PODIATRISTS IN THE NEWS - PART 1
Calluses Can Be Dangerous: MN Podiatrist

According to Conway McLean, DPM, "Calluses on the feet, be they on the toe, sole, or heel, can be dangerous. There are a multitude of treatments, methods, and approaches for minimizing the consequences of chronic skin pressure. These calluses can be treated, and often should be, so see a specialist. There are consequences to calluses… they just may not be today."

Dr. Conway McLean

"Calluses, once an afterthought, often turn into a source of pain. This process of skin thickening has more implications than just pain. Fibrosis is scarring of the deeper layers of skin and can be intensely painful with pressure. Skin breakdown can also result at these sites. Alternatively, ulcers can develop when skin is thin and unhealthy, as in those with arterial disease. Those with heart disease are also prone to this common occurrence," says Dr. McLean.

PODIATRISTS IN THE COMMUNITY - PART 2
WI Podiatrist Discusses Plantar Fasciitis

If you experience chronic heel pain, chances are you suffer from a condition called plantar fasciitis. An estimated 10 percent of Americans suffer from this condition. "Patients with plantar fasciitis often experience a sharp pain in the foot the first thing in the morning or when getting up after a prolonged period of sitting, said Dawn Anderson, DPM

Dr. Dawn Anderson

"The condition often develops gradually and can affect one or both feet.” The extent of the pain and inflammation from plantar fasciitis is different from person to person. Once diagnosed, treatment options can include simple changes, such as modifying your activities, losing weight, icing the area, changing your running or exercise regimes, or purchasing more supportive shoes. Other patients may require medication, physical therapy, or orthotics. An extremely small percentage of patients require surgery to repair the problem.

PRACTICE MANAGEMENT TIP OF THE DAY
2021 Remote Patient Monitoring Changes:  Key Takeaways - Part 2

In December, the Centers for Medicare & Medicaid Services (CMS) released its 2021 Medicare physician fee schedule final rule. Within the final rule were some significant changes concerning remote patient/physiological monitoring (RPM) and the provision of other remote services. Here are the most significant takeaways from the final rule.

2. Clarification concerning device requirements - The final rule clarified uncertainties surrounding the types of devices supplied to patients as part of CPT 99454. Of note, CMS stated that such devices must meet the definition of a medical device per the Food, Drug, and Cosmetic Act and electronically (i.e., automatically) collect and transmit a patient's physiologic data rather than permit patients to self-report or self-collect data.

The latter is important because it clarifies the separation between self-reported and automated RPM patient data. There have been a few new platforms in the RPM market that would have patients take pictures of their device readings or manually enter them into an app each day. This ruling makes clear that these and other mechanisms of self-recording data are no longer acceptable.

RESPONSES/COMMENTS (CLINICAK)
From: Steven J. Kaniadakis, DPM

Basically, I think that the concern should include that the patient continues to "itch" the wound or another lesion like this one.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: Ciox Health Responds to Podiatrists' Concerns and Asks for Input
From: Dave Yohe

Ciox understands that the primary concern of providers is patient care. Administrative exercises, including fulfilling medical record requests can be burdensome. Ciox is proud to be leading the digital transformation of medical record exchange, which helps minimize the work providers must do to share medical records. In fact, Ciox provides release of information support for over 15,000 clinics. For clinics that are not customers, Ciox has a solution that allows providers to grant controlled remote access to their EHR, so providers do not have to manage chart requests.

As more context, when Ciox calls to request medical records, it is on behalf of health insurance plans. The health insurance plans are often seeking records to fulfill CMS requirements. And data from those records also helps fund provider payments.

We are always seeking to improve our services and welcome direct discussion on how we can make the medical record exchange process easier for you. Please send us a note at help@cioxhealth.com

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2A
From:  Allen Jacobs, DPM

In my opinion, the expressions of doctors Markinson and Agostinelli regarding the issue of “parity“ are spot on. Yes, there are frustrations inherent to the limited podiatry degree. It seems illogical that I can prescribe opioid analgesics and other potentially dangerous medications, yet I am not permitted to prescribe medical marijuana. It is intriguing that I can perform a Syme amputation or utilize an IM nail to stabilize a Charcot’s ankle but require an MD's signature to prescribe therapeutic footwear for a diabetic patient. It defies logic (to me) that a podiatrist is paid less than on orthopedic surgeon for the same service when, in fact, a 3-year residency and fellowship-trained podiatrist is better trained in foot surgery.

The parity argument is, to my thinking, a straw man argument. As a podiatrist, you are a limited licensed healthcare provider. You are not a “specialist”. You did not graduate medical school. Your training is not...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2B
From: Sheldon Miller, DPM

Parity. Who cares about parity? I practice with a brilliant board-certified neurologist. Over the past 5 years, he has been increasingly treated like garbage from the insurance companies, Ciox, etc.  His reimbursements have gone down just like ours. If the public doesn't care about a life-saving doctor, they will never care about our profession (podiatry, specialty). I don't care what you want to call us, but we have to hold our own by ourselves. Getting paid with INCREASES annually shows respect. Pay me what I deserve and you can call me whatever you want to.

RESPONSES/COMMENTS (PM NEW QUICK POLLS)
RE: Participate in PM News Polls
From: George Jacobson, DPM

After reading the varying views on oral antifungal protocols and minimalist or barefoot function, I have no doubt why it is not difficult for the plaintiff's attorneys to find "expert" witnesses to testify to their theories of malpractice. Fortunately, both of these subjects appear have a preponderance of what could be the defense's views. I encourage all readers to vote in the PM News polls, so that we have the broadest views in the results of these queries and not just the same 800-900 respondents. I always look at the results and although they are not scientific, they are a good general indicator.
