarna lotion, or even one of the numerous emollients which you can dispense from your office.

Dr. Kesselman reports to us that the reason for DME inspections is to be sure we are living breathing suppliers of DME products. Non-living and non-breathing suppliers of DME are either dead, or not suppliers of DME, and therefore should not be a problem to anyone. I still don't know the purpose of these regulations.
