TX Podiatrist Helps Prepare Runners for Austin Marathon
 
Sally Tanner pushed herself in training to reach her goal of running the Austin Marathon, and it took a toll on her feet. She developed plantar fasciitis. "It's kind of hard to run if you can't feel your feet," said Tanner. Her podiatrist, Dr. Ana Urukalo, fitted her for arch supports. "We have been very busy," said Dr. Urukalo. "Typically this time of year in Austin with the marathon and the half marathon ... the last 6 months have been quite busy."

Dr. Urukalo says she treats a lot of marathon runners for plantar fasciitis or stone bruises caused by fallen arches. The key is giving your feet support. "Number one: start with a good pair of shoes," she says.

INTERNATIONAL PODIATRISTS IN THE NEWS
UK Podiatry Students Tend to The Homeless
 
The homeless were given foot care from a podiatry lecturer and students from New College Durham, attending to 40 people at the Crisis Centre in Newcastle. They took fresh socks, creams and medical supplies, which were donated by Robinson Healthcare and DLT Podiatry, and delivered treatments including bathing feet, general foot and nail care, and patient-specific foot health advice and treatment. The homeless people were given new socks.

Podiatry lecturer Barbara Chambers, who also volunteered on Christmas Day, said: "Helping out with the Crisis team is not only a very rewarding thing to do but it is also very humbling. It makes you appreciate how much we all have in our own lives. The Crisis 'guests' we saw were so grateful for the very basic commodities in life such as a bowl of warm soapy water to soak their feet in and a clean pair of socks."

QUERIES (CLINICAL)
Query: Partial Plantar Facial Tears
 
Does anyone have any pearls for treating partial plantar facial tears? These patients are usually in agony with high levels of pain. I have dabbled in PRP and amniotic injections, but I have concluded that results are mixed. I have referred patients to colleagues for radio frequency ablation, also with mixed results. Is there someone with a treatment protocol with more consistent results?

Does Xarelto meet the Medicare criteria for the ICD-9 286.9 (other and unspecified coagulation defects) diagnosis for qualifying routine foot care?

Response: Xarelto (rivaroxaban) is classified as an oral anticoagulant. Additionally, the pharmaceutical website states the following: “Xarelto is the first and only novel oral anticoagulant …” 
Coumadin (warfarin) also is classified as an anticoagulant. I believe one could argue that any policy covering Coumadin should also cover Xarelto (Rivaroxaban). 
 
It is my opinion that if a Medicare carrier approves treatments for routine foot care based upon diagnosis code ICD-9 286.9 (other and unspecified coagulation defects), Xarelto should meet the criteria.

$4.000 is normal for a "refurbished" head and $7,000 is normal for a new one. In my opinion, this is the main problem with this laser.  After so many uses (or pulses fired from the head of the unit), the head burns out and it will need to be replaced.

Perhaps if the schools trained students better in routine foot care, they would become more proficient, and it would take them less time to render more effective treatment.

I think we are missing the point when we are discussing RFC. As I see it, there is no real entity called RFC. We are examining the feet of at-risk patients. Although the end point is nail care and hyperkeratotic care on an at-risk patient, the main concept should be evaluation and management of the at-risk patient. Examination in RFC involves vascular and neurovascular status, dermatologic exam, orthopedic exam, etc. Unfortunately, the patient and everyone else doesn't perceive or understand what is involved in nail care. The same concept exists for care of hyperkeratotic lesions, which in addition to causing foot pain and ulcerations in the at-risk patient, also contributes to falls. Literature I've seen links hyperkeratotic lesions to gait disturbances, and falls. With all this emphasis on fall prevention, shouldn't this be part of the equation.
 
We think too little of the value of the services we provide, yet our services are valuable and important. I strongly feel podiatric services and foot care have finally come to the forefront of many specialists now (endocrinologists, vascular, rheumatology, orthopedics, primary care). This has resulted in a reduction in extremity amputations, and podiatric care is leading the way. Forget about "RFC" and consider it "chronic necessary foot care to the at-risk patient," which has and is resulting in decreased mortality, morbidity, and costs.
