PODIATRIST IN THE NEWS
TX Podiatrist Discusses Treatment of Diabetic Neuropathy

The best treatments for burning feet depend on what’s causing the pain. People with diabetes are at a higher risk of developing diabetic neuropathy, says Gurpreet Mukker-Casperson, DPM at Houston Methodist The Woodlands Hospital. This type of nerve pain often results in painful burning and tingling sensations, as if your feet are “falling asleep.”

Dr. Gurpreet Mukker-Casperson

“Neuropathy can occur in your feet when blood sugar levels are elevated for an extended period of time,” she explains. “High blood sugar levels cause glycosylation within the nerves that interfere with signal transmission. This results in a burning/tingling sensation in the feet that can last for hours, if not for days.” In this case, managing the underlying condition is the most effective strategy, says Dr. Mukker-Casperson. This entails controlling your blood sugar on a daily basis with a healthy, well-balanced diet low in sugar and carbohydrates and rich in vegetables and lean protein such as chicken and fish, she notes.

INTERNATIONAL PODIATRISTS IN THE NEWS
Pakistani Podiatrist Advocates For Quarterly Medica Pedicures

Medical pedicure is a procedure performed in a clinic to treat various foot problems, including nail fungus, ingrown toenails, corns, and calluses. Talking to Associated Press of Pakistan, podiatrist Salma Roshan said that medicated pedicure is recommended every three months to prevent foot-related issues. She explained that performing the pedicure helps in preventing the spread of fungus from its initial stage to advanced stages.

Salma Roshan

Salma highlighted the importance of exercising extra caution when treating diabetic patients, particularly those taking blood thinners. “Diabetic patients with ingrown toenails, corns, or calluses should avoid attempting to cut their own nails to prevent wounds that may lead to serious complications,” she warned. “To address persistent issues, custom orthotics or shoe inserts are used, which help in dealing with abnormal arches, hammertoes, and bunions,” she said.

PRACTICE MANAGEMENT TIP OF THE DAY
10 Small Ways to Provide Great Customer Service to Patients

Providing great customer service to patients will make them feel like you are all on the same team, they will be more apt to send referrals, and they will be more willing to make sure you are paid for the time and effort you have spent on their care. Here are 10 ways to provide excellent service to your patients outside of the exam room:

2. Keep Your Promises

If you or your staff says, “I will get back to you today,” do it. Even if you don’t have the answer, a quick ring to let patients know you are still working on it is always best. Keep your commitments, no matter how small.

QUERIES (CLINICAL)
Query: AV Malformation

My granddaughter was diagnosed with an AV malformation in her foot, located around the plantar aspect, slightly distal to the calcaneal tuberosity. She is 11 years old and has been dealing with this for around two years. Because of the difficult location being against the nerve and artery, my daughter and son-law were advised to try sclerotherapy by an interventional radiologist. This was done twice at CHOPS in Philadelphia. Neither attempt gave relief to my granddaughter.

The latest MRI which was done this week showed the AVM was slightly more flattened than the last MRI and involved the flexor digitorum brevis and quadratus plantae muscles. Also noted was edema of the diaphysis of the 2nd metatarsal which has developed since the previous MRI. The marrow of the 2nd metatarsal is heterogeneous and may reflect an infarct or be indicative of stress. If anyone has dealt with this condition and can give some input or doctor recommendations, I would greatly appreciate it.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: The Ethics of Obfuscation
From: Rod Tomczak, DPM, MD, EdD

I was starting on a crossword puzzle and the clue for three across was a ten-letter word for “foot doctor.” The answer was far too obvious, so I started thinking about other terms for a minute or  two. After completing all that, I could this morning I decided to look at the other terms we now use to replace the term podiatrist. I cross matched “podiatrist” with cities in the United States to see what they had to offer. After a half dozen or so cities I quit the hunt. I found the following portrayals:

Lower extremity orthopedics; foot and ankle surgeon; reconstructive rearfoot surgeon; minimal incision bunion surgeon; lower extremity surgeon; ankle surgeon; foot and ankle trauma surgeon; biomechanical surgeon; board certified lower extremity surgeon; cosmetic foot and ankle surgeon; toe shortening surgeon; cosmetic...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
RE: Why Podiatry School Applicants Remain Low (Robert Kornfeld, DPM)
From: Evan Meltzer, DPM

I’ve read with interest most of the letters regarding the low number of applicants to podiatry schools and the current state of podiatric education. I graduated from PCPM in 1977 along with just over 100 of my classmates. Two others and I went into the Army. At that time, there were only 3 states that required a one-year residency for licensure. I remember only 2 other locations where additional training was available beyond the one year: Dalton McGlamry’s program in Atlanta and Earl Kaplan’s in Detroit.

I believe that podiatric training should be both medical and surgical. While I don’t intend to write this letter as a promotion for my recent memoir, I do discuss the frustration that I experienced when my surgical privileges were...

RESPONSES/COMMENTS (PODIATRIC EDUCATION)
From: James DiResta, DPM, MPH

Dr. Kass couldn't be more correct when he states, "Everyday I read how various professions are increasing their scope of practice... nurses, NPs, PAs, etc. I wonder when podiatry is going to jump on the bandwagon." I read today where pharmacists are expanding their scope from treating covid with Paxlovid, flu symptoms with (oseltamivir), Relenza, or Xofluza and in some states now prescribe antibiotics for various illnesses like a urinary tract infection. The horse and wagon left the barn years ago; when the heck will podiatry jump on? The profession needs to increase its scope in medical care.
