TN Podiatrist Joins Maury Regional Medical Staff
 
Danielle L. Malin, DPM, a specialist in podiatry, has joined the medical staff at Maury Regional Medical Center. She is associated with Advanced Foot & Ankle Centers in Columbia and Spring Hill, Tennessee.

Dr. Malin received her doctor of podiatric medicine degree from the Ohio College of Podiatric Medicine in Independence, Ohio. She completed a residency in podiatric medicine and a surgery residency at the St. Francis Hospital and Medical Center in Hartford, Connecticut.

NC Podiatrist Retires After 48 Years
 
He has helped the lame to walk and brought patients to tears of gratitude. Now, after 48 years as a Durham podiatrist, Dr. John Iredale has retired from a profession that made him known throughout the world. Born in Canada, Iredale graduated in 1967 from the Ohio College of Podiatric Medicine south of Cleveland and moved to Durham the same year. His desire to be a foot doctor started when he was a young boy and had a wart removed from his foot. When he told the surgeon he’d like to be a doctor, he suggested the young Iredale consider being a podiatrist. As in many areas of his life, he felt God was leading him, so he followed.

Iredale estimates that he has treated 75,000 to 90,000 patients since he opened his practice in Durham in 1967. While most of his patients were from the Durham area, the reach of his practice has far exceeded the Triangle. Last year, Iredale spent 70 days in a hospital devoted to the care of patients with leprosy in Nepal, helping medical professionals update their technology and teaching them how to manage diabetic feet by using principles of biomechanics. “As podiatrists, we’re in the business of saving feet,” Iredale said. “I’ve had a lot of really wonderful help — doctors and incredible staff. It has been a good life.”

NJ Podiatrist Attends AAPL Winter Institute
 
Des Moines University graduates Peter C. Paicos Jr., DPM (CPMS '86) and Lt. Col. Jason Musser, DO (COMS '03) recently attended the Winter Institute of the American Association of Physician Leadership (AAPL). The mission of AAPL is to unite physician leaders into leadership roles throughout the healthcare delivery system. Physicians of all specialties meet to discuss, learn, and train for leadership in the healthcare arena. Members include chief executive officers, chief medical officers, a vice president of medical affairs, medical directors, and other physician leaders.

Bipartisan Senate Bill Seeks to Expand Telemedicine Services
 
A bipartisan group of U.S. senators Wednesday are introducing a bill aimed at expanding telemedicine service through Medicare benefits. “Telehealth is the future of healthcare. It saves money and improves health outcomes,” co-sponsor Sen. Brian Schatz (D-HI) said. “Our bipartisan bill puts us on a path to transform healthcare delivery, making it less costly and more convenient for patients and providers.”
 
The bill, called the Creating Opportunities Now for Necessary and Effective Care Technologies (CONNECT) for Health Act would expand the use of remote patient-monitoring for some patients with chronic conditions, increase telemedicine services in community health centers and rural health clinics, and provide basic telemedicine benefits through Medicare Advantage.

I have an identical picture of my wife's foot from about 6 months ago. She has psoriatic arthritis and has been treated with Embrel for about 7 years. Before that, it took about 8 years for her to be diagnosed. All blood tests consistently were negative and her rheumatoid profile was negative repeatedly. The diagnosis was made through clinical patterns and then, when she started Embrel, the lesions (mostly in the palm of her hand) and pains went away.
 
Psoriatric arthritis
 
We had a scare (swollen lymph nodes) about 4 years ago, so she stopped Embrel. Six months later, all the signs and symptoms returned. She started the injections again about a year ago now and she is doing fine. We attribute this recent flareup to her psoriatic arthritis.

Consider psoriatic arthritis. Since treatment may require long-term anti-inflammatories, either older ones such as sulfasalazine or plaquenil, or newer disease-modifying anti-rheumatic drugs (DMARDs), all requiring periodic lab monitoring. Why not let the rheumatologist treat this patient and perhaps become your best referrer?

Dr. Boudreau, thank you for your query. Most everyone working with feet, postures, shoes, and fitness, including shoe salesmen/women, are capable of functional foot typing within an acceptable margin of error. This assumes proper training, experience, and monitoring. The FFT qualitative tests have high intra- and inter-tester reliability after a short learning curve. As this foot typing method is U.S. patent protected, those claiming to be “functional foot typers” if poorly trained, inexperienced, or unattached to the standards and licensing that exist for this IP are violating federal law. Think of where biomechanics and podiatry would be if Dr. Root had patented his inventions, preventing them from being corrupted and degraded for seventeen years.

When I take a medical history on my patients, I often have to look up unfamiliar medications. For us lifelong learners, I thought a little list of some of the new ones would stimulate interest. I believe none of these are directly prescribed by podiatrists, but we should be familiar with them. I pity the primary care docs who have to keep up and use these!
 
First, I promise I did NOT make this up. Second, I have tried to be very careful about spelling. Ready? Herein lies one of the problems with perception in the podiatric community. To conduct a poll asking "if it took 6 months to upgrade to MD would you enroll?" It's a ridiculous premise and impossible to achieve. Rotations in Internal Medicine and Surgery alone take 6 months. What about all the other core rotations? It takes a minimum of two years to complete them. This conversation about degree change has been going on for more than three decades. Until the powers that be realize that podiatric and medical education are not mirror images, nothing will change.
