PODIATRISTS AND FOOTWEAR
NY Podiatrist Provides Tips on Selecting Walking Shoes

To find reliable shoes that can bring fun and comfort back to your adventures, Bustle asked a podiatrist just what to look for in a shoe. Bruce Pinker, DPM, a board-certified podiatrist and foot and ankle surgeon at Progressive Foot Care, recommends you choose a shoe with a rubber outsole and make sure that it has “adequate traction for proper gripping of the walking surface. Choose a midsole made from ethylene-vinyl acetate (EVA),” suggests Dr. Pinker. This material “provides shock absorption and cushioning, which helps to make for enjoyable walking and ... to prevent injury. [It is] highly recommended for the midsole.”

Dr. Bruce Pinker

Giving plenty of thought to the insole is important, especially if you have custom or favorite inserts you like. “Some insoles are removable,” says Pinker. “[This] is preferred, as it allows for the placement of a custom-made orthotic (if necessary).” When it comes to the toe box, “mesh material is preferred to allow for the exchange of air to prevent increased temperatures.” Not only does that air exchange make for happier and more comfortable feet but “if the foot becomes very warm and sweaty,” he says, “tinea pedis, onychomycosis, and verrucae may develop.”

PRACTICE MANAGEMENT TIP OF THE DAY
2023 Asset Protection Resolutions

We start 2023 with some asset protection and risk management resolutions for physicians and medical practice owners that will help make your success more predictable.

5.  Secure personal electronics like laptops, smartphones, smartwatches and iPads with passwords, biometrics, and location services (like “find my iphone”). They should not be unlocked and accessible to others. Millions of Americans get new devices over the holidays due to upgrades in technology and financial incentives. Both personal and business usage of these devices has greatly increased due to remote working scenarios and the fact that trade-in programs are more common than ever, especially for Apple users.

Given the amount of sensitive personal and financial data they contain, it’s important you and all your family members (are you sharing any accounts?) think and act carefully about security and properly deleting your information before trading them in, gifting them, or otherwise disposing of them.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: CMS Audits for Routine Foot Care
From: Lawrence Kobak, DPM, JD and James Tudor, XPX, PCA

Lately, we have been involved with a spate of CMS audits involving routine foot care. The chief problem the government auditors still find is a lack of proper documentation.

First, there is sometimes a failure to sign the chart note in a timely fashion. Secondly, the services must be medically necessary. When not diagnosing onychomycosis, the necessity is evidenced by referencing class findings. This is often misunderstood. If you are using two Class B findings, at least one of them must be for an absence of either the DP or PT pulses in the foot or feet being treated.

The other Class B finding may consist of any three of the...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
From: Randy Anderson, DPM

We have had similar conversations at one of the hospitals where I am involved in leadership. How we address the issue of multiple certifying bodies is to have the applicant provide proof that the alternate certifying body is as rigorous as the traditionally accepted organization: examples such as ABFAS for podiatric surgery, AHA for ACLS certification, etc. This approach avoids charges of prejudice or bias against any specific provider or certifying body.

This issue is also handled at the credential committee level as part of the credentialing process.

RESPONSES/COMMENTS (PM NEWS QUICK POLLS)
From: Robert Kornfeld, DPM

Dr. Hofacker wonders what the MDs and DOs feel about practice. In fact, a 2019 Medscape poll of 15,069 MDs and DOs revealed that nearly half reported "burnout" with 11% suffering colloquial depression. Urology topped the heap at 54%. The reasons given were having to see too many patients and the redundant and maddening paperwork required by insurance companies. The lowest burnout rate came from solo practitioners at 41% burnout and up to 50% for healthcare organizations. Only 26% said they would recommend medicine as a profession to their children. I was unable to find a survey of chiropractors but perhaps one exists somewhere.
