IL Podiatrist Discusses Running Shoes Versus Cross-Trainers

Go with running shoes if you mostly jog or walk, since they’re engineered for heel-to-toe motion. Why not walking shoes? “Running sneakers cater to a wider range of foot types and are built to last longer,” says Megan Leahy, a doctor of podiatric medicine at the Illinois Bone & Joint Institute, in Chicago.

Dr. Megan Leahy

Go with cross-trainers if your routine includes an activity like aerobics, weight training, or kickboxing (basically any exercise on a hard surface that involves side-to-side movement).

AT THE COLLEGES

Two New Clubs Formed at NYCPM

Among the myriad of clubs at NYCPM, two more clubs have been recently organized: the Asian American Podiatric Student Association and the Podiatric Radiology Club. The Asian American Podiatric Student Association (AAPSA) is a student organization founded in 2011 to reflect the growing number of Asian-American students at NYCPM. Membership is open to all students, not exclusively to Asian American students. The primary focus of AAPSA is to educate and increase awareness of podiatric medicine among Asian-American communities.

The Podiatric Radiology Club is geared toward helping the student body become familiarized with reading radiographs, as well as being able to radiographically identify multiple pathologies. Drs. Randy Cohen and Jose Loor are the advisors to the club, and graciously volunteer their time in order to benefit the students by holding lectures regarding relevant podiatric radiology topics.

PODIATRISTS IN THE COMMUNITY

GA Podiatrist Helps His City Earn RRCA Runner Friendly Community Logo

The Road Runners Club of America has named Columbus a Runner Friendly Community. The honor comes with a plaque and royalty-free use of the RRCA Runner Friendly Community logo for five years, among other benefits. But its significance goes deeper than signage. "It makes me proud to see that the community has grown so much. I want to continue the growth of health and fitness," said Dr. Ed Lopez. The podiatrist is a past president of the Columbus Roadrunners and a current member of the organization's board of directors.

Dr. Edward Lopez

He spent an estimated 80 hours working on Columbus' application to become a Runner Friendly Community. His application included a video, letters of community support, and more. Lopez remembers seven years ago, when the Columbus Roadrunners had approximately 50 members. Now, the group encompasses nearly 700 people, he said.

E-HEALTH NEWS

Proposed Meaningful Use Stage 3 Criticized as Hasty and Too Strict

Criteria floated for the final stage of the federal electronic health record incentive program would be extremely difficult for physicians to meet, causing those using the systems at their practices to fall short of requirements and exposing them to lower Medicare payments, organized medicine groups say. The earliest that physicians will see stage 3 is 2016. Stage 2 won’t begin until 2014 for the earliest adopters, and physicians who have not yet adopted EHRs will spend two years under stage 1 criteria before they start progressing to the next stages.

A health information technology committee under the Dept. of Health and Human Services has drafted preliminary recommendations for stage 3 of the EHR meaningful use program. The committee wants the program to support new care models, apply broadly to specialties, and reflect technologies that are becoming available. But physician organizations, including the American Medical Association, are urging federal officials to review and improve the first two stages of paperless record standards before jumping ahead too hastily.

QUERIES (DME)

Query: Diabetic Therapeutic Shoe Bill

With tougher DMERC regulations and increased annual fees, I am wondering if it's still a good idea to participate in the diabetic shoe program in my office. I have been referring my patients out for their diabetic shoes. Is it too late to start participating now?

RE: Pediatric Digital Deformity (Edmond F. Mertzenich, DPM, MBA)
From: Dennis Shavelson, DPM

Why wait until this youngster needs foot surgery. Functional foot type this child (he is probably the double flex type) and then apply foot centering pads as a test drive for improved motor function and posing of the patient’s feet. If the test drive is successful, cast for UCBL foot centering orthotics in the optimal functional position instead of STJ neutral and Rx foot type-specific.

Anecdotally, we have found that this type of device remodels feet in a positive progression rather than a negative one (via Wolf’s and Davis’ laws). This improves foot posture, muscle engine power, and performance over time. Every two years or so, this process is repeated with serial castings and Rxs similar to those for met adductus or internal tibial torsion corrective casting.

Muscle engine training is added when the child is old enough to be trained. This interventional biomechanical paradigm, starting with foot typing will, although unproven, presents a logical progression to try to reverse some of the inherited biomechanical pathology, deferring or delaying familial foot and postural problems from developing over time. I’m sure the parents will agree this is a better course than just waiting.

RE: Are Venous Thromboembolisms Preventable?
From: Michael M. Rosenblatt, DPM

It has long been assumed by plaintiffs’ attorneys that venous thromboembolism (VTE), a very dangerous complication of various highly invasive abdominal surgeries and those originating in the lower extremities are the fault of the operating surgeon. Certainly, they will be looking to find a deep pocket to pay for such complications.

But a recent study published on-line from the American College of Surgeons points to the real possibility that VTE is not actually preventable, despite surgeons' following correct and proper pre- and post-surgical prophylaxis. High on the list of causative factors are pre- and post-operative infections.

The study itself mainly dealt with abdominal surgeries, but since many VTEs can originate from a lower extremity, it is possible that the results of this study also apply to lower extremity surgery, although that is not specifically mentioned in the study. At the very least, podiatric surgeons should consider post-operative infections a pre-disposing factor for this serious complication.

Since this particular study did not propose any specific preventative measures, at the very least, podiatric surgeons should be aware of this complication when an infection occurs. This also begs the question of whether or not patients with pre-existing infections should be specifically informed of the concomitant risk of VTE in the case of an invasive procedure done to treat the infection.

RE: Changing Office Hours (Olga Luepschen, DPM)
From: Kathleen Neuhoff, DPM

Our practice sees patients from 7:00am to 6:00pm Monday, Tuesday, Thursday, Friday, and 7:00am to noon on Wednesday and 9:00 to noon on Sat. We have two DPMs, so we schedule one doc from 7:00am to 3:00pm and the other from 11:00am to 6:00pm. Times with highest patient demand are 7:00 to 9:00am and 3:30 to 6:00pm, so we do not schedule any "routine care" patients during these times. 11:30am to 1:30pm are also fairly busy with lunch hour patients. Wednesday is our slowest day (partly because our strappings are done twice a week). If we wanted a shorter work week, we would probably close on Wednesdays.

Fridays are our busiest and most productive days. They are our...
