IA Podiatrist Performs In-Office Ankle Procedure Using NanoScope
 
Using a flexible camera smaller than a pencil, Kosta Antonopoulos, DPM, a podiatrist at UnityPoint Clinic Foot and Ankle, was able to treat Tanya McPeek's painful ankle in November in a procedure room right in his office. Rather than go back into the operating room for a second surgery, Dr. Antonopoulos suggested that McPeek have a minimally invasive procedure performed with the NanoScope, an operative arthroscopy imaging system.

Dr. Antonopoulos said he has been doing procedures with the NanoScope in the operating room for several years. But, in the last year, he said it was granted in-office approval. He said the procedure room is made to be a sterile environment, just like the operating room.

NY Podiatrist Discusses How to Avoid Frostbite
 
To help us take care of our feet this winter, CareMount Medical podiatrist Raisa Tsvaygenbaum, DPM shared some tips on how to avoid frostbite. Easier said than done, right? Not really, explains Dr. Tsvaygenbaum. To keep frostbite far, far away from your toes, limit your time outdoors to a minimum.

“Don’t wear anything too tight, including shoes, because tight clothing increases risk of frostbite. If you start to experience redness and/or a stinging, burning, throbbing, or a prickling feeling, head indoors fast and use warm water to help bring feelings back to normal,” she says. As an additional preventive measure, she recommends staying hydrated and limiting alcohol consumption, since both alcohol and dehydration can lead to greater risk of frostbite.

ONC Looks For Comments on How to Improve Prior Authorization
 
The Office of the National Coordinator (ONC) for Health Information Technology is taking aim at one of the prime pain points for clinicians, providers, payers, and patients – the prior authorization process. Finding standards and using them, which would enable organizations to use data exchange to automate at least some of the prior authorization process would help improve patient safety, ensure continuity of care, reduce administrative burden, and help relieve clinician burden.
 
ONC last week announced that it is seeking public comment on electronic prior authorization standards, implementation specifications, and certification criteria that could be adopted within the ONC Health IT Certification Program. “Responses to this (Request for Information) may be used to inform potential future rulemaking to better enable providers to interact with healthcare plans and other payers for the automated, electronic completion of prior authorization tasks,” the ONC announced.

I'm sure the initial query written by Dr. Hodgson hits home to many podiatrists that bill Medicare and other insurance companies. In recent years, the -25 modifier has come under close scrutiny by Medicare because of its increased frequency of use. In my practice, I use a -25 modifier approximately 10-15% of the time when a separate and identifiable diagnosis exits. Although I do agree with Dr Hodgson's logic when using a -25 modifier in that particular case, the use of a level 3 (99213) E/M code is questionable. 
 
Maybe I need a refresher course in E/M coding but in my practice, I would have used a level 2 (99212) in the scenario Dr. Hodgson presented. Unless there were underlying systemic conditions that she documented in her chart, identifying and treating a painful hammertoe and callus would not cover enough "bullets" or actual "hands-on" time to justify a level 3 E/M code. Maybe I am just more conservative in my billing but that is my opinion. It would be very interesting to see if the insurance company would have denied paying a CPT 99212 code also.

I will never forget the good times, debates, collaborations, and inspiration that Don gave me over the past decade. His legacy would have been greater if more of us just accepted the changes that he improvised to podiatry practice. I consider him a glorious comet in my life that blazed before me and now is gone.

I read with a heavy heart today of the untimely passing of Dr. Peacock. I only met Dr. Peacock a couple of times when attending MIS seminars. Without a doubt, he was one of my favorite people to learn from. I have attended many seminars over my career and there are some docs that love to share and teach so that everyone collectively gets better and there are those who love to throw their ego around. Dr. Peacock was a gifted podiatrist, a great communicator, an excellent teacher, and a true mensch. My condolences to his family. Podiatry has lost a gentle giant.

RE: The Passing of Don Thomas Peacock Jr., DPM (Burton Katzen, DPM)

Don was a real gentleman and a class act. He was a brilliant and skilled podiatric surgeon both in open and minimal incision surgery; and sometimes in a blend of both. He was always there as a mentor and as an instructor to me and many others at the Academy meetings. I always left those meeting with a pearl of his wisdom that would benefit me and  improve my skillset and outcomes. On many occasions, I would call up or e-mail Don to get his input and opinion on some of my difficult cases, and he would always respond quickly and he was always right.
 
Don's media and social media presence really shined a positive light on minimally invasive surgery and podiatry both in North America and around the world. I reached out to Don several months ago hoping to catch up with him at the next Academy meeting but his wife got back to me saying that he was unable to speak. Don will be sadly missed by me and the profession. RIP.
