CO Podiatrist Discusses Dangers of Self-Treatment of Corns
Corns are just calluses that are on the toes instead of the bottom of the foot. “Corns are caused by friction, just like blisters or hand calluses,” said Dr. Gordon Rheaume, a Pueblo podiatrist. “They don’t have roots; they’re just a thickening of the skin.”

Corns can be shaved down to create more space, and toe pads help keep the shoe from coming into direct contact. “Sometimes people get too aggressive and they put medicated pads on the toe and create a hole in the skin,” Rheaume said. An open wound can lead to infection, so the recommendation is to use regular pads that have no medication in them.

IL Podiatrist to Lecture at ICDC in Mexico
Recently admitted as a surgeon to the medical staff at Shriners Hospital for children, Dr. Paul Bishop is an invited faculty member to the International Conference on Deformity Correction (ICDC) to be held in March in Mexico City, where he will be lecturing, teaching, and performing specialized surgeries.

Bishop has been performing pro bono surgeries on children internationally through the Chicago Lower Extremity Surgical Foundation (Walk-Run-Dream Foundation) since 2010, according to a news release. He and his colleagues travel abroad to take on surgeries funded by the doctors and the foundation. Bishop and his partner, Dr. Thomas Rappette, have practiced together for 17 years at the Foot and Ankle Centers in Yorkville, Morris, and Sandwich.

Dr Ivan Bristow, a senior lecturer in podiatry at the University of Southampton and the first podiatrist to use Clearanail, said: “Clearanail is a great British innovation. It’s very difficult for medication such as an over-the-counter paint or lacquer to reach where it needs to be through a thickened nail. "Clearanail is a 20-minute treatment which is unique in that it allows the impermeable structure of the nail to be made permeable in order to reach the source of the infection," says Bristow.

CMS Relaxes Meaningful Use Data Exchange Requirement
The Centers for Medicare & Medicaid Services (CMS), which just announced it plans to shorten this year's reporting period for meaningful use from a full year to 90 days, is trying to lighten the burden on healthcare providers in other ways, too. Late last week, CMS gave a reprieve to eligible hospitals and professionals who have found it difficult to meet the transition-of-care requirements in meaningful use stage 2.
Under part of this measure, a provider must exchange a care summary with at least one other provider or third party who has a certified electronic health record (EHR) that is different from the sender's system. Alternatively, the provider may conduct a successful test of a care summary exchange with a CMS-designated test EHR. The catch is that to do the latter test, a provider must use an EHR that can send Direct messages to a disparate EHR, using a health information service provider (HISP) that has been accredited by DirectTrust, a non-profit trade organization. Not all EHRs, however, connect with one or more accredited HISPs.

Obama Threatens To Veto Latest ACA Repeal Bill
The Obama administration issued its 10th veto threat of the year Monday, promising to scuttle a House bill to repeal the Affordable Care Act. The veto threat wasn't surprising -- Obama has long said he would defend his landmark legislative achievement with his veto pen if necessary.
But it continues to put the beginning of the 114th Congress into record-setting territory: nine veto threats against House bills, plus a verbal threat against an Iran sanctions bill being drafted in the Senate. That makes more bills under threat of veto to begin the year since the Office of Management and Budget first started issuing formal positions on bills under President Reagan in 1985.

Sanofi Launches Inhaled Insulin for Diabetics
Sanofi launched an inhalable insulin in the United States on Tuesday in a potential boost for its flagging diabetes drug sales and for patient quality of life. Developed by Mannkind Corp, Afrezza will be the only inhalable insulin on the U.S. market, where Sanofi competes with Eli Lilly and Novo Nordisk for sales of traditional injectable insulin.
Inhalation promises to be faster-acting and much more convenient than injections, but an inhaled product has failed in the past and there are concerns about the potential risks associated with breathing powdered insulin. Afrezza, which uses a whistle-sized inhaler and works to control blood-sugar levels in both type 1 and type 2 diabetes, was approved by the U.S. Food & Drug Administration in June.

Patient-Driven Teledermoscopy for Atypical Nevi Successful
Mobile devices equipped with cameras can effectively enable patients to monitor atypical nevi and send images to their healthcare providers, according to an article published online January 28 in JAMA Dermatology. Nevus monitoring typically spans 2.5 to 4.5 months. Approximately 19% of lesions change during that time, with 11% to 18% of them eventually becoming malignant. Clinically significant changes seen by comparing images side-by-side is evidence for excision of the lesion.
Patient-driven mobile teledermoscopy uses an application, DermScope, on an iPhone to transmit images from the patient to the healthcare provider. The technology may help alleviate the increased patient load in dermatology resulting from the aging U.S. population and increased numbers of insured patients.

Since I posted the suggestion to use doctordefender.com to help collect from patients who skip out on high deductibles, Dr. Beaton has been besieged with questions about the service. I wish I could address them all, but here are answers to the most FAQs.
1) HIPAA doesn't apply to collections. doctordefender.com does not share any patient medical information and HIPAA is therefore not applicable. You can't tell which patient saw which doctor. You can only see if a patient was posted by another doctor as a non-payer.
2) You do not need the patient's email address.
3) You can pay the annual fee and just utilize the database to identify patients who don't pay their bills. You do not need to use the notification letter; however, we find it to be very effective and much more affordable than a collection agency.
4) They don't have a phone number that is public. We figure it's because they don't want patients calling. They are very accessible through their email address.
