Custom Orthotics Can Reduce or Eliminate Pain Caused by LLD: VA Podiatrist
Humans are not perfectly symmetrical. Almost everyone has one ear that’s higher or one foot that’s larger than the other. Similarly, leg lengths are often not quite the same. There is disagreement as to what constitutes a clinically significant difference, but some studies suggest that leg length discrepancy (LLD) can lead to osteoarthritis (OA) of the hip or knee. Foot height discrepancy can be a significant contributor to LLD, says George Lane, DPM, a podiatrist at Achilles Foot & Ankle Center, Richmond, VA who developed a novel technique for evaluating foot contribution to LLD.

“A mild to moderate foot height discrepancy, which is often just a few millimeters, means there are typically significant misalignments within the joints of at least one of the feet,” Dr. Lane explains. “This can not only have a profound damaging effect on the joints of the feet, but also on ligaments, tendons and joints of the lower extremities, and back. Properly made custom orthotics can play a pivotal role to dramatically reduce or eliminate pain, progression of deformity, and joint damage in this situation.”

Query: Non-Painful Bump on Ankle
This is an unusual incidental soft tissue finding in a 50 year old male who came in with painful retro-calcaneal pain. He recently noticed a non-painful bump on his ankle.

There are elevated LFTs and there are elevated LFTs high enough to stop therapy. Having LFTs high enough to stop therapy is exceedingly rare. Just for perspective, most PCPs will not think about stopping a statin drug until the LFTs are 2-3 x normal range! I get asked weekly by colleagues if they should D/C terbinafine for a 5 point elevation. This degree of caution is unwarranted. I still maintain that the far greater medico-legal risk is prescribing terbinafine without laboratory confirmation of a fungal infection, a way too common scenario, not only in podiatry, but in primary care and even in dermatology.
In that setting, a negligence claim arising from a truly serious idiosyncratic reaction to terbinafine, (having nothing to do with whether or not laboratory confirmation was done) will be forced to settle, as every juror understands the concept of culture and appropriate prescribing. As soon as the possibility is brought to light that the patient had therapy initiated without confirming the diagnosis, the case is lost, or the risk/cost of defending it too great, even though getting a laboratory confirmation would not have made a difference.

Dr. Williams said it all when he wrote that in the case of where one of his patient's liver enzymes elevated while on oral terbinafine, he sent the patient back to his primary care physician. Here is where a potentially serious problem may lie. I often tell patients that I never had a patient die from onychomycosis, but over the years, I have had to send condolence cards to families who lost loved ones who died from liver disease. The primary care physicians are well aware of the serious nature of hepatic disease. So what may happen if you or I send a patient back to his or her internist with elevated liver enzymes either caused by or exacerbated by oral terbinafine?
The doctor will have to evaluate what other medications the patient is taking, and in all likelihood will take the patient off of all meds including Tylenol in order to give the liver a rest. This is why most of the internists in my area will not prescribe oral terbinafine for patients who are taking many other meds. Lastly, could we be held legally responsible if a patient had an adverse event after his internist was forced to take him off of any life-sustaining meds because we overloaded the liver with terbinafine?

I remember when I was in school. There was 'Vision 2015'. It essentially stated that the goal of podiatry was to have parity with MDs and DOs by 2015. Yet here we are, some 6 years later, still seen as second class physicians, or "allied healthcare providers". Even with the excellent work of the APMA, a podiatrist serving in Congress, and mandatory 3-year residencies (the same as PCPs nationwide), we still have to have a 'real doctor' sign off on our patients. Who knows our patients’ feet better than us? Who sees our diabetic patients more than us?
The point is this: We should be able to sign for the shoes ourselves. My staff and I spend countless hours explaining to our patients that the NP and PA they see for their primary care doesn't 'count' in getting shoes signed for. This also creates a potential rift between podiatrists and NPs and PAs, who see us sending patients they referred to us to another provider for this visit. By signing for ourselves, it eliminates barriers to diabetic ulcer prevention and lets our patients see their PCPs without burdening the system with unnecessary MD/DO foot checks.
For better or worse, there will be political changes to the practice of medicine. Likely, one of these multi-thousand page bills will be signed into law. Can we have just one of those pages give parity to DPMs? Title XIX has hindered us long enough. It's time for DPMs to take their seat at the table with MDs and DOs.

I must compliment Barry Block, DPM, JD (editor of this forum) for allowing colleagues to express opinions here. It is becoming a little bothersome to continuously read problems the profession faces at this stage of the “profession”. Podiatry has evolved. Our colleagues have advanced this profession by leaps and bounds. Drs. Sokoloff and Marcus eloquently write about their frustrations with volunteering for administering Covid shots and the absurdity of the rules and regulations of the diabetic shoe program. I agree with them both as I’m sure the overwhelming majority of podiatrists do.
I’m curious what the APMA’s position is on these topics and why it is so difficult to straighten the BS out? Multiple drug companies figured out in “warp speed” how to create a vaccine in record time but podiatric leadership can’t figure out a way for a podiatrist to dispense a diabetic shoe to a patient missing multiple toes without writing an encyclopedia, asking an MD to sign off on it, and then having the money taken back anyway for not including an Rx even though he was the prescribing doctor.
I encourage those who have ideas how to change some of the dilemmas the profession faces to write in with them. Whether that means having every diabetic patient sign a petition or what not. I mean people - we really ought to put some of these issues to rest already.

RE: No Evidence That Running is Associated with Chronic Knee Pain: MN Podiatrist

I am hoping that Dr. Langer, while probably technically correct that hard evidence does exist for a correlation of high intensity exercise/running and knee problems, does not believe that NO correlation actually exists. There is NO question that over-pronators cause stress to the medial knee. This causes pain - the more you pound on your feet with high intensity exercise, the more damage and pain that will occur. Of course, once a proper orthotic is made, this pain will decrease or subside.

Dr. Langer Is correct that meta-analysis of published studies did not show scientific correlation of knee arthritis associated with running. There was a sports-specific participation literature review by Jeffrey Dibran in 2017 in the Journal of Athletic Training that was interesting. They collected 56 studies and found there was a 7.7% vs. 7.3% incidence of knee arthritis in elite and non-elite soccer players and elite runners (non-elite runners were not mentioned).
Anecdotally, I have noticed that after playing soccer sometimes for up to two hours in a pick-up game on a knee that is already compromised with a previous meniscectomy, I will experience knee pain and stiffness. I also noticed this if I run for long distances over 4 miles at a fast pace. The nature of getting good longitudinal studies for these types of problems is difficult and as authors suggest, but there is still work to be done.

I couldn't agree more. Let's all honor this great and good man's memory by doing that for the specialty he loved. RIP, Uncle Joe.
