PODIATRISTS IN THE COMMUNITY
MI Podiatrist Volunteers at CBC Church Day of Hope

Tom Biernacki, DPM of Advanced Foot & Ankle Specialists provided foot care evaluation and foot screenings at the CBC Church Day of Hope. Foot and ankle problems can be especially prevalent in those in financial distress with both diabetes, poor overall health, and limited access to resources.

Dr. Tomasz Biernacki

Dr. Biernacki is a podiatrist and foot doctor at Advanced Foot and Ankle Specialists in Howell, Dexter, and Brighton Michigan. He spent a full day working with other primary care doctors and nurses at community Bible Church in Brighton. The medical team was able to see hundreds of patients providing foot care screenings, flu shots, diabetic screening, and other important health services.

RELEVANT RESEARCH
The Efficacy and Safety of Pulse vs. Continuous Therapy for Onychomycosis

This systematic review and network meta-analysis showed that continuous daily and pulse regimens of terbinafine and itraconazole were similar in mycological cure. Continuous terbinafine 250 mg daily for 12 weeks was not significantly different from continuous terbinafine 250 mg for 16 and 24 weeks, pulsed terbinafine 500 mg, weekly fluconazole, or pulsed and continuous itraconazole regimens in mycological cure. Continuous daily terbinafine 250 mg for 24 weeks performed best in rank probabilities over other therapies, but the clinical significance of this is debated.

Continuous and pulsed regimens of terbinafine and itraconazole for onychomycosis had similar efficacies and adverse events. Whether dosing of daily terbinafine 250 mg for longer than the FDA-approved 12 weeks yields higher efficacy could be investigated in larger studies.

PRACTICE MANAGEMENT TIP OF THE DAY

6 Ways Your Practice’s Website is Hurting Your Bottom Line - Part 2

Here are some of the most common website mistakes we see practices make, and how your practice can avoid them:

2. Vague or cliched copy. It's important to craft your unique value proposition (or what sets your practice apart from the competition). This value proposition should be highlighted throughout your website copy. If you’re not sure how to get started with crafting the copy, consider this question: If you only had three sentences to explain the type of care you provide and why it’s better than your competitors, what would you say? Take time to form your response, then run it by your colleagues and others you trust. If they affirm that your response is credible and compelling, begin to use it as your North Star when modifying your copy.

QUERIES (CLINICAL)
Query: Oral Med Replacement for Famotidine

In 30+ years of practice, I have yet to find a great treatment for warts. However, I have had some success with mosaic type warts by using topical acids along with oral Tagamet 400 mg tid. When Tagamet became more and more difficult to obtain, I began using Pepsid 40 mg once daily at bedtime. Today, I found out that Pepsid (famotidine) is on recall. Are there any other oral meds out there that you all are using to battle this condition?

RESPONSES/COMMENTS (CLINICAL)
From: Clifford Wolf, DPM

“The historic rationale for choosing to do a Lapidus included those patients with flexible or "hypermobile" first ray, foot collapse with medial displacement at the 1st metatarsal-cuneiform joint, or arthritic first metatarsal-cuneiform joint, in conjunction with increased 1st-2nd metatarsal angles (high IM angle). More recently, it appears to be the procedure of choice for many foot surgeons who have expanded the criterion to include those patients who in the past would have received a more distal procedure such as an Austin, or a Scarf (or variations on an Austin or Scarf)."

And why do you think this is? "Metatarsal frontal-plane rotation pronation of the 1st metatarsal rotation matters. While hallux valgus has traditionally been viewed as a transverse plane deformity, research indicates that it is actually a three-plane deformity with 87% of patients having abnormal frontal-plane rotation (i.e., pronation) of the 1st metatarsal.1 Correction of metatarsal rotation is critical for restoration of anatomic first ray alignment (MTP joint congruity and sesamoid position) which, if left unaddressed, is linked to high rates of radiographic recurrence.2,3 Due to the ability to address the triplanar deformity at its origin, the 1st tarsometarsal (TMT) joint provides the optimal surgical site for true anatomic restoration."

3. Okuda R, Kinoshita M, Yasuda T, et al. J Bone Joint Surg Am. 2009, 91:1637-4

The Lapidus bunionectomy affords the surgeon an opportunity to correct the deformity in all three planes in an evidence-based reproducible manner.

RESPONSES/COMMENTS (OBITUARIES) PART 1A

Life won’t be the same without my brother from another mother...after over 60+ close years together, he'll be SO missed. But I know we (my extended Fischman family) will all smile thinking about all our great and fun(ny) times together.

(L-R) Dr. Arthur Gudeon, Dorothy Ann, and Dr. Edward Fischman

Ed was a true podiatric icon, involved and a leader in every aspect and receiving accolades in every important organization in our profession, and all will miss his intelligence and humor. Loads of love and hugs to all the Fischmans, and finally, Eddie, Rest In Peace my brother.

RESPONSES/COMMENTS (OBITUARIES) - PART 1B
From: Arthur E. Helfand, DPM, David M Davidson, DPM

A great loss for his family and the profession. A true gentleman and man of integrity. May he rest in peace.

Arthur E. Helfand, DPM

I heard the news of Ed's passing with mixed feelings. He had a very difficult last few months and finally he is at peace. Ed was responsible for elevating our profession to new heights. Without his efforts, it would have been decades before we were granted hospital privileges and equality with other members of the medical community. I will always remember the many good times we had together. I will miss his humor, his smile, his kindness, and his graciousness. Rest in peace, my friend.
