UK Podiatrist Discusses Onychogryphosis
 
Anything you do can affect nail health in the long-term; for example, style of shoe, involvement in a sport, or just nail neglect. “It’s as if the body’s protective mechanism against the trauma is going into overdrive,” explains consultant podiatrist Michael O’Neill, of the UK’s College of Podiatry.

This can make nails more difficult to cut, and one person in 50 will develop a condition called onychogryphosis – a ram’s horn toenail. “It’s not just unsightly, it is painful when pressing against the end of a shoe,” explains O’Neill.

Open Your Door and Heart to New Employees
 
Most human resource experts estimate that it costs one and a half times a staff member’s salary to replace him/her with a recruitment and training process. A worthy investment in time and expense is to make all employees feel very welcome and appreciated from the start in your office. Here are some simple ways to wow your new staff member:
  • Have the lead physician in the office call new employees before they start, welcoming them to the office and stressing how happy you are that they will be working in the office.
  • Hold a new employee luncheon the week they start.
  • Leave a small bouquet of flowers or chocolate and a welcome card signed by all the doctors and staff in the office.
  • Pair the new hire with an experienced staff member to act as a mentor.

QUERIES (CLINICAL)
Query: Inflammatory Reaction to MMI Staple
 
I'm wondering if anyone has had a patient react to an MMI staple. The patient does not have a nickel allergy. Chronic swelling and blistering have been occurring for the past 6 weeks over the area of the staple. This began one week after surgery. The rep suggested taping a similar staple to the patient's skin, which did not create a reaction. She has had "deep" sutures before without any problems. The staple has not moved on radiograph. It appears to be an inflammatory reaction. It improves while taking a Medrol dose pack/prednisone, but then soon returns. Any ideas?

Some suggestions I have are:
1) Attend different CME conferences than what you are presently attending to get some fresh ideas.
2) As new patients come in who need RFC, assess them differently: one cannot afford to spend 10 minutes debriding, grinding, and cutting one great toenail just to make a patient happy – since this is a body appendage with a nasty, incurable disease; at the very least, you should surgically excise the nail plate. 
3) You need to have a discussion with your office staff about scheduling a patient with heel pain as a priority. Your staff will always find it easier to keep scheduling RFC patients instead of new patients because new patients take more of their time and effort. 
4) Schedule your RFC every 90 days instead of every 63 days.
5) Limit the number of RFC patients that your office will schedule per day and per hour.
6) If a patient presents with a year’s worth of nail growth and is not a surgical candidate, dispense (sell them) a salicylic acid, urea-based product to soften and chemically debride the nails with instructions, and have them return in a couple of weeks.

Being busy is a good problem to have. If you can't make enough room in your day/week to accommodate RFC patients as well as new patients, maybe your practice has reached a saturation point and it's time to expand. In today's economic environment, I'm not sure it's wise to start eliminating entire groups of patients. You can never predict when that RFC patient may have a problem that will need surgical intervention, or a friend or relative will require your services for a non-RFC problem. 
 
You may want to consider hiring a part-time or full-time associate to pick up the load. You may be able to find a non-surgically trained doc or more mature doc who will be happy to provide RFC services in your practice.  It certainly seems to make more sense to expand your practice and accommodate the full spectrum of patient needs, rather than eliminating specific patients with a possible resultant decrease in income.
