PODIATRISTS IN THE NEWS
Interest in Aesthetic Foot Surgery Growing: NY Podiatrist

Though statistics on purely cosmetic toe-shortenings are hard to nail down (it's often performed in conjunction with non-cosmetic surgeries, such as bunion removal), podiatrists agree it's growing in popularity. "Over the last two years, there has been more interest in aesthetic foot surgery overall," says Dr. Neal Blitz, a Park Avenue aesthetic and reconstructive foot surgeon operating out of Mount Sinai hospital, and the creator of the Bunionplasty (plastic surgery for bunions).

Dr. Neal Blitz

"A lot of women who've already had more standard procedures - say rhinoplasty, or Botox - want to see that same improvement in the appearance of their feet. "And of course some women just love shoes; surgeries like this allow them to wear styles they love without experiencing pain or feeling self-conscious," says Blitz.

INTERNATIONAL PODIATRISTS IN THE NEWS
Canadian Podiatrist Discusses the Biomechanics of Bunion Formation

Ignoring foot pain is the normal human response, Toronto podiatrist Dr. Hartley Miltchin admits, but not the right one. In fact, it’s akin to ignoring a mysterious rattle under the hood of your car. The trouble with foot pain — and engine rattles — is that they seldom go away on their own and usually indicate that something is very wrong and only likely to get worse.

Dr. Hartley Miltchin

“Most people think bunions are bumps that grow on the sides of feet, but what you’re seeing is actually the head of the bone — it’s not something growing, but bone moving,” Miltchin said. “If you overpronate, when your heel hits the ground, your foot rolls toward the big toe as you propel yourself forward.” This is tough on the toe because, unlike straight walkers, overpronators hit their big toes harder every time they walk. “It puts extra stress on the toe joint and pushes it out to the side, so that the bones and joints become misaligned, causing a bunion.”

PODIATRISTS AND OBAMACARE
Obamacare and ICD-10 Force TX Podiatrist to Close Practice and Retire Early

A Marble Falls podiatrist who recently retired says Obamacare-related requirements for healthcare professionals prompted him to close his doors late last year. "[The reform] is a massive change and it’s affecting all of medicine. I’m a small office and meeting the new requirements would have overwhelmed me,” said Dr. Perry Peterson, who retired in December 2013.

"That entered into [my decision] and there are other changes that under the Affordable Care Act that will go into effect. It didn’t make sense since I was nearing retirement. One of the changes scheduled to take place Oct. 1 has been in the works for years, and will require healthcare professionals across specialties to log more details about the care and treatment they administer to patients. "With new sections of data to fill in, it’s the accumulation,” Peterson said. "Even though they are done quickly, they just keep adding up.”

RESPONSES/COMMENTS (CLINICAL) - PART 1A
From: Brian W. Fullem, DPM

Dr. Amol Saxena and I published an article in the American Journal of Sports Medicine in 2004. Treatment Plan Summary:

In the study, all patients returned to activity after 4 – 26 weeks. 14 of 18 patients returned to activity using custom orthotics to protect the plantar fascia. Mean time to return to activity was 9 weeks.

Week 1–3: Non-weight-bearing
Week 3:     Cast for custom orthotics
Week 3–6: Weight-bearing in boot and begin physical therapy
Week 6:     Begin weight-bearing in shoe with custom orthotic. This orthotic should be a “total contact orthotic” in order to best relieve tension on the plantar fascia.

Saxena A, Fullem B. Am J Sports Med. 2004 Apr-May;32(3):662-5. Plantar fascia ruptures in athletes.

RESPONSES/COMMENTS (CLINICAL) - PART 1B
From: Eric Edelman, DPM

Plantar fascial tears respond well to immobilization. I have found the patients are most comfortable in a short leg cast for 3-4 weeks with crutches, and then another 3-4 weeks in a CAM-walker boot with partial weight-bearing.

When the patients look at me with disbelief that this injury requires "so much care", I explain that the torn part is on the bottom of their foot, and every time they put their foot on the ground, they will aggravate it.

RESPONSES/COMMENTS (OBITUARIES)
RE: The Passing of Donald Arenson, DPM

Dr. Arenson had a very positive influence on me from my earliest days as a podiatric medical student in Chicago. He was bright, articulate, and an outstanding professional role model and always generous with his time, teaching students excellence in podiatric medicine and surgery.
He was a true credit to our profession and will most certainly be missed by his patients and colleagues. My condolences to the Arenson family.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
From: Michael Forman, DPM

I again agree with Dr. Simon Young - this time in reference to "routine" and "at-risk" foot care. This service is in the domain of the podiatrist and that is where it should stay. I can accept healthy patients seeing a pedicurist if they choose, however all "at risk" patients must be seen in a podiatry office.

It is wonderful that so many of our colleagues are so busy that they don't want to bother with this type of patient. This is the patient who needs us the most. I have several podiatric colleagues who...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B
From: Burton J. Katzen, DPM

I always get a kick out of our profession’s younger generation when they write in about getting rid of routine foot care patients. First  of all, as a matter of perspective, my career in podiatry ranges from 1967, the end of the chiropody era when chiropodists took weekend courses at PCPM to get their DPM degree, to the present era of external fixators, total ankle replacements, etc.

In other words, I have been there from the beginnings of the modern era. My post-graduate training consisted of arguably the finest surgical residency in the...

RESPONSES/COMMENTS (CODINGLINE CORNER)
From: Robert Bijak, DPM

The code you are trying to use specifies "coagulation defects." This refers to genetic defects like Von Willebrands, Hageman's deficiency, Christmas factor, chronic hypofibrinogemia, and all the intrinsic and extrinsic coagulation factors from Factor 1 (fibrinogen) to Factor 13 (the L-L factor). What you describe is a drug-induced, purposeful anti-coagulation, not a true defect.
