RI Podiatrist Discusses Onychomycosis

When it comes to fungal toenails, Dr. Robert Gallucci, a Warwick podiatrist, offers some helpful tips for preventing this unsightly infection. First, patients should keep their feet clean and dry - bacteria thrive in dark and damp places, especially on the feet.

Dr. Robert Gallucci

If patients perform pedicures at home, it is also important to disinfect those tools because the infection can linger on them even after the fungus is treated. Dr. Gallucci also encourages patients to avoid applying polish to nails if an infection is suspected, and to wear shoes in public facilities whenever possible.

NY Podiatrist Receives Excellence in Action Award

The New York State Podiatric Medical Association (NYSPMA) has awarded Dr. Ronald Guberman its Excellence in Action Award. This honor, presented at the New York Clinical Conference, was in appreciation for Dr. Guberman's efforts in helping pass the recently enacted scope of practice bill.

(L-R) NYSPMA trustees Drs. Andrew Shapiro and Paul Liswood present award to Dr. Ronald Guberman

At a time when orthopedists were writing letters suggesting that podiatrists who performed ankle surgery would present a danger to the public, Guberman, the residency director at the Wycoff Heights Medical Center, was able to recruit 25 physicians to write letters of support negating the effect of those letters.

NJ Podiatrist Facing Prison After Admitting to Selling Painkillers

A Park Ridge doctor is facing up to five years in prison after pleading guilty to distributing oxycodone on Friday, The Record reported. Dr. Carnig Shakarjian was arrested in 2011 following an investigation into large doses of the painkiller he prescribed to several patients. Undercover officers were later able to secure pills from him despite expressing no medical need for them, and police allegedly found that he sold pills to around 50 people whom he had not seen as patients.

RE: Pediatric Digital Deformity (Edmond F. Mertzenich, DPM, MBA)
From: Stephen Musser, DPM

Something you can try now is Moldable Podiatry Compound (or something similar) to splint his toe(s) in a more rectus position and see if this will help correct the deformities.

RE: PTTD Surgery (Tip Sullivan, DPM)
From: Peter Walimire, DPM

My suggestion is to perform a double TN/STJ arthrodesis. You can avoid additional trauma by simply extending your TN incision proximally and resecting the STJ through the medial incision.

I suggest this for a few reasons. First, the patient is a healthy 75 year old with documented radiographic signs of arthritis in the STJ. It seems that your only reason not to do it is to spare her the pain. I postulate that not fusing the joint will...

RE: Elongated Halluces (Robert Chelin, DPM)
From: Ed Cohen, DPM

The long big toe can be a major part of the etiology of some bunions. When you add metatarsus adductus combined with narrow fashionable shoes, there is a good chance that the big toe will become laterally deviated and the patient could develop a bunion. At the very least, this could be a significant shoe problem for this young lady and should be surgically addressed.

I frequently have patients with a bunion and a long big toe. In some bunions, the IMA and HAV can be adequately corrected with a ...

RE: ASC Billing Software (Wes Daniel, DPM)
From: Michael M. Rosenblatt, DPM

Wes Daniel, DPM wrote requesting information about ASC billing software. There is an excellent venue that represents the interests of ASCs politically and in other areas. It is called the Federated Ambulatory Surgery Association. I believe they have a site that offers advertising for various vendors, or they could probably put you in contact with some software vendors. Their web address is: ascassociation.org/Home/. They offer membership services that can be very valuable as well.

RE: Changing Office Hours (Olga Luepschen, DPM)
From: Keith L. Gurnick, DPM

When your office is open only four days (10 hours) instead of 5 days (8 hours), you will become less efficient and see fewer patients. You will lose the Friday ankle sprains and fractures that cannot wait until Monday to be seen, and can't even schedule a Monday appointment when they phone after you close Thursday. In addition, when an employee is sick or needs a day off, that one day off represents 25% of their weekly productivity lost, instead of 20% for a 5-day work week.

Also, you may find your employees become less productive as their energy level diminishes near the end of longer shifts. I suggest that if you want your staff to have 4-day work week, you should hire some part-time staff and rotate them into your 5-day schedule. If you are the one who wants to limit seeing patients to only four days, it is possible but there is a cost in that you will lose those patients who need to be seen on the day you are not at work because you and your staff are enjoying your 3-day weekend.

RE: Be Prepared for Healthcare Changes (Joseph Borreggine, DPM)
From: Daniel Grapel, DPM

While I agree with 80% of Dr. Borreggine's extensive post, allow me some remarks. I think that the comparison with dentistry is not proper. While many aspects of our profession can be performed by orthopedists, dermatologists, or physical therapists, dentistry can ONLY be practiced by dentists. Nobody else! In addition, while about 60-70% of all people ever visit a dentist, how many visit a podiatrist? I doubt if more than 10%. This is a huge difference.

Dentistry never had to prove the value to the public since poor dental health can have immediate deleterious effects. With podiatry, it is not so...
