AZ Podiatrist Uses RFA for Recalcitrant Neuromas and Plantar Fasciitis
 
Plantar fasciitis and neuromas can be debilitating conditions, causing persistent heel pain and discomfort for those affected. Board-certified podiatrist Alan Shih, DPM at Head to Toe Healthcare, PLC in Southern Arizona advocates for alternative approaches when conservative treatments and surgery may not be the optimal solutions. While surgical interventions are often the go-to option for recalcitrant neuromas and plantar fasciitis, Dr. Shih introduces Radiofrequency Ablation (RFA) as a viable alternative.

Podiatrists Discuss Achilles Reconstruction vs. Casting
 
Missy Journot, DPM, Richard Blake, DPM and Ben Pearl, DPM recently discussed the Achilles tendon repair during a virtual biomechanics forum. Dr. Journot was asked her opinion of an ACFAS consensus panel that concluded that there were no significant differences in long-term outcomes for Achilles ruptures with surgery vs. conservative management. Dr. Blake noted that he managed his Achilles tears and ruptures with casting.

Dr. Journot presents the pros and cons of both options. She prefers surgery in most cases because of the quicker recovery. High level athletes and significant heel varus can also tip the decision to surgery.

Canadian Podiatrists Warn of Frostbite in Poor Housing Areas
 
A couple of podiatric surgeons in Calgary are sounding the alarm about a concerning change at the hospital they say is spurred by the city's housing crisis. Francois Harton, DPM and Karim Manji, DPM specialize in treating diabetes-related foot conditions with the Zivot Limb Preservation Centre at Peter Lougheed Centre in northeast Calgary. Their main goal is to prevent limb amputations.

But when extreme cold hits Alberta, they're also among those at the forefront of frostbite treatment. Lately, Drs. Harton and Manji have seen more Calgarians who are living in poor housing conditions come in with frostbite — sometimes acquired while inside their homes. "I think there's a stigma attached [that] it's only a certain type of people that gets frostbite in society, and it's not," said Dr. Harton. "Unfortunately, we're seeing a greater shift now on people who just live in sub-optimal conditions."

Hal Ornstein, DPM won a silver medal at a recent Minor League Pickleball (MLP) tournament at the Atlantic Club in New Jersey. “Pickleball has been a life changer, reaping its many benefits of a good workout, social connections, and cognitive challenges," says Dr. Ornstein. MLP was founded to provide players a safe and fun environment to compete locally against other pickleball enthusiasts in a structured format beyond usual tournament play.

While AI undoubtedly will help clinicians become more efficient and improve patient outcomes, the sector differs from other fields in these important ways.
 
2.  The "Quintuple Aim"
 
Although some companies truly are committed to social equity, corporations have a fiduciary responsibility to maximize returns for their shareholders. This bottom-line focus stands in contrast to healthcare, where the idea of a “Triple Aim” – incorporating the patient experience, population health, and costs – has been widely accepted for many years. More recently, this has expanded to a “Quintuple Aim,” incorporating staff experience and health equity. 
 
The emphasis on equity, in particular, sets the goals of healthcare organizations apart from many businesses in other sectors. While corporations will often shutter stores in areas that have become unprofitable, healthcare has an ethical obligation to try to bring high-quality care to all populations, including in underserved locations. The AI tools adopted by the sector will reflect this emphasis.

Yes, this happens all the time. It’s just accounting to them, debits and credits. They owe you $200 for patient X, and according to them, you owe them $70 back on patient Y. Net $130, that’s what they send you and from their point over view, you’re happy. All the arguing in the world isn’t going to get them to budge. You can put in for a review for patient Y, but don’t get your hopes up. Since you’re non-participating, bill the patient for the recovered payment.  
 
Wait until more claims are AI evaluated vs. the current hard algorithms; I predict scenarios like this will be more frequent.

I was interested in yesterday’s post which indicated the use of a robot in foot surgery by a young podiatrist who claims to have been trained in this technique. Many of my general surgery and OB/GYN friends use a robot daily but when I searched for the use of a robot in foot surgery there, was not a lot there. I was wondering, are there any residency programs teaching this technique?
 
Our multispecialty center looked into buying a robot—they are expensive! Is this just another minimal incision technique? I would love to learn more about it and I am sure a large number of PM News readers would be interested.

I can see from a superficial point of view how you could equate and justify your remarks made in your post concerning ethical conversion. Dr. Jacobs comments about the effectiveness of the accepted standard vs. the new PSSD device really is the point here. The standard “old” exam that is free vs. one that costs the patient $300 or so is the point. If you explained to the patient that the old technique was 87% effective as opined by Dr. Jacobs and was free, and then offered them the $300 test that they would have to pay for, then fair enough. Let the patient decide. I would bet very few patients, if they were informed properly, would opt for the $300 test knowing that there is an 87% chance that the free test would work.
 
Again, I inform patients of what problems they have, and they make up their own minds about what service THEY choose. I think you can understand the difference now. It is like telling every patient they need surgery and failing to try or recommend conservative care. If patients have NO option, they will tend to decide based on what the doctor tells them. We are ethically required to explain ALL options and let the patient make THEIR decision.
