Men with Diabetes Less Likely to Seek Medical Attention: TX Podiatrist
 
Diabetes is one of the most common diseases across San Antonio, and data from the Texas Department of State Health Services shows that amputations because of complications are higher in Bexar County than the rest of the state. "In Bexar County, in particular, it is very very prevalent," said Michael Sobolevsky, DPM, a podiatrist with University Health. "And I feel like it’s gotten even more prevalent as time has gone on.”

"Men are less likely to seek medical attention – there’s a little machismo," Dr. Sobolevsky said. "Men can be stubborn. A lot of our men are blue collar workers, they can’t take time off work."

CVPH Board Adds NY Podiatrist
 
Three new members of the University of Vermont Health Network – Champlain Valley Physicians Hospital (CVPH) Board of Trustees begin their three-year terms this month. Joining the 12-member board is Joseph Donnery, DPM of Plattsburgh Podiatry.

Dr. Donnery has practiced as a podiatrist for more than 35 years and previously served on the CVPH Board from 2013 to 2021 where he was an active member on a number of committees including Quality, Physician Recruitment, Building and Planning, and the CVPH Executive Committee. He currently maintains a part-time practice at the Plattsburgh Podiatry Center.

MEETING NEWS
AAPSM Holds International Biomechanics Summit
 
AAPSM Fellow Ben Pearl, DPM hosted the Third Biomechanics Summit this week, attended by over 50 doctors and students. This year’s summit featured a notable presence of UK doctors, in addition to several AAPSM members and the biomechanics team from Brooks Running Shoes. Attendees had the opportunity to provide feedback on newly released Brooks running shoes. The program was set up to get valuable insights from podiatrists on improving the feel and performance of the shoes.
 
Ed Nyman, PhD, a biomechanics specialist at Brooks, discussed the lab’s holistic approach to understanding running. He highlighted the importance of aligning an individual’s movement profile with their habitual motion path to enhance performance. Richard Blake, DPM presented a methodical, head-to-toe approach for conducting biomechanics exams, focusing on one patient at a time. Missy Journot, DPM offered insights into ankle injuries, advocating for a comprehensive approach to ankle exams that extends beyond the ATFL and CFL ligaments. She shared a case study of a bifurcate ligament injury, diagnosed through clinical examination and confirmed by a cuboid fleck fracture on x-ray and MRI. Dr. Pearl led an engaging shoe forum, featuring previous AAPSM lecturers Golden Harper and Simon Bartold, as well as former English elite miler Clifton Bradeley.

RE: Changes are Needed in Podiatry

It costs IPMA/IAPPS a lot to send six delegates to the APMA HOD. Finally, the APMA sold their building. Now, they need to move the HOD to the country's center. The delegates act like they are members of the House of Lords. If the APMA wants to save itself, cut the HOD to two representatives per state. Some of our delegates talk about the Alliance and the Acey-Deucy sub-groups like they are "political parties." Enough is enough! Components could use the savings to promote podiatry medicine and podiatric surgery in their states. 
 
Next, the colleges need to offer all the courses offered in medical schools. We have been begging the Scholl College/Rosalind Franklin University to do so, especially since they teach those courses to their medical students. All we have gotten is excuses.

I see the situation Dr. Udell posed, as well as many others, as no different than the many other meds we use for systemic distribution for foot-related problems. Antibiotics and antifungals given for foot issues are also effective for other co-existing infections. Oral anti-inflammatories for foot problems are effective for inflammatory and arthritic problems elsewhere. Orthotics not only help for foot pain and function but oftentimes back pains as well.

We follow CMS guidelines. Care provided is a medical necessity. My point is not about coding or medical necessity. The point is that there is a clear bias against hospice patients receiving care as Medicare feels because they’re in hospice and at the end of life that they do not need the care. 
 
Some patients go on and off hospice and some are in it long-term (years). Also, Medicare sets a limit on the total number of hospice patients that can be treated and a set dollar reimbursement limit.

The 2023 guidelines published by the American Diabetes Association suggest the evaluation of temperature perception (for small fiber neuropathy) and vibratory perception (for large fiber neuropathy) by standard manual and clinical examination. PSSD is not recommended within the 2023 guidelines. Guidelines of the American Association of Neurology similarly fail to endorse routine PSSD testing as needed for the evaluation of diabetic neuropathy.
 
The potential benefits of QST are increased sensitivity and to a lesser extent specificity. However, the question to be addressed at the practical clinical level is whether the additional cost of on average $300 or more is justified. It has been demonstrated that diabetic neuropathy may be accurately diagnosed in 87% of patients with standard history and rather simple examination. If a patient demonstrates signs and symptoms of neuropathy, does quantification by QST alter the management beyond theoretical propositions? The rental of the PSSD equipment typically exceeds $1,500 a month.
 
Abuse of this technique is a concern. Given a patient with symptoms of neuropathy, an examination confirming peripheral neuropathy, clinical exclusion or inclusion of superimposed entrapment neuropathy, how does PSSD testing alter my care for the average patient? The enthusiasm for this technique appears to be within the AENS, “peripheral nerve surgeons”, and their enthusiasm for aggressive decompressive surgical procedures for the management of diabetic neuropathy, an approach generally not embraced by our neurology or endocrinology colleagues.
 
The theoretical benefits of QST (for a price) remind me of a Mark Twain quote, “Data is like garbage. You better know what you are going to do with it before you collect it." Or, as Albert Einstein noted, “there is the theoretical and the practical. And sometimes the theoretical is not practical."
