PRACTICE MANAGEMENT TIP OF THE DAY
7 Ways to Take Back Uncompensated Time -  Part 4

Physicians spend a significant amount of their workdays—probably 20% or more—on uncompensated tasks thanks to a barrage of regulatory requirements, says Joseph Valenti, MD, a board member of the Physicians Foundation, a non-profit physician advocacy group. “This equates to at least $50,000 of lost revenue per physician annually,” he says. Here are some tips from board members on reducing the amount of uncompensated time you spend in your practice:

4. Limit tasks to those that require a medical license.
  • Don’t open the mail, answer phones, or room that patient.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: Investing in the Future of Podiatry
From: Allen Jacobs, DPM

Over the last several years, I have observed the commendable efforts of the New York State Podiatric Medical Association in promoting our profession and encouraging recruitment of students to enter the field of podiatry. Each year, the NYSPMA offers a one-day program for college career counselors and premedical advisors introducing them to podiatry. A hotel room, meals, and an excellent program are provided. This includes a tour of the exhibit hall and attendance at a lecture. Students, residents, young and older practitioners, as well as local thought leaders provide a brief synopsis of their experiences and perspective. Additionally, college students are given conference and exhibit hall tours and discussions regarding podiatry as a potential choice for patient care.

Might I suggest that each state society and regional conference take responsibility for doing the same. Yes, some effort and money is required. However, from my observation, it would appear that participation of each state society, possibly with the assistance of a college of podiatric medicine, could potentially serve as an excellent means of student recruitment. Organizations holding scientific conferences, for example ACPM, ACFAS, DermFoot, PRESENT, and many others, should consider the same. There is no crime investing in the future of podiatry.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
From:  Timothy P. Shea, DPM

Like other podiatrists, I have been interested in, and pleasantly pleased, by the presentation of podiatric medicine and surgery on this series. These young podiatrists present a very pleasant, personable, and professional image of our profession. This can only help to promote our image in the public arena. I know we can be a little nit-picky about topics and credentials, but recently I was approached by the chief of medical staff at one of my local hospitals about the program. He and his wife (a physician) also watch it and enjoy it very much. The key point he made to me about this series is that it presents interesting cases and good information to the public about foot and ankle care. That was a real compliment. Kudos to the stars of this show.

RESPONSES/COMMENTS (PM ARTICLES)
From: Jeffrey Kass, DPM

In the article “Welcome to the NY Clinical Conference”, the executive board of NYSMA was asked “what is the biggest challenge facing podiatric medicine in 2020?” The board responded mentioning “the new healthcare landscape is providing podiatric medicine with great opportunities.” After mentioning podiatric medicine another four times in the same paragraph, the concluding sentence was “however [this], will only happen when the profession proudly endorsed the skills, education, training, and degree of its members.”

One of the biggest challenges I see is the fact that podiatric medicine has been left behind by New York unlike its podiatric surgery counterpart. The scope of practice in New York has been increased to the tibial tuberosity for those board qualified/certified in RRA by the ABFAS. The ABFAS podiatrist has been given the right to practice not only podiatric surgery but podiatric medicine as well. Yet, the board certified podiatrist in ABPM can’t advance their scope.

The profession needs to stay unified. Why should those with the skills, education, and training not be allowed to advance their scope? NY podiatry has been fragmented into two professions. This has created a dangerous landscape within the profession. Rather than staying unified, some are using the “surgical criteria” against their fellow colleagues in areas of credentialing and privileging. Before we ask “others” to respect the profession, we need the respect of each other within the profession. I wish the NYSPMA executive board all the luck in 2020 and hope they are successfully able to gain advanced scope for all licensed podiatrists.

RESPONSES/COMMENTS (PM NEWS QUICK POLLS)
From: George Jacobson, DPM

I don't think anyone mentioned debt. If your mortgage and student loan debt are paid off and your have no credit card debt what expenses are left? Taxes, utilities (including phone and Internet), insurance, food, and gas. Living in a low tax state helps. You can add all that up now and get a feel for the basics. Although many hate annuities (and I question myself), I took a percentage of my principle to "guarantee" income for life that is 2x my basic current necessities with my social security (another question mark). If I die, my spouse will have that income stream for life (supposedly). The other 80% of assets will continue to grow and provide the recreational income within my means.

Additionally, life insurance cash values and the equity in the home are further backdrops to catastrophe. Unfortunately, even being too financially conservative, the country's credit card never maxes out, so each of us always have increasing debt even when we are "debt free." Massive new social programs could have the "debt bomb" explode during our retirement, further threatening my so called "guaranteed income." Former Senator Tom Colburn, MD wrote The Debt Bomb in 2012, which is still a relevant book today. It's an easy read.
