Dry Skin a Particular Threat for Diabetics: IL Podiatrist
 
Dry skin can lead to many major concerns in diabetes. Cracking and peeling result in openings in the skin, allowing bacteria or fungi to enter the body. “With bacterial infections, you most often first see a redness around the area of the cut or crack,” said Matthew G. Garoufalis, DPM, a podiatrist in private practice in Chicago.

“People with diabetes are at a disadvantage because they often will not feel pain, which can be an early warning that an infection is occurring. As it progresses, you may also see drainage from the wound, and it may become warm to the touch,” said Dr. Garoufalis.

Melissa Memorial Hospital has announced the appointment of podiatrist Dr. James Yakel to its staff. He graduated from the Ohio College of Podiatric Medicine and completed his residency at Mt. Sinai Medical Center, also in Cleveland. He has been in private practice for 18 years.

Yakel is past president of the American Academy of Podiatric Sports Medicine, and lectures locally and nationally on sports medicine topics. He is a Fellow of the American Academy of Podiatric Sports Medicine, Diplomate of the American Board of Podiatric Surgery, and a Fellow of the American Professional Wound Care Association.

CA Podiatrist Admits to Allegations He Groped Patients
 
As up to nine women were set to testify against him in court, a Torrance podiatrist pleaded no contest Wednesday to charges that he groped patients’ breasts, hips, and pubic areas during foot examinations over a 10-year period. Arnold Serkin’s open plea came as jury selection was set to begin for his trial in Torrance Superior Court. The terms will require him to register as a sex offender and earn him a sentence ranging from probation to seven years behind bars, the term he could have received if convicted by a jury. The 70-year-old Rancho Palos Verdes resident entered his plea to four counts of sexual battery by fraud with the understanding that his sentence would be no more than a year in county jail, attorneys said.

Query: Subtalar Arthroereisis Coding
 
Since there is no specific code for subtalar arthroereisis, can we use CPT 28725 with "-22" modifier?

Response: The code you are to use is 0335T (extra-osseous subtalar joint implant for talotarsal stabilization). This is a Category III code specific for the procedure you are performing. 
 
It would be unacceptable to use CPT 28725 (arthrodesis; subtalar). It certainly would be inappropriate to add a "-22" (increased procedural service) modifier. There is a "CPT Assistant" article specifically addressing this. Using CPT 28725 would be misrepresenting what was actually done. 
 
You should be aware that there are a number of payers who do not cover the subtalar arthroereisis procedure (denied as investigational). You should request pre-authorization of 0335T.

Thank you, Dr. Horowitz for bringing up a very important point about rheumatic diseases. The blood tests, whether positive or negative, are only a piece of the puzzle. In your wife's case, the tests were negative for years and psoriatic arthritis went undiagnosed. In many other cases, we have found patients with positive ANAs and even positive RA factor tests, who turn out to have nothing wrong with them.
 
We as doctors do not treat lab tests, we treat people. If a patient presents with a swollen toe, even though the bloods might be normal, there is pathology that needs to be addressed and treated and visa versa. If the tests are abnormal, but the patient is feeling fine, that person should not be treated with all of the "great and powerful" new disease-modifying drugs that we now have. For certain patients, sometimes taking a step back and monitoring the situation is often best.

Regarding bloodwork, psoriatic arthritis is often a diagnosis of exclusion. Having skin lesions and or radiographic changes consistent with the disease, i.e. pencil and cup deformities, makes the diagnosis easier. Biomarkers in the blood should be considered. MMP3 (metalloproteibase-3) and hs-CRP (high sensitive C-reactive protein) as well as VEGF (vascular endothelial growth factor) have been shown, for example, to be useful in early detection of psoriatic arthritis and to monitor disease progression.

RE: Gross Underpayment of Medicaid Surgeries--an Attack on Access to Care

In a study authored by Dr. Charles Mabry of the University of Arkansas in Little Rock and colleagues in the Journal of the American College of Surgeons, Dr. Mabry reports that: "When Medicaid fees are too low relative to payments from Medicare, doctors may refuse to treat Medicaid patients, potentially making it much harder for poor people to get treatment." 
 
Some of the results of the study showed shocking underpayments to surgeons: "When they looked at mastectomies, often done for women with breast cancer, Medicaid paid $226.47 in Connecticut, 69 percent less than...

If an easy path were to be created for a person with a DPM to earn an MD, would that person give up practicing podiatry? I would not because I am happy practicing podiatry and my patients are happy that I practice podiatry. I hope that when and if an MD or DO degree becomes available, that it will not diminish the number of podiatrists out there because we really do a lot to eliminate pain and suffering for so many people.
