OH Podiatrist Discusses Posterior Tibial Tendon Dysfunction
According to Lance Johnson, DPM, "Posterior tibial tendon dysfunction, also known as progressive collapsing foot deformity, occurs with aging and wear and tear, resulting in a painful flatfoot. It occurs when chronic stress is placed on the posterior tibial tendon and other soft tissue structures of the arch, resulting in inflammation, and sometimes tearing of the tendon. Posterior tibial tendon dysfunction can occur with underlying deformity or arthritis."

"Treatment of posterior tibial tendon dysfunction depends on severity. Rest, ice, compression, elevation, and anti-inflammatory medications can all be used to initially treat pain and inflammation. Orthotics/inserts, CAM boots, or ankle braces may all be used to help support the foot and treat symptoms. Physical therapy and steroid injections can also be indicated in certain circumstances. Surgical intervention is typically recommended when pain is interfering with a patient’s hobbies, work, and activities of daily living," says Dr. Johnson.

MI Podiatrist Pleads Guilty In Sex For Drugs Diversion Scheme
A Michigan podiatrist entered a guilty plea today to a two-count indictment charging unlawful distribution of controlled substances for his involvement in a sex-for-drugs diversion scheme, announced United States Attorney Dawn N. Ison. Dr. Maninder Deswal, 44, of Lansing, was charged with unlawfully prescribing oxycodone-acetaminophen (Percocet) and hydrocodone-acetaminophen (Norco), highly addictive opioids, to various women outside the course of professional medical practice.

Query: Chemical Neurolysis Using 4% Alcohol
It seems that Blue Cross Blue Shield has a medical policy attached to CPT 64640 and CPT 64632 which says it’s ablation and “therefore experimental, investigational, and/or unproven. Criteria used in making a non-approval determination may include Milliman USA Care Guidelines, HCSC Medical Policy, Blue Cross Blue Shield Association Reference Manual and/or a healthcare professional’s recommendation.” Any suggestions?

Response: Each carrier can make certain policies it feels are necessary. That does not mean the policies are correct or offer the best and latest medical care for their insured. They do try to defend their policies by citing other references versus making a unilateral decision. So we as a profession, need to come up with our own peer reviewed studies to try to get this carrier and other entities such as Milliman to rethink their policies/recommendation. If I remember correctly, the original article regarding 4% alcohol sclerosis was more of an anecdotal paper versus a formal research study.
CPT 64640 and CPT 64632 fall under the nerve destruction code section. Are there studies that show that 4% alcohol concentration is truly “destructive”? It is my understanding that 4% is not. I believe the package insert states that 40% concentration is required to be truly destructive to the nerve.
So a provider can:
-File a formal appeal asking for a one time override of the policy for this particular patient.
-Perform this service and inform the patient ahead of time that their carrier deems this procedure investigational and the treatment would be rendered on a cash basis.
-The patient could file their own appeal.
Since you signed a contract with this carrier, you agreed to abide by their policies. So the final option is not to treat BCBS patients with this procedure.

RE: The Passing of Don Thomas Peacock Jr., DPM
It is with extreme sadness and personal heartache that I announce the loss of one of podiatry’s best and brightest, my close friend Dr. Don Peacock of Whiteville, North Carolina. Don preceded me as president of the Academy of Minimally Invasive Foot and Ankle Surgery and was one of the most brilliant members of our profession. He was always my “go to” person for advice on a complicated case, and I could always depend on an answer based on his extensive experience and scientific evidence.

Don came to the academy in 2013, and quickly became one of our “superstars” as one of the first multiple year residency trained traditional surgeons to recognize the value of minimally invasive surgery and incorporate it as the main focus of his practice, which attracted patients from all over the country. Of course, we who knew Don best knew that Don’s first love besides his family was not podiatry, but music. He was the lead singer and guitar player in a rock band, and his seminar lectures were always preceded by a southern rock guitar solo.
Don had a great inquisitive scientific mind, always looking for ways to improve our MIS procedures and though he will be greatly missed by his family, his patients, and his professional collegues, we will all continue to benefit by Don’s extensive research, his many scientific articles, and his wonderful YouTube channel. RIP my good friend.

Billionaire Mark Cuban HAS started a company that sells generic drugs at cost. Some of them are useful for our patients. For details, go to https://costplusdrugs.com/
They do not accept insurance but the prices are usually cheaper than the co-pay if you had good insurance. For example, Colchicine which now cost about $176 for 30 tablets cost $8.70 (plus shipping). Antibiotics, anti-inflammatories, and steroids are among the 100+ drugs that he now sells.
I found most of the medications I personally take, and the savings were huge.

Dr. Ivar Roth's opinion that Dr Hodgson's attempt to bill for a rendered E&M service is what's causing insurance companies and patients to lose trust in doctors is both sad and laughable. It's laughable because Dr. Roth stopped accepting insurance assignments a while ago, so his reimbursement for a neuroma injection is undoubtedly significantly higher than Dr. Hodgson's meager insurance payment.
I'm assuming that inadequate payments were the primary motivation for Dr. Roth's withdrawal from insurance contracts and the establishment of his concierge spa practice. I'm not sure insurance companies need to trust doctors at all since we are the ones trusting the insurer to make the payment after the services are rendered.
