Kent State Signs Agreement with EIP to Open Podiatric MRI Center

Bryan Caldwell, DPM, MS, Interim Dean at Kent State University College of Podiatric Medicine, and  David Blue, President of Extremity Imaging Partners, Inc. (EIP), announced that Kent State and EIP have entered into an agreement whereby EIP will open and operate a podiatric-specific MRI center within Kent State’s College of Podiatric Medicine Cleveland Foot and Ankle Clinic in Cleveland. The MRI center will be available for the community’s patient referral use as well as research and educational purposes by the faculty at the College of Podiatric Medicine. The MRI center is expected to open at the end of March 2014.

Bryan Caldwell

EIP is a specialty MRI provider operating extremity-only outpatient MRI centers with expertise in podiatric imaging. Through the use of dedicated extremity MRI technology, the company provides podiatric MRI services in the markets it serves. EIP has been providing podiatric-specific MRI services since 2001 and currently owns and operates twelve (12) extremity imaging centers in Ohio, Pennsylvania, and Kansas.

PODIATRISTS AND FOOTWEAR
Minimalist Shoes Not for Everyone: DC Podiatrist

Minimalist shoes are designed to honor the form and function of the foot. They are the antithesis of the comfort shoe. Advocates say that cushioned supportive shoes encourage runners to land on their heels, which can lead to bad form and chronic injury.

Dr. Erika Schwartz

However, minimalist shoes are not right for everyone. "We've seen enough patients with Achilles tendon issues and stress fractures from running in these," podiatrist Dr. Erika Schwartz says. Their use for walking and standing has not been nearly as well studied.

INTERNATIONAL PODIATRISTS AND HUMANITARIAN CAUSES
UK Podiatrist Heads on Charity Mission to West Africa

A kind-hearted Llanelli woman is preparing to jet overseas and use her medical expertise to help some of the world's poorest people. Bryony Wilson, from Stebonheath, will fly over to West Africa in February to support people with little or no access to medical care. The three-week trip to Burkina Faso has been organized by Christian charity World Horizons and Ammanford Evangelical Church.

Bryony Wilson

As part of the visit, Bryony, a podiatrist, will be flying out alongside a group of eight people who specialize in finance, administration, teaching, and film-making. Bryony hopes to use her own knowledge of podiatry to help people in the poverty-stricken areas. "As far as we are aware, there are no foot care clinics in the country, and we are aiming to reach people who have no or little access to medical care due to location or being unable to afford it," says Bryony.

CODINGLINE CORNER
Query: Coding for Total Contact Cast

What coding is recommended for application of a total contact cast applied to the foot for an ulceration on a Medicare patient?

Donald Triolo, DPM, San Diego, CA

Response: The code for a total contact cast application is CPT 29445. It does NOT include the cost of supplies. You are allowed to bill the appropriate cast materials code - Q4038 (for Medicare for fiberglass material) - in addition.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
RE: Elimination of RFC Patients
From: Bob Hatcher, DPM, Brian Kashan, DPM

Hire a trained pedicurist to take your RFC patients. We started with one a few years ago and now have three. We are considering a 4th soon. You'll be happier and your patients will be too.

Bob Hatcher, DPM, Raleigh, NC,  bob@blhatcher.com

I have this debate periodically with some of my colleagues who have the exact same complaint as you. Namely, that they are inundated with RFC patients and new patients have to wait weeks to get in. This results in many of the new patients going to another doctor, as they can’t wait weeks for an acute problem. The loss is huge.

I have several suggestions. If possible, perhaps you can stretch out the time between visits for the RFC patients. Let’s say from 8 to 10 weeks. Then, RAISE YOUR FEE FOR THIS SERVICE. As RFC is a non-covered service, you can...

Editor's note: Dr. Kashan's extended-length letter can be read here.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
RE: Minimal Incision Surgery and the AAFAS Experience
From: Eric M. Hart, DPM

I recently had the pleasure of attending the Academy of Ambulatory Foot and Ankle Surgery seminar in New Orleans. I found them to be a wonderfully congenial group. I had heard enough from naysayers that have either little or no personal experience with minimal incision surgery (MIS) that it is either dangerous or archaic. I went to see for myself. I've been curious after seeing more European publications advocating MIS procedures for hallux valgus repair and reading Maffulli and Easley's book MIS of the Foot and Ankle. I was impressed with what I heard, saw, and was able to do in the cadaver lab.

Now, I haven't jumped the "traditional surgery" ship, but I'd like to issue the challenge to anyone who is either curious or has no personal experience with modern MIS to visit aafas.org and sign up for one their seminars. See for yourself what techniques might help your patients. If you only prove to yourself that it's not for you, I can guarantee that you'll meet some great people in the process.

RESPONSES/COMMENTS (NEWS STORIES) - PART 1
From: Kenneth L Hatch, DPM, Shashank Srivastava, DPM

There are podiatrists nationally who do follow the doctrine and philosophy of the Republican party that less government is better for all Americans. Dr. Rogers stated that he favored a single-payer system.  People who began practice in the mid '70s, before the surge of HMOs,   IPAs, and limited panels - before the lowered Medicare rates became the benchmark rather than the UCR as a standard - do not necessarily want to support someone by vote and/or donation just because he is a podiatrist.

Kenneth L Hatch, DPM, Annapolis, MD, KLHDOC@aol.com

I completely understand Dr. Silhanek's sentiments about this subject. My feeling is that we may all have significant personal differences with Dr. Rogers or Dr. Wenstrup about many of our convictions. I am sure I have my differences with both of these men. However, I also  believe that this is a unique and unprecedented time to have two podiatrists from both parties who are in Congress. This can give us significant political exposure that the profession may not get again for generations. My thoughts are that the profession should embrace both these men and have them be a legislative juggernaut for podiatry.

RESPONSES/COMMENTS (NEWS STORIES) - PART 1A
From: Harold W. Vogler, DPM and Jeff Christensen, DPM

The name ABFAS has been a long time coming. Combined examination for “foot-ankle orthopods” and podiatric foot-ankle surgeons is the next logical important step going forward. This likely will also come slowly, but it is necessary. Changes within ABFAS by-laws will be required (and CPME would have to approve this as well) to provide the opportunity for fellowship-trained foot orthopods to sit for Board Certification examination in Foot or RR Foot & Ankle Surgery. This could begin the process of "ultimate parity" between foot and ankle orthopods and DPMs. Few foot orthopods will do this in the near-term here in the USA due to the political landscape with the AAOS.

Additionally, “official recognition” within the orthopedic community in the USA would also require certain document changes within their organizations to provide for this. This will present a significant resistance barrier politically for obvious reasons, but it needs to be done. But foreign orthopods are very interested in this certification, and this concept could help start the process and serve to level the playing field of qualification for all and go a long way in starting the process of achieving quality assessment Internationally for all who are performing foot and ankle surgery. Ultimately, there does need to be some form of combination examination of both, the orthopedic and podiatric communities in this specialty. Combined certification would help ensure proper quality to surgeons performing foot-ankle surgery. The public trust is at stake and we all owe our patients the highest level of quality possible.

RESPONSES/COMMENTS (NEWS STORIES) - PART 2B
From: Joel A Feder, DPM, Ivar E. Roth DPM,MPH

I totally agree with Dr. Feder about the certificate replacement for ABPS. We both go back to the days that our first executive, John Bennett, worked for gratis because the organization could not afford his salary. Now there is plenty of income to support Dr. Feder's recommendation.

Ed Fischman, DPM, Jupiter, FL, docfish20@msn.com

I agree with Dr. Feder. The ABPS charges outrageous fees for their re-certification, something close to $1,000 for a 100 or so question test at a computer testing center. I do not think they thought this one through as we are not replacing a lost certificate. We must get the certificate re-framed/laminated, etc. which is another cost. Possibly due to the sheer volume of certificates to be issued, they might engage a company to offer the certificates with lamination at a great price as an incentive to getting the new document. I would like to get the business to laminate 10,000 certificates @$100 each - that is a million dollars.
