IL Podiatrist Joins Provena Saint Joseph Medical Center Staff

Provena Saint Joseph Medical Center in Joliet has welcomed eight new healthcare providers to the hospital's medical staff. Among the recent medical practitioners who will be joining the hospital staff Dr. Robert MacNab, a podiatric surgeon with Rezin Orthopedic Centers.

Dr. MacNab graduated from Dr. William Scholl College of Podiatric Medicine in 1990 and completed his podiatric residency at Leyden Community Hospital, Northlake, IL and Columbus-Cabrini Hospital in Chicago. He is board certified in podiatric orthopedics and in foot surgery. He joined the Rezin Orthopedics and Sports Medicine team in 2001.

NY Podiatrist Takes Icy Plunge for Make-A-Wish

Braving sub-freezing temperatures, Podiatrist Dr. Andrew Shapiro from Lido Beach, NY, took the plunge on Super Bowl Sunday into the frigid Atlantic ocean waves in Long Beach, N.Y. This was the 6th year in a row he did this for Make-A-Wish Foundation. Cheered on by Long Beach residents, who have been recovering from the aftermath of Super Storm Sandy, and battling an air temperature of 29 degrees, more than 5,000 people jumped into the Atlantic Ocean for the 16th annual Long Beach Polar Bear Club dip.

Organizers expect to raise over a half a million dollars from this event. The Super Bowl Sunday tradition started in 1998 with two friends who began collecting funds for Make-A-Wish Foundation to honor the memory of their friend's 4-year-old son, who died of cancer. To date, they've raised more than $3 million to grant wishes for kids with life-threatening medical conditions.

TX Podiatrists Prevail Against Hospital Which Tried to Restrict Ankle Privileges

Cory Brown, DPM and Martin V. Sloan, DPM were on the Hendrick Medical Center's medical staff. The medical center gave them notice that their podiatry privileges would be administratively reduced by eliminating all ankle privileges. The podiatrists sued and sought injunctive relief, and a temporary injunction was granted. The medical center filed a motion to dismiss under Tex. Civ. Prac. & Rem. Code Ann. § 74.351, but the trial court denied the motion. On appeal, the court affirmed.

For purposes of Tex. Civ. Prac. & Rem. Code Ann. § 74.001(a)(1), the claims of the podiatrists and the Texas Podiatric Medical Association did not involve treatment or care that was rendered to any patient. Instead, their claims related to a dispute with the medical center as to the scope of the podiatry practice. The elimination of ankle privileges was not an inseparable part of a patient's treatment of care, and thus the medical center's act of eliminating privileges was not directly related to healthcare. Quality of treatment was not at issue. The trial court therefore did not err in denying the medical center's motion to dismiss.

INTERNATIONAL PODIATRY IN THE NEWS

Complaints Against UK Podiatrists Orchestrated by Orthopaedic Surgeons

Orthopaedic surgeons claim they regularly have to repair disasters caused by podiatric surgeons. "There are two groups of podiatric surgeons, one of them is willing to work alongside orthopaedic surgeons in hospitals, and the other is not," says Anthony Sakellariou, spokesman for the British Orthopaedic Foot and Ankle Society. "All the problems I have seen come from this second group."

The Society of Chiropodists and Podiatrists insists the complaints about podiatrists are being orchestrated by orthopaedic surgeons anxious to protect their territory. Spokesman Ron Finlay said: "A small number of cases go wrong, whether conducted by podiatric surgeons or orthopaedic surgeons. It is not the case that podiatric surgeons are any less competent than orthopaedic surgeons when it comes to foot surgery."

I was saddened to hear of the passing of Rick Reinherz on January 29th. Rick succumbed to a long battle with cancer. Rick was one of my co-residents at Kern Hospital -1976-77. Rick contributed to our profession immensely. He served as Editor of The Journal of Foot & Ankle Surgery in the early '80s. May you rest in peace, my friend.

RE: Alternatives to Ester and Amide-type Anesthetics (John Doolan, DPM)

If you decide to use Benadryl, you should be aware that it needs to be diluted with NSS at a ratio of 5:1. There is anecdotal evidence that using injectable Benadryl straight may cause skin necrosis (possibly massive). When used, Benadryl has the same onset and duration as Lidocaine. Another option is phenergan, which can be used straight and also has the same onset and duration as lidocaine.

One last option is benzyl alcohol. It can’t be purchased separately (unless you’d like a 50-gallon barrel of it), but it is commonly used as a preservative in NSS (in multi-draw bottles, as single-draw bottles/vials use no preservative). Benzyl alcohol also has about the same onset and duration as lidocaine and the volume of the block itself will cause anesthesia.

If my recollection is correct, coverage for diabetic shoes was instituted by an Act of Congress. It appears that current attempts to effectively deny that coverage via the application of unreasonable bureaucratic hurdles then violates that legislation. APMA really needs to show some resolve.

We have been doing the diabetic shoe  program since 2000. While it is not an easy road, it does provide a valuable service to patients who comply with it. Our audits increased to 30% of DM shoe cases last year. Most passed the first round, but, some required resubmission of documents to the next level of redetermination, which has a 40-60 day delay in payment. Our documentation is ethical, extensive, and fully compliant but some reviewers obviously or purposely don't see that. We submit a letter with a timeline and copies of EVERYTHING REQUESTED.

We find Dr. Josh White's Worry Free DME procurement system for the MD/DO documents helpful. You must have a certifying physician office note copy including DM diagnosis and treatment within 6 months of the DME dispense date in documents. A few doctors are sluggish or, rarely, unable to sign the Medicare required forms. So be it. We feel that the program benefits our diabetics who choose to comply with it. We monitor and document that as well. Cautiously optimistic, we accept that certain aspects of podiatry will continue to be challenging, and difficult but not insurmountable in the future. We hope.
