NJ Podiatrist Promotes Two Charity Events During Super Bowl Week 
 
In preparation for the Big Game, the Foundation for a Drug-Free World, Bergen County Chapter, sponsored by foundation director Dr. Dan Margolin of the New Jersey Foot and Ankle Center, has been giving back to the community with the help of local football stars and organizations. The foundation has been a rallying point for volunteers from Margolin's podiatry office as they try to eradicate the use of illegal drugs in Bergen County.

This past week, the foundation has been on the playing field for two local charity events: the Super Community Spirit Showcase and the Big Game 5K. “With the spotlight on New Jersey, no time in history has the importance of volunteering been so relevant,” said Margolin. “It is important that we show the country that the dependency on government is being replaced by community teamwork."

WI Podiatrist Discusses How to Choose Exercise Shoes 
 
Thurmond Lanier, DPM, Aurora Health Care Oshkosh and Fond du Lac, helps us better understand the importance of proper foot care for exercise. Lanier suggests people consider four characteristics when purchasing exercise shoes: Size, Shape, Stability, and Style. Shoe size is very important when choosing the right shoe.

The shape of the shoe should match the curve in the foot. Dr. Lanier recommends holding the shoe up to the bottom of the foot to match the shapes. Stability of the shoe can be checked by grabbing the heel of the shoe and squeezing it. If you can press it all the way down, it’s not a good shoe.

Michael Brody: No, the doctors’ notes are a stream of consciousness, just text. Meaningful use (MU) is about structured data. CBCs, blood pressures, etc. will always be in the same place in the system. Then, the databases can apply the structured data into any provider's note system. The physician’s note is the least important aspect of MU; today it's about sharing the structured data.

I have treated many of these over the years. I usually confirm them with an MRI and then put them in a pneumatic walking cast for up to two months. Definitely follow up with orthotics to minimize possibility of re-tearing. Use good supportive sneakers/boots with a shock-absorbing outersole. Future measures should include losing weight, avoiding impact activities, and limiting exercise to a stationary or real bicycle, elliptical machine, or swimming. Avoid treadmills and jogging. Then pray for the best outcome.

RE: The Passing of Donald Arenson, DPM
 
I was shocked, but most of all saddened, when I read of the passing of Dr. Don Arenson. He was one of the brightest lights in the podiatric profession; an extremely intelligent and articulate individual, the likes of which is truly a rare find today. This man was an educator first and foremost and a real gentleman. At the Scholl college, he taught all of us biomechanics and how to apply it to surgical reconstruction .  
 
I personally owe him a great deal of gratitude for the influence he had on my education. He taught us how to approach a patient in an analytical fashion and strive for the best treatment outcomes through systematic and practical means. I am overwhelmed now but offer my sincere condolences to his family. This profession has lost a significant and special individual.

I am in practice for about 30 years, and 95% of my practice is conservative foot care. Anyone in the Queens, NY area who is too busy to perform this "inferior" service is more than welcome to send patients to my office. I'll perform this service, NOT my super assistant. I totally disagree with any doctor relegating some important services to an assistant. This is outrageous. If I go to the dermatologist for a minor skin issue and instead are seen by a PA, I will walk out immediately. 
 
My insurance premiums are high, and I chose many times to go out-of-the-plan to see the doctor, not the assistant. When I refer a patient for a vascular consult, I tell them to see the specific physician and expect a report from him, NOT from the PA. Otherwise, no more referrals. It happened to me once with a very busy dermatologist. When I called him regarding my patient seen by a PA and not him, he immediately apologized and promised that it would never happen again. Once more, any podiatrist who considers himself a surgeon only, please send the conservative care to this "second-class" podiatrist.

Well said Burton! As the grandson of the late Morris L. Selby, DPM, and the father of a "pre-podiatry" college student, I understand the history of podiatry and respect it. We are the medical and surgical specialists of the foot and ankle. I have been a residency director and interact with students and residents all the time. I have been asked to help out with the Alumni association of my school (TUSPM), of which my grandfather was one of the founders. I guess, I am following in his footsteps, and hope my son will soon too.  
 
There has been a transition in our field, but I feel very strongly about the need for most, if not all of us, to provide "at-risk foot care (RFC)" for our patients. I always tell my students that RFC helps pay many of your bills and, more importantly, it is a great service to our patients. Many podiatrists tell me they just don't get many of those patients. I tell them that is because they do not promote and educate what they do to their medical community. It is also due to the lack of actual hands on, at-risk foot care training.

Dr. Bijak correctly notes that the use of medication describes a drug-induced, purposeful anti-coagulation, not a true defect. Even so, the Florida LCD for Routine Foot Care states: “The following diagnosis related to anti-coagulation therapy does not require a Q modifier: 286.9* | OTHER AND UNSPECIFIED COAGULATION DEFECTS.” 
 
I still find it best to have a signed statement from the treating MD or DO that says this is the correct diagnosis for this patient.
