PA Podiatrist-Turned-Lawyer Says He Still Wants to Help People
 
Dr. Lawrence Kansky wasn't interested in the law as a young man. He studied medicine instead, hanging a shingle as a podiatrist in Hazleton, Mountain Top and Blakeslee. After 24 years, an encounter with the state Attorney General's office changed his life. Kansky decided to retire from his practice in November 2007, five months after being charged with obtaining a controlled substance by fraud or misrepresentation. "My heart wasn't in it," he said.

Rather than settle quietly into retirement, the then-50-year-old Mountain Top man decided to go to law school to help people trapped in similar situations. "I made the decision to go to law school to help people," Kansky said. "I also had to go to law school to understand what happened to me. I just had to do it." He would have liked to say thank you to his podiatry patients for having the privilege to serve them for more than two decades, but never had the chance. "I miss my practice but I'm still serving people," he said.

NY Podiatrist Buys marijuana.nyc Domain Name
 
The city sold marijuana.nyc at auction for $60,920 and made 40% of that sum per its deal with domain name administrator Neustar.

Marijuana.nyc was bought by Upper West Side resident and podiatrist Dr. Bruce Pinker. Others purchased weed.nyc, pot.nyc and cannabis.nyc, which sold for a combined $13,516.
"All I can say right now is that legalization of recreational marijuana is a 'no brainer' for New York state, as well as most of the 50 states," said Dr. Pinker.

Novel Anti-Fungal Wins FDA Panel Support

Astellas' novel drug, isavuconazonium (Cresemba) for treatment of invasive aspergillosis won the unanimous endorsement of an FDA advisory committee. A second indication, treatment of invasive mucormycosis, also won approval from a majority of the members of the Anti-Infective Drugs Advisory Committee by a vote of 8-2, but that tally suggests a greater enthusiasm than that which was expressed during deliberations.

Invasive Aspergillosis is caused by molds that grow indoors and outside. Healthy people can breathe in aspergillus spores daily without any serious consequences, but individuals whose immune systems are already compromised are at risk for infection. Mucormycosis, also known as zygomycosis, is triggered by fungi found on dead leaves and decaying wood. It is an uncommon infection, but it is "one of the most life-threatening mold infections in patients with hematologic malignancies and in [hematopoietic stem cell transplant] recipients," FDA said in briefing documents.

Query: Parkinson's Disease and Gout-like Symptoms
 
I saw two patients last week with a history of Parkinson's disease and a complaint of transient inflammation of the great toe which occured in the middle of the night. These symptoms resolved on their own in a few hours, and the patients presented with no evidence of inflammation. Does anyone have any knowledge of a gout-type attack or other inflammation of the toes related to Parkinson's?

Query:  At Career Crossroads
 
I am at a crossroads in my podiatric career. I am 58 years of age. I've been in practice for 30 years. I have worked hard to get to where I am and the status I enjoy in the medical community. I train residents and see the wonderful opportunities that they are afforded that were not there when I graduated. I'm grateful to the profession and I'm proud of how it has grown over the years; however, I am at a crossroads.
 
I need to decide which way to take my solo practice. I need to decide whether or not I should expand or just fade away into the distant sunset. I estimate that I have another good 10 years of practice. My retirement nest egg is modest. Yet, I have the opportunity to expand my office, double the size with the anticipation of bringing on an associate in the next year or so to carry on. However, given the practice of medicine these days (i.e., dealing with insurance companies, declining reimbursement, ICD-10,  increasing business expenses, PQRS, Meaningful Use, OSHA, PECOS, DME requirements, diabetic shoes, malpractice, etc.), it just might be better for me  not to expand,  but to wind down, keep my expenses low, and ride out the storm. What are other people in my situation thinking?

After over 30 years of reliable usage, my Oralix broke about 3 years ago. I was told that the head was unfixable, but I may have held onto the collimator. I had the antennae replaced a few years prior to that by the same company that sold me the unit. I will look to see if I still have their name. They were a dental company, but I had no trouble getting the antennae. They sent it to me (for a very reasonable price), so they may still sell the whole collimator unit. During the process, I was told that the Oralix brand was sold to another company, and eventually I was directed to an x-ray repair company out of Minnesota. I'm sorry that I can't recall all the actual names, etc.

The enrollment fee is mandatory for all DMEPOS providers and has been so for several years. There never was a category of "physician supplier" as Dr. Werner's post suggests. Rather, one should simply understand that your DME supplier is a separate category of provider. Despite the efforts of many physician organizations including, APMA, AMA, and AOA to name but a few, CMS and Congress several years ago enacted a mandatory fee for all those wishing to enroll in Medicare. There are several exemptions, including physicians enrolling with their Part B Medicare Administrative Contractor. 
 
As for stricter enrollment guidelines, the NSC now requires that all new suppliers and those who are revalidating have a physical inspection of their facilities. Some new suppliers in certain categories (not yet determined) will require fingerprinting, supervised by yet another new Medicare contractor. The enrollment fees, fingerprinting, and inspections are just a few of the efforts being taken by CMS and The National Supplier Clearinghouse (NSC) in an effort to reduce initial enrollment of those with a checkered past. 
 
Honest providers and taxpayers alike should applaud those efforts. However, the NSC must be careful not to unduly burden physicians and most honest suppliers with more costly regulations which increase the costs of doing business and are somehow still circumvented by those bent on being fraudulent.

It is with great sadness that I read of the passing of Bob Levoy. He came to the UK in the seventies and opened our eyes to practice management. Those who heard him speak have never forgotten his philosophy and his ideas. He was also one of the best speakers I have ever heard... witty, informative, and entertaining. He is sadly missed but his writing is his legacy.
