PODIATRISTS IN THE COMMUNITY- PART 1
NY Podiatrist Featured at Holocaust Remembrance Event

When Rabbi Aliza Erber, DPM is asked what her first language is, her response is “mute.” Dr. Erber’s statement invokes her harrowing history — as a baby she had her mouth taped shut while hiding from the Nazis in a forest in the Netherlands. But Erber, 80, had a voice on International Holocaust Remembrance Day.

Gillian Lamb and Dr. Aliza Erber (photo: Eugene Gologurksky)

A portrait of the rabbinic pastor and retired podiatrist’s face was projected onto the base of the Brooklyn Bridge on Saturday night, along with her words, “It was not okay then, it’s not okay now.” The image of Dr. Erber was part of the launch of Live2Tell, a project started as a result of the rise in anti-Semitism in the U.S. and globally in the wake of the Oct. 7 terrorist attack in Israel.

PODIATRISTS IN THE COMMUNITY- PART 2
NJ Podiatrist Enjoys the Diversity of Podiatry

When she worked as a physical therapy aide earlier in her career, Sara Mateen, DPM, saw a variety of foot and ankle injuries. Having played many different sports, she was interested in how the body—especially the lower extremities—cope with the rigors of athletics, so she shadowed a few podiatrists when she could. She knew that career was ideal for her. She liked the diversity of podiatry, whether practicing sports medicine, treating diabetes, doing fracture care or elective reconstructive surgery.

Dr. Sara Mateen

Today, Dr. Mateen joyfully welcomes those challenges in her new role as a podiatrist and foot and ankle surgeon at Palisades Medical Center. Dr. Mateen is excited about the evolving future of podiatry. “The specialty is going more towards robotic-focused procedures,” she says. “I was trained in minimally invasive techniques, but not every case is suited for those. Advancements in technology, such as orthopedic implants, have great promise, and I’d like to be part of an innovative product design in the future.”

PRACTICE MANAGEMENT TIP OF THE DAY
5 Ways AI is Different in Healthcare

While AI undoubtedly will help clinicians become more efficient and improve patient outcomes, the sector differs from other fields in these important ways.

1.  "Good Enough" is Not Good Enough

In some fields, AI tools are going to perform slightly worse than humans, at least at first, and this is going to be okay. People are already using AI to draft response emails, for instance, and it’s not the end of the world if these tools fail to capture your tone and voice with perfect accuracy when confirming a video meeting. Basically, if an AI tool’s performance is only 90% as good as a human’s, but it makes a process significantly faster or simpler, that tradeoff will work for many people, workflows, and industries.

But this tradeoff won’t work in health care. Patients’ lives are on the line when clinicians change the way they deliver care, and providers simply won’t use AI tools that force them to compromise on quality—no matter how efficient those tools may be.

QUERIES (MEDICAL/LEGAL)
Query: Insurance Carrier Withholding

In the case of a non-participating provider, can a carrier withhold reimbursement which it claims represents an overpayment from one patient from the reimbursement that it legitimately owes for another patient?

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
From: Kristin Happel

I am wondering if Dr. Wolf and his colleagues are possibly misinterpreting what their CBRs say, and what the purpose of a competitive billing report (CBR) is. I bill for a mobile podiatry practice in PA, and they also recently received a CBR from their MAC concerning the amount of hospice patients they are treating. Nowhere in the CBR did it say that there would be limits placed on anything, including reimbursement. I explained to the doctors how CBRs are generated and what they mean, and that it was not surprising they received one, due to the sheer amount of patients they've seen on hospice compared to other practices in PA.

But under no circumstances were they doing anything wrong, and they should continue billing as...

Editor's note: Ms. Happel's extended-length letter can be read here.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2 A
From: James Wilton, DPM

I read with some interest Dr. Allen Jacobs’ comments regarding "AENS surgeons operating on diabetic peripheral neuropathy with nerve decompressions". As a member of that society and director of the basic peripheral nerve surgery course, this is as far from the truth as can be stated. The surgeons that take our course for training have a much broader background in diagnosing, and conservatively and also interventionally treating peripheral nerve disorders. We do not advocate on any level the use of PSSD testing for the evaluation for peripheral nerve pathologies. We specialize in developing skills for our surgeon students in giving a complete neurologic extremity "hands-on" examination.

As the first DPM surgeon admitted to the American Society for Peripheral Nerve, it has been eye-opening seeing the difference in training between allopathic peripheral nerve surgeons and what is being taught in traditional podiatric residencies. I had excellent medical and surgical training through Dr. James Ganley, however peripheral nerve injuries and syndromes were not a part of my training. In having taught podiatric surgeons and international plastic surgeons over the past 20 years peripheral nerve surgery techniques and also diagnostic evaluation of patients, the current podiatric residency model falls way short of the allopathic model for plastic surgeons in these fields. It is through advanced training that the AENS offers, that podiatric physicians can become better diagnosticians and surgeons

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2 B
From: Cosimo Ricciardi, DPM

I read with curiosity Dr. Ivar Roth’s comments on “practicing ethically” and not “chasing a dollar”.

Perhaps he could expound on his previous post on his ethical conversion of a patient’s $125 cash nail care office visit to a $3,525 cash office visit.
