APMWA Announces 38th Annual Student Writing Competition
The American Podiatric Medical Writers Association (APMWA) has announced its 38th Annual Student Writing Competition.

All papers MUST be non-technical in nature and original (not AI generated). Appropriate subjects include practice management, ethics, law, education, healthcare, or any topic that would be suitable for a lay publication.

There is no word limitation. Papers will be graded for content, style, grammar, neatness, and overall impact.

First prize one thousand dollars ($1,000), Second prize seven hundred and fifty dollars ($750) Third prize five hundred dollars ($500) sponsored by an APMA Educational Foundation endowment from Dr. and Mrs. Steven Berlin, and recognition in the APMA NEWS and the APMWA Newsletter. Honorable Mention Certificates may also be awarded.

OH Podiatrist Discusses How Feet Change with Aging
“The ligaments and tendons that support the arch in your foot become worn from use,” says Nicole Nicolosi, DPM, a podiatrist with the Cleveland Clinic. This wear leads to a gradual arch collapse, resulting in feet that may become longer or wider. Weight gain, common in older age, further exacerbates this change by adding pressure on these supporting structures. Additionally, the fat pads cushioning the feet thin out, altering shoe fit and reducing shock absorption, which might contribute to increased foot discomfort.

Dr. Nicolosi emphasizes the importance of regular shoe fittings to accommodate these changes: “It’s important to get fitted for new shoes every so often to make sure you have a proper fit.” Regular foot health checks are also important as you age, as they can reveal conditions like diabetes or circulatory issues.

Amol Saxena, DPM, MPH, Past President of the American Academy of Podiatric Sports Medicine, lectured this past week to the Orthopedic, Sports Medicine and Physical Therapy Departments at the request of Rajesh Rohilla, PhD, Head of Training & Placement and Dean of Alumni Affairs at the Delhi Technical University in North Delhi, India.

The lecture, titled "Track & Field Injuries Below the Knee", covered Dr. Saxena's research findings on Achilles and navicular injuries as well as shockwave indications. The orthopedic department requested him to return to help set up a foot and ankle clinic, and a potential fellowship.

I could not agree more with Dr. Jacobs. I smell the rat also when using this new diagnostic test for neuropathy at a great cost to the patient and little extra, if any, diagnostic value. Unfortunately, many DPMS will gravitate to questionable new diagnostics and treatments to make an extra buck at their patients’ expense.
Practicing ethically should be every professional’s goal, not chasing the dollar as so many of our colleagues are doing.

I was not responding to the value of the PSSD, the machine which was introduced so many years ago when we had so few examinations except for clinical vibratory, Semmes-Weinstein, and nerve conduction study/EMG. I no longer have the machine or use it. The question was billing and abuse with the Medicare system.
My response was it was never covered by Medicare. That should have been made clear at the time that the machine was used. Unfortunately, that was not the case with the person that I was responding to. I absolutely agree with everything Dr. Jacobs said. Many abuse the system, trying to charge for nerve testing. My response was not the value or lack of value of the PSSD machine, but the billing of what was done.

The point is that there is a clear bias against hospice patients receiving care as Medicare feels because they’re in hospice and at the end of life, they do not need the care. I agree that if one performs routine foot care for the chronic condition of PVD or diabetes, these diagnoses are likely listed on the hospice's list of diagnoses to make the patient eligible for hospice care. Thus, it might be difficult to justify a -GW modifier regarding procedure codes relating to routine foot care. Ulcer and wound care would be covered by hospice and likely not reimbursed by Medicare. If the -GW modifier is used, there might be a risk of an overpayment request if Medicare is billed for care that the hospice is supposed to provide.
The problem is when patients who are not likely to die are in the hospital. Some of them might be encouraged to go on hospice and lose their direct routine foot care benefits to be paid by Medicare to a private podiatrist, even if they are a diabetic with PVD.
