Consider Minimalist Shoes Before Going Completely Barefoot: NY Podiatrist
When a toddler is learning to walk, parents are encouraged to let this process happen naturally. Without shoes. “That’s because shoes can affect how a child uses the muscles and bones in their feet,” says a recent Healthline Report. Kids receive feedback from the ground when they walk barefoot, which improves awareness of their body in space. Translating that to adults, it is also noted that advocates of barefoot walking and exercising “are pushing back on wearing shoes all day long and encouraging all of us to let our feet be free.”

As board-certified podiatrist and foot surgeon Bruce Pinker, DPM points out to Healthline, the benefits are considerable: better foot mechanics, which begets improved mechanics of the hips, knees, and body core; Stronger leg muscles, supporting the lower back region; and relief from improperly fitting shoes, which may cause bunions or other shoe-caused foot conditions. Dr. Pinker also suggests considering using a minimalist shoe before going completely barefoot.

NY Podiatrist Provides Advice on When to Replace Running Shoes
The right pair of shoes can take you far — although you want to be careful not to clock too many miles in one single pair. Generally, you should be replacing your running shoes about every six months, Suzanne Levine, DPM, a podiatrist at Institute Beauté in New York City, told TODAY. After that time period, your shoes can lose their cushioning, which might put you at risk for stress fractures in the feet and legs. "Most people wear out the shoes unevenly," Dr. Levine said.

"With use, this can add to instability and can result in an increased risk of sprained ankles, plantar fasciitis, shin splints, runner’s knee, and even hip injuries." Of course, the six-month time frame isn't a hard and fast rule. A marathon runner is likely going to need to replace their shoes far sooner than someone who laces up a few times a month. That's why a good rule of thumb to follow is replacing them every 400 to 500 miles, she said.

Query: Wound Care Coding for Hospice Patients
I often provide wound care for hospice patients and append the -GW modifier. However, I recently read that it would be hard to defend this as the wound(s) and wound process is likely related to the patient being deconditioned and malnourished due to their hospice qualifying condition. I am looking for clarification regarding this. Should I stop performing wound care services for hospice patients?

Response: The appropriate modifier is -GV, indicating that the treating physician is not paid by the hospice provider. We generally use both -GW and -GV and have never had a problem being paid.

Tilden Sokoloff, DPM, MD, says regarding DPMs being able to perform vaccinations, "Time to up the ante and gain a plenary license". There is an old mathematical theory that states, "Things equal to the same thing are equal to each other or 1=1." Podiatric physicians should complete the medical licensing examinations because they are, in fact, physicians (i.e., 1=1).
Our students should complete each of the parts of the medical licensing examinations followed after the first or second year of a podiatric residency by what is now Part 3 of the podiatric medical licensing examinations. This would provide us with the appropriate license and qualify us in our specialty. The time is long overdue.

You are paying for the sins of others. When I was in practice, patients would come in with shoes from suspect private stores where the shoes were obviously not custom-molded. The inserts came right off the shelf with no corrections for lesions, etc. I repeatedly would tell the patient to go back and complain. One showed me a pair where some charlatan placed a one cent rubber pad inside and charged him $80.
I had a family of patients that would brag to me as to the number of shoes they could get. Read old OIG reports about the abuse with braces, scooters, TENS units, and power chairs. As an ethical practitioner, you are being inundated with paperwork for saving people's feet. Blame the system. The free lunch is never free.

I have read the postings regarding the time for podiatry to do "something" about DM shoe requirements. Unfortunately, the "something" has been equated to actually succeeding. And while unfair, that assumption is readily understandable to me as both a provider of therapeutic shoes and chair of APMA's DME Workgroup. APMA and other organizations have had a litany of meetings with the DME MAC contractors regarding this matter. I actually attended my first meeting with the then SADMERC (now PDAC) well over 15 years ago, and I have attended many more since with other DME MACs on this issue.
At one point, a former DME MAC Carrier Medical Director joined me and APMA officials to speak to the DME MAC Carrier medical directors, asking for relief for suppliers. Unfortunately to no avail. Recently, NP/PAs have been given...

The sudden passing of Joseph (Joe) Hughes, DPM, was a sad event for all of us members of the CCPM Class of 1983 and for the California Podiatric Medical Association. Joe was just one of those great guys that always had a smile on his face and never had anything negative to say about anybody. However, behind the scenes, he worked tirelessly year after year for the CPMA and its many members.
Dr. Joe Hughes represented the best of podiatry for his community, his state, and especially for those of us who knew what a wonderful, selfless individual he was for his profession, and for his family. Condolences to his lovely wife, Pam, and their family. He will be sorely missed.
