According to Christopher Reeves, DPM, "For years, the only option from a reconstruction standpoint has been ankle fusion or ankle arthrodesis where the joints actually are taken away and made stiff. Ankle placements, though they’ve been around for many years, in some form in the last 15 years or so, the new generations of implants have continued to improve."

"[Post-operatively] typically by six to eight weeks, we have them back into a shoe and starting to ambulate fairly comfortably without an assistive device. Well, from there still dealing with swelling, you’re still dealing with some overall general post-operative discomfort but doing well enough at that point to to start getting around a lot," says Dr. Reeves.

7 Ways to Take Back Uncompensated Time - Part 3
Physicians spend a significant amount of their workdays—probably 20% or more—on uncompensated tasks thanks to a barrage of regulatory requirements, says Joseph Valenti, MD, a board member of the Physicians Foundation, a non-profit physician advocacy group. “This equates to at least $50,000 of lost revenue per physician annually,” he says. Here are some tips from board members on reducing the amount of uncompensated time you spend in your practice:

Query: New Practice, Same Patient
I have a new practice location in Southern California. I used to see a Blue Cross PPO patient at the previous medical group I worked for. That patient was billed under their EIN. Now, she is seen at my new office. I billed 99203, because I had to completely work her up again and used my new TIN. Both were denied. For the 99203, the denial explanation was that a new patient visit was previously submitted within the past three years. The CPT 11057 denied because it is not a covered expense of the patient’s plan. I billed 99203-25 with M7740, M79673 and CPT 11057 with L851, M79672, M79671. She has no systemic problems.

Response: Regardless of whether or not you are in a new practice or not, if the patient was seen by you and billed in association with your NPI number within the last 3 years, then this is not considered a new patient according to the insurance company. With no systemic problems, CPT 11057 most likely would not be covered.
While the conversation may be a difficult one, you need to inform the patient that if they wish to continue to be seen, these services are not covered and would need to be paid out of pocket.

RE: The Passing of Robert Lee, DPM
Dr. Robert Edward Lee, 92, of Elk Point, devoted son, brother, husband, father, grandfather, great-grandfather, and friend, passed away peacefully on Jan. 22, 2020, at Holy Spirit Assisted Living Facility in Sioux City, Iowa.

Dr. Lee served honorably in the Army and graduated from the Illinois College of Podiatric Medicine in 1952. After practicing podiatry for several years, he joined the Veterans Administration and embarked on a career in hospital administration.

In response to Dr. David Krausse’s question: He posits, “Does it not bother anyone that neither one of these two doctors are not board certified in foot surgery?” Really? Who cares? The only ones who are "bothered” by this are the minority of our colleagues who keep those DPMs who are not board certified off of hospital staffs and insurance plans. Unfortunately, podiatric insecurity that has its genesis in podiatric medical school is still “alive and well.”
As a ABFAS board certified podiatrist, I or my staff had NEVER been asked if I was board certified. “Patients don’t care what you know; they want to know that you care.” These two TLC podiatric surgeons will have, through this national exposure, a positive and infinite benefit to our profession.

The bad news is that many physicians are saving little to nothing. According to the same study, nearly 60 percent of female doctors were not making maximum contributions to a retirement plan. Roughly 45 percent of male physicians were not either. In a 2013 survey by the American Medical Association, about half of responding physicians said they consider themselves behind in preparing for their financial futures.
A recent Medscape survey of physicians showed that 34 percent between the ages of 50 and 64 have a net worth under $1 million. And nearly a quarter of doctors 65 and older have a net worth under $1 million. Since a person’s net worth includes more than retirement savings, it’s likely these physicians do not have enough money for a comfortable retirement.
