NY Podiatrist Issues Frostbite Alert

A frostbite alert has been issued for all of New York State by Dr. Gary Stones, President of the New York State Podiatric Medical Association. Dr. Stones recommends that anyone over the age of 65, people who have diabetes or poor circulation, and children under 13 years of age should remain indoors or limit time outside to no more than 20 minutes. Care should be taken to keep feet and hands covered, warm, and dry. Scarves, gloves, and hats should be worn.

If you are exposed to the frigid temperatures and develop any numbness, tingling or discoloration of your feet and/or toes, contact a podiatrist. Soak feet in warm water. Do not apply hot compresses or soak feet in hot water.

NY Podiatrist Collects Supplies for Sandy Victims

For North Castle residents, Hurricane Sandy seems like it took place when it did — last year. However, for New York residents of places such as Far Rockway, Freeport, Long Beach, Island Park and Breezy Point, Hurricane Sandy is still happening right now. Dr. Kathy Reilly Fallon, co-founder and chairwoman of Armonk charity Heavenly Productions Foundation, has made sure to remember that.

Fallon’s foundation’s latest efforts occurred last week when she visited H.C. Crittenden Middle School. Deborah DeFrancesco, a sixth-grade teacher at the middle school, helped Heavenly Productions organize a collection of food and supplies for Hurricane Sandy relief. Items such as toothbrushes, toothpaste, baby wipes, cleaning agents, cat and dog food, facial tissues, and bathing goods were donated by sixth, seventh and eighth-grade students.

Aussie Podiatrist Discusses Children's Shoes

With less than two weeks to the start of Spring term, parents have been busy trying to find the best footwear for the year ahead. Tasmanian Podiatry Association president Andrew Chappell said it is important for shoes to be the correct size. "And make sure they're not too large," Chappell said. He recommended shoes with laces or Velcro straps with strong support in the heel and midfoot. "Shoes should bend at the toes not through the midfoot."

EHR Incentive Payments Near $10.7B: CMS

The federal government's electronic health-record incentive payment program has paid out nearly $10.7 billion to more than 190,000 hospitals, office-based physicians, and other “eligible professionals” through December, according to the latest report from the CMS.

Both numbers climbed substantially in a month, with total payments up 15% from $9.3 billion in CMS' November totals, and paid providers, including those in Medicare Advantage plans, up 7.7% from 176,561 in November.

QUERIES (CLINICAL)

Query: Pediatric Digital Deformity

I have a 2 year old patient with a slightly plantarflexed 3rd toe at the MTP, causing a slight abduction of the 2nd toe, and a slight adduction of the 4th toe both feet. This is creating a slight overlapping of the 3rd toe by the other lesser toes. The patient's toes are still very flexible, and there are no fixed contractures, however the deformity does not correct itself.

X-rays were normal. The patient is not in any discomfort, however the parents are concerned this may cause a problem in the future. My questions are: 1) Does this problem need to be corrected at this time, and if so, in what manner? 2) Can treatment be delayed, and if so, what type of treatment protocols are recommended?

QUERIES (EMR)

Query: Management/Billing Software Program for ASCs

I have a single OR accredited surgery facility attached to my office. We only perform 8-12 surgeries per month in this facility. I have been using a management/software program called Advantx by Source Medical, but have been finding their customer service leaves much to be desired for the price I pay for the program. Does anybody have a similar set-up to mine who can suggest a better management/billing software program geared toward ASCs?

Query: Medicare Denial of L4360

We billed L4360 with modifier "KX" and "RT". This was dispensed for a heel fracture post open reduction internal fixation. The claim was selected by Medicare for complex medical review. We sent in office notes, proof of delivery, and order form...and received an EOB stating service is not medically necessary. What are we doing wrong?

Response: Medicare requires that the place of service be the patient's home ("12"), not office. Was this done?

L4360 is a prefabricated device and falls under ankle-foot orthoses. Did your documentation follow this guideline? "Ankle-foot orthoses (AFO) described by codes L1900, L1902-L1990, L2106-L2116, L4350, L4360, L4386 and L4631 are covered for ambulatory patients with weakness or deformity of the foot and ankle, who require stabilization for medical reasons, and have the potential to benefit functionally."

I was surprised to read the suggested treatment by Dr. Morelli for an elongated hallux. Considering that  he mentioned that the patient is a “young lady”, I cannot understand the rationale for the fusion of a functional and asymptomatic IP joint of the hallux when several other effective and function-preserving procedures are available. Perhaps he can indicate his reasoning.

I have read many great suggestions for correcting this condition, and most have seemed quite reasonable. However, in a day of rising healthcare costs and litigation, I have to ask why Dr. Morelli suggests "correcting" this patient's metatarsus adductus, and why he suggests using "trinity" bone graft to fuse an interphalangeal joint. I admit I may be missing something here. I am a young practitioner and have much to learn yet.

I would opt for a shortening phalangeal osteotomy and 1st metatarsal shortening osteotomy. But if IPJ fusion were chosen, I would see no problem using a simple 4.0 cancellous screw or crossed K-wires which can be buried. I cannot fathom suggesting surgical correction for metatarsus adductus for this patient. Addressing the chief complaint and clinically symptomatic pathology with a weight-bearing surgery would suffice. I think we all need to ask ourselves, "Which procedure would I want performed on my own foot, and which would I be willing to pay out-of-pocket for?"
