TX Podiatrist Closing Practice After Nearly 40 Years
After 39 years of treating people’s feet, Gerald McCool, DPM is closing his podiatric medical practice. “My plans are to take some time off,” said Dr. McCool, 65, who sees more than 30 patients daily and takes care of many ailments, including foot pain, bunions, and athlete’s foot.

Working as a medical specialist, McCool said, has changed since he started his private practice in 1986. The challenges have included declining insurance reimbursements and rising medical supply costs. Some supplies have also been in short supply since the pandemic. “Reimbursements are now less than they used to be, so we must increase the volume of patients in order to make the same amount of money as previously,” McCool said.

Focus on Six Key Areas for Successful Physician Partnerships
Physician partnerships require much pre-planning, careful structuring, and a whole lot of continued communication in order for them to be successful. Here are the six areas I recommend examining in great detail when undertaking the process of beginning and nurturing physician partnerships.
1. Consider structure. One of the first areas to consider is the practice structure; both how it is currently configured as well as potential future arrangements. How many locations are involved? What types of treatment services are offered at each location? Is the equipment leased, owned outright, or being paid for? Are the buildings owned or rented? Who are the current owners? Is this a family-owned practice (any nepotism)? Are there any joint ventures with hospitals? Are there any medical directorships with other facilities?

Query: CPT 28750 Billed with CPT 28285
When I look up CPT 28750 and CPT 28285 under the CCI edits tab to determine the column 1 and 2 for these two codes, the response shows it both ways: CPT 28285 in column 1 and CPT 28750 in column 2; and also CPT 28750 in column 1 and CPT 28285 in column 2.
I am not sure what this means and which code to add the -59 modifier to..

Response: The reason the CCI edits exist is that there is the possibility that the great toe is also a hammertoe. In this scenario, obviously only one procedure can be performed. However, if you are performing an arthrodesis of the great toe and a hammertoe correction on a different toe, then you can bypass the edit by appending either a modifier -59 or -XS.
In this scenario provided, I ran the two codes back and forth and when you compare CPT 28750 to CPT 28285, the column 2 code is CPT 28750; and when you switch by putting in CPT 28285 to CPT 28750 the column 2 code is CPT 28750. The only code needing a -59 or -XS is CPT 28750. I am not sure where you compared these two but on the https://www.apmacodingrc.org/cciedits.asp, it provided me with the information I just provided. I understand this is not what you are seeing. I'm not sure what you used as a tool to get your information. This is the correct way to proceed.

RE: Sequestration - Reduction in Federal Payment from Medicare Advantage Plans

I noticed on a resent ERA from Humana Medicare Advantage that there was a 2% reduction in the allowed amount due to the sequestration, remark code CO253. I was under the impression that only traditional Medicare was allowed to reduce our payments due to the sequestration.

Dr. Ribotsky brings up an interesting point. I think the onus of this decision relies on the governing credentialing board that approves the CME credits for a particular lecture. Content, validity, and the value of lecture information are far more important to me than past indiscretions of the lecturer. Obviously this must be done on a case-by-case basis. How interesting would it be to listen to a lecture given by a person who was convicted of billing and coding fraud, not for the how-to content but rather the how-not-to information provided.
Maybe this is a bad analogy, but if you Google Frank Abagnale (Catch Me If You Can), he gives lectures. Here is a man who was incarcerated in three different countries and is now a very successful businessman. He is an excellent speaker and most of you will find his lectures fascinating. The real crime would be if people like Abagnale were not allowed to lecture based on past mistakes.
To answer another question Dr. Ribotsky brought up in his query, I'm not sure of the importance about having to disclose if a lecturer's license has been revoked. The ironic side to that is it probably would bring more public interest to that lecture series if the status of the lecturer's license status was revealed.

Thanks, Rem, but no thanks. I am 100% against the continued dumbing-down of what we do. Rem, you could write 10,000 words that would not be as effective as my profession referring to ourselves as podiatric physicians and surgeons. Do you realize how many people's grandmothers saw a chiropodist/podiatrist? I bet most Americans don't know how far beyond that my profession has come.
I figured this out a long time ago. My department at Humboldt Park Health (formerly Norwegian American Hospital) is the Department of Podiatric Medicine and Surgery. We have had a residency program since 1988.

Regarding DPMs who don’t call themselves podiatrists: We have a great podiatrist in the House of Representatives who calls himself a “physician” in everything that I have read about him. Please correct me if I am wrong but, it seems that a movement to properly identify ourselves should begin at the top. If those who we respect cannot do it, then don’t expect much from the rest of us.
I have always identified as a podiatrist here in Israel where they couldn’t believe that a non-MD could perform surgery of any kind. It was the only way to establish the profession in a new country. It is not expecting too much for podiatrists in America to properly identify themselves where most people already know about the profession.
