Scholl Faculty Publishes Study on Custom Balance AFOs
Older adults may be able to avoid falls due to balance instability by wearing custom-made ankle foot orthoses, according to a study conducted at the Center for Lower Extremity Ambulatory Research at Rosalind Franklin University of Medicine and Science in North Chicago, IL, home to the Dr. William M. Scholl College of Podiatric Medicine. The study, published December 2014 in the journal Clinical Biomechanics and reported this month by O&P Business News, saw improved balance and stabilization in subjects, age 65 and older, who were provided custom-made AFOs (Moore Balance Brace, Arizona AFO) developed by podiatric physician Jonathan Moore, DPM, MS.

“This was an initial evaluation at initial presentation of the device,” said Ryan Crews, MS, CCRP who, along with fellow CLEAR researcher and assistant professor Sai Vikas Yalla, PhD, led the study. “We definitely saw improvements in balance and stability, which ultimately could lead to a reduction in falls due to [participants] being more stable,” Crews said. The study showed improvement in all balance-related assessments. Adam Fleischer, DPM, MPH, associate professor in Scholl College’s Department of Podiatric Medicine and Radiology, co-authored the journal publication.

CT Podiatrist Provides Winter Safety Running Tips
Winter running presents hazardous conditions that may not be present at other times of the year. "In the winter, we are often forced to work out in the darkness of early morning or after work. One of the most important pieces of safety equipment for added visibility is a reflective vest. It is a small investment but may be the best life insurance one can buy for winter running. Also available are reflective tapes and arm bands. In addition, many runners carry a flashlight to alert oncoming motorists, as well as to see the road in front of them, "says sports podiatrist Dr. Robert Weiss.

"Safety in footgear must also be followed; good traction in snow and on wet roads is very important. Also, make sure the shoes are roomy for movement of the toes, as tight shoes will reduce blood supply to your feet," says Weiss.

The Conference on The Economics of Podiatric Medicine and Surgery held in Chicago this past Jan 23 and Jan 24 proved to be a success as participants gave it a thumbs up. "The information presented here is exactly what is needed to move forward in today's healthcare world," Stated attendee Dr. John Meyers.

The faculty included leaders in the healthcare field including hospital CEOs, attorneys, and healthcare consultants. An excellent Presentation by Dr. Joseph Borreggine titled "Is the Future Bright in Podiatry?" and another by Dr. Sev Hrywnak on "How to Use Metrics and Gain Entry into an ACO" highlighted the conference. As the keynote speaker for the conference, state representative Jim Durkin, of Illinois, spoke on the current condition of the health insurance exchange. The next seminar is scheduled for this coming June of 2015.

Obama Would Have Few Options If SCOTUS Rules Against ACA
If the Supreme Court tears apart Obamacare this summer, the president won't be able to put it back together all by himself. Executive action is all the rage in the White House these days, and it's hard to imagine a better candidate for unilateralism than fixing the Affordable Care Act in the wake of a crippling Supreme Court decision.
There's just one problem: A good administrative solution might not exist. "There are no administrative fixes that are realistic," said Neera Tanden, president of the liberal Center for American Progress. "We don't believe there's any administrative fix."

Query: Genetic Testing
I recently diagnosed peroneus brevis insertional tendonitis vs. 5th met base stress reaction in a 22 year old male patient. I prescribed non-weight bearing in a CAM boot for 3 weeks, likely to be followed by 3 weeks weight-bearing in the Cam boot. I reviewed DVT signs/symptoms/risk factors with the patient and discovered that his mother has protein S deficiency. I explained the condition to him and recommended that he be tested. He agreed, and signed a consent for this. I tested for Protein S and Factor V Leiden. The protein S returned abnormal.
I notified his PCP as a courtesy to see if he wanted to treat the patient for DCT prophylaxis, as I understand that if he is immobilized, it is warranted with his condition. He agreed to, and in his note wrote that the patient received no counseling as mandated by law prior to getting genetic testing, and this could require lifelong therapy and could have potential career-ending consequences for the patient for his particular occupation. So should we test for this, or set up genetic counseling prior to getting these tests?

Query: Hardware Removal
Our doctor removed a pin and an external fixator under fluoroscopy in the ASC. The patient was under IV sedation (not general anesthesia) with the soft tissue locally anesthetized. Can we code both procedures, CPT 20680 and CPT 20694-52 (reduced because no general was administered)?

Response: If the pin is buried (not protruding above the skin), and it requires an incision to remove it, you can bill CPT 20680 (if deep - at level of bone) or CPT 20670 (if superficial, buried in the subcutaneous tissue).
You cannot bill CPT 20694 if you remove it under a local with sedation. "CPT Assistant" (July 2000) notes: "CPT code 20694, Removal, under anesthesia, of external fixation system, may be reported if the removal procedure is performed under general anesthesia. General anesthesia is not usually required to remove an external fixation system, therefore, removal not requiring anesthesia is not a separately reportable service." You cannot alternatively add a "-52" reduced service modifier since the value assigned the application includes removal without general anesthesia.

While Parkinson’s disease is typically associated with a low uric acid level, it would be best to check the patient’s serum uric acid level sometime when they are not having a flare to determine if the patient has a high uric acid level and would need to start a uric acid lowering therapy.

Dr. Amarantos hits the nail on the head when he comments on not complying with the onerous IT requirements and says, "...who cares?" Let Medicare take their %. Will a 7-10% decrease in Medicare reimbursement really change your quality of life?" If you think about it from a business perspective, if you calculate the expense of all the human resource time needed to comply, and if you take that amount of money and invest it in solid practice marketing programs, you should be able to profit far in excess of your Medicare loss.
