NC Podiatrist Describes Treatment for Football Star with Plantar Plate Tear
 
There's finally a medical term to attach to Taylor Martinez's foot injury that changed the course of Nebraska's football season. Dr. Mark Quist, a podiatrist and president of Carolina Foot and Ankle, diagnosed Martinez after the season. He described Taylor's injury as a “plantar plate tear” of the second metatarsophalangeal joint, or “2nd MPJ,” in his left foot.

“His treatment has consisted of physical therapy, immobilization with a leg walker, injections, inserts, and medications,” read Quist's message. “He is no longer having pain plantarly, but the injury has left his 2nd MPJ in an elevated state, which can make the joint more vulnerable to capsulitis/tendinitis/metatarsalgia. The tough part of this injury is relieving the plantar pain, which, fortunately for Taylor, has completely resolved. The other types of pain associated with this injury can easily be relieved with toe straps/orthotics/pads and medications if needed.”

Osteoarthristis Can Be Limited to the Feet: Aussie Podiatrist
 
One person in six over 50 will develop osteoarthritis in the middle part of the foot, according to a recent study at Keele University’s Arthritis Research UK Primary Care Centre. The condition is characterized by inflammation around the joints, damage to cartilage, and swelling, resulting in pain, stiffness, and trouble moving, and sometimes bony bumps over the top of the foot. It is possible to develop osteoarthritis in the feet alone, says podiatrist Dr. Anthony Redmond, a professor in clinical biomechanics from the Leeds Institute for Rheumatic and Musculoskeletal Medicine.

Risk factors include genetic predisposition, injury to the area,, and overuse. Action: Good training shoes will minimize stress to the feet, and losing weight can ease pressure on joints. Research found that people with pain that varies with activity had a higher prevalence of lesions in the bone marrow, thought to be a precursor to osteoarthritis. “Judicious use of orthotic insoles” can help to reverse these problems, says Redmond.

Record Turnout At Codingline NY Meeting

Despite near record low temperatures, over 350 podiatrists braved the elements to attend the Codingline New York Meeting. A packed ballroom of attendees, concerned about the onset of ICD-10 this October, listened to some of podiatry's most knowledgeable speakers discuss the implementation of onerous coding changes. Dr. Harry Goldsmith presented an ICD-10 primer and discussed wound care coding.

Dr. Barry Block discussed the legal aspects of EMR auditing, while Dr. Paul Kesselman provided a DME Update. Dr. James Christina's presentation focused on PQRS, eRX, and Meaningful Use. Dr. Bryan Markinson provided a dermatology coding clinic. The program concluded with a Surgical Coding Clinic presented by Dr. Allen Jacobs.

Routine foot care patients have been the very basis of DPM practice ever since the need for our services started. Even though I owned and operated a Medicare-certified surgical center, I still saw RFC patients. Often I was glad to see them listed, because a large surgery schedule is tiring (even exhausting), and RFC requires much less thinking or planning. It is also necessary, even if Government and private insurers are unwilling to pay for it. 
 
The crux of the issue: patients expect it for free or universally covered by insurance. I think even Kaiser has problems with this, with expectation of coverage, or at least a very...

In evaluating the increased levels of documentation for Stage 2 Meaningless Use (that was not a spelling error), I agree that it will entail a lot of work for the lowest amount of incentive funds. CMS has changed the rules of the game. Assuming that one stated attesting in 2011, there has been a 2% reduction in payment, such that the incentive funds will be $7,840 in 2013, $3,940 in 2014 and %1,960 in 2015. In 2014, the attestation period is any one quarter of the year – only three months compared to the usual one year attestation period. I read this information directly from the CMS website. 
 
So, if you want to wait and make your decision in October 2014, you could still receive the full amount of 2014 incentive funds. The issue that is mentioned but not clearly stated is whether there will be a retroactive incentive payment reduction if one does not complete the entire program. Does anyone have a specific answer to that question?

Dr. Gordon is a little short of completely characterizing the situation when he comments that we are one of the few professions required to accept negotiated rates. If an auto mechanic charges you based on an hourly rate chart, you have to pay him before he'll release your car. Even if we know our negotiated fee, in many instances we aren't even allowed to collect it.  I pose the question: "In what other business is it required that instant credit be extended to all customers, and with terms that are undefined and difficult to enforce?"
