Biggest Problem is That Runners Do Too Much, Too Fast: CA Podiatrist

"The biggest problem is, runners try to do too much too fast," says Dr. Gennady Kolodenker, an Irvine, CA-based podiatrist who treats many athletes, from elite to weekend warriors. "If somebody wants to start running for exercise, you really don't need to do it more than 20-30 minutes a day, two to three times a week. If they enjoy running, then they can build up the miles."

Kolodenker says it's more important to stretch after a run than before, "though a good warm-up is important." Walk for 10 minutes, then work your way up to a slow jog.

MI Podiatrist Invites Patients to Annual Patient Appreciation Dinner

When was the last time you were invited to dinner by your physician? If you are a patient at any of the Hosey Foot and Ankle Centers in Macomb County, you receive an invitation every year from Dr. Thomas Hosey to his Patient Appreciation Dinner. Every year, Dr. Hosey introduces his wife, Mary Ann Hosey, and the other physicians at the practice, Dr. Jennifer Somers and Dr. Ryan Murphy. Dr. Hosey also makes a point to introduce every member of the support staff ranging from the medical assistants to the receptionists.

"I think it's important that our patients meet everyone who contributes to the success of Hosey Foot and Ankle Centers," Hosey said. "Our physicians, of course, have their special medical relationships with their patients. Our Patient Appreciation Dinner gives our patients the opportunity to also meet some of our staff they might not otherwise ever meet, such as those employees who work in billing or on insurance."

Facts: A 62 year-old male plaintiff suffering from gout in his right foot's great toe, claimed he was not properly informed that his surgeon intended to perform a fusion procedure or of the risks of that procedure. He also claimed that the fusion procedure was done incorrectly, causing him to suffer a deep vein thrombosis. The plaintiff required ten months of treatment and rehabilitation in a nursing home following the fusion procedure and suffers limitations and deformity.

HIPAA UPDATE

HIPAA Gets Tougher on Physicians

A revised set of federal privacy rules is expected to have a significant impact on the way physicians run their practices. Revised privacy notices will need to be displayed in prominent areas of doctors’ offices and on practices’ websites. Patients will be able to ask for copies of their electronic health records or restrict the information given to health plans if they self-pay for services. And perhaps most important, practices might be subject to serious fines if any of their business associates cause security breaches.

Under the new privacy rules, doctors now must assume the worst-case scenario in the event of a possible privacy breach. Previous regulations had required a practice to notify affected patients and the federal government only if it determined that a breach involving patient records had occurred and that it carried a significant risk of financial or reputational harm to patients.

I would advise against using Benadryl, phenergan, or benzyl alcohol as local anesthetics. I know of one DPM who tried undiluted benadryl as a local and the patient's toe was amputated secondary to necrosis. Without significant evidence of safety and effectiveness, I would never experiment with these drugs as local anesthetics.

With all due respect to Dr. Feinberg's comment to young practitioners giving up on DME, I vehemently disagree. I am a solo practicioner who generates 130 thousand dollars a year from all of the various forms of DME within our scope of practice. This revenue not only helps the practice to thrive, but also gives my patients improved outcomes from compliance and directed treatment protocols.

With the proper documentation, as you would find in an operative report, all hurdles can be overcome with due diligence. DME makes up 13% of my practice. I would gladly help any of my colleagues with issues regarding DME or refer them to the Codingline listserv.

RE: The Passing of Richard P. Reinherz, DPM

I was informed of the death of Richard on January 29th. I was shocked by this as he was still a very active man with a wonderful practice and family, taken by illness far too young. I knew Richard best during my surgical residency at American International Hospital in Zion, IL (1988) as he was the acting residency director at that time. He was also the editor of the Journal of Foot Surgery for many years. While we hadn't spoken in some time, I will miss him, and I will always remember how he helped educate me as a young podiatrist.

I find the discourse regarding changing and/or decreasing office hours quite interesting. I recently engaged in a conversation with a radiologist, and she explained that she took Mondays off. She stated that many people took Fridays off, but few people took Mondays.  She proclaimed that Mondays were best because they prolonged the weekend, stores were less crowded, and as a golfer, she found the links easier to navigate. That was food-for-thought. Of course a radiologist may not have the same practice constraints as a podiatrist. But I wonder if the patient that has to get in on Friday to prevent a weekend disaster would be the same patient that would wait until Monday to call to be seen.
