MI Podiatrist Discusses Frostbite

Frostbite is a serious tissue destroying disorder. It is also something that can be avoided. The Michigan Podiatric Medical Association (MPMA) has some advice to share during these winter months.

“When you're out in the cold, your body works hard to stay warm by altering blood flow toward your heart and lungs,” said Dr. Jodie Sengstock, MPMA Immediate Past President. “This leaves your extremities – arms, legs and feet – vulnerable to cold injury, especially toes and fingers.” Depending on the severity of the exposure, frostbite can affect the skin or underlying tissue. In most cases, the area becomes numb and feels frozen. Skin will appear waxy, white, or grayish. Any exposure should be evaluated and treated by a physician.

VA Podiatrist Leads Annual Shoe Drive

For the rest of January, area schools and businesses in central Virginia will be collecting shoes. The effort is part of the annual shoe drive called “Let Someone Fill Your Shoes.” “We think the shoe drive is valuable every year because it gives people an opportunity to recycle and get some new ones if you need some, and if you have some that you're not using, dig them out of the closet and they will be used,” said Dr. Bryan Snyder, podiatrist, Albemarle Family Foot & Ankle.

“We provide really big bags for people to just take droves of shoes back to their communities if they need to,” said Dr. Snyder. It's important that people have shoes that fit well. Dr. Snyder says otherwise, it could cause a lot of foot problems down the road. “Foot problems start with having ill-fitting old worn out or just unsanitary shoes…rubbing, irritation on the skin, blisters, corns, calluses, toenail trauma, and pain,” said Dr. Snyder.

NY Podiatrist Sponsors Local Shoe Collection Drive

More than 350 pairs of boots and shoes were donated to Grace Church Community Center by Dr. Bruce Pinker’s patients at Progressive Foot Care in White Plains.The annual shoe drive provides footwear for the needy through the community center’s Open Arms men’s shelter, Samaritan House women’s shelter and the Soup Kitchen.

Dr. Pinker hosted an eggnog and cookie reception to thank his patients for their donations. Local Boy Scouts are helping Grace Church Community Center sort the shoes and boots for distribution.

ONC Admits Problems with EHR Test Tool

The Office of the National Coordinator for Health Information Technology at HHS is acknowledging that some snags on its end are creating a bottleneck in its program to ensure that electronic health-record systems used by hospitals, office-based physicians and other professionals are able to help them meet Stage 2 meaningful-use criteria.

The ONC sent a memo Thursday to five independent organizations testing and certifying EHRs for the program, acknowledging problems with custom-made software to be used by those organizations as well as software developers to test EHRs. ONC expects the coming fixes to a version of the tool that can be downloaded (a separate version is available to run online) should be completed by Feb. 15.

CodingLine New York Seminar Breaks Attendance Record

Despite frigid temperatures, over 200 podiatrists and assistants packed the Broadway Ballroom at the New York Marriott Marquis for this year's annual pre-clinical conference coding seminar. The morning session featured Dr. Harry Goldsmith, who provided a 2013 Medicare Update, Dr. Paul Kesselman, who provided a 2013 DME Update, and Dr. Barry Block, who discussed Meaningful Use Audits.  

In the afternoon session, attendees heard clinical presentations in dermatology by Dr. Bryan Markinson, and clinical presentations in foot and and ankle surgery by Dr. Allen Jacobs. Codingline panelists also participated in these presentations.

It’s easy to overlook the people you value most, the team members who work with you every day. So remind yourself to recognize them. People skills coach Kate Nasser recommends this exercise:

  •  List your team members, with their photos beside the names, if possible.
     
  •  Identify two positive traits for each person and one thing that is unique about that person.
     
  •  Watch for examples of the people exhibiting those traits.
     
  •  Take the opportunity at those times to point out why you respect or appreciate the trait the person has just shown.

I am saddened to inform the profession that my former associate, Joe Perlow DPM, passed away peacefully this last weekend from cancer. Joe was a West Point graduate and had a 25 plus year career in the army as a full colonel before he entered podiatry school. Joe was an extremely nice, gracious person who represented our profession well to all who met him. Joe will be missed.

1. Correct the met adductus.

2. Make a template to shorten the proximal phalanx so as to recreate a normal parabola. For the sake of discussion, let's assume you have to remove the distal 50% of the bone. Cut the proximal phalanx in half, remove the cartilage from the base of the distal phalanx, and then perform an IP fusion. Add 1 cc. of trinity bone graft, or stem cells, or whatever your personal preference is to assist in the fusion. Partial amputation on a "young lady"?

RE: Patient Safety and HIPAA Privacy Laws (David Gurvis, DPM)

Dr. Gurvis asks an interesting question regarding PHI that can be released to family members - whether the information would be beneficial or not. It is not up to the family or their podiatrist to determine competency. My understanding is that PHI cannot be released by the holder unless a consent to do so is signed by a competent party. It is the family's responsibility to have the patient examined by a mental health professional capable of making this determination.
 
As covered entities, we are required to train our employees about the nuances of HIPAA. I would strongly urge that every office hold a meeting to discuss HIPAA regulations every six months. The attendees should sign a sheet indicating their participation.

As I read the well-intentioned comments on this subject, I came to the understanding that in America, we have the finest healthcare in the world, and we are going to change it. Success in any new delivery model is available if you have the correct strategies. The following companies were started in depressed economies: FedEx, Apple, and SouthWest Airlines.

Sometimes, the worst of times can become the best of times if you have the correct focus. This has been my passion and quest for four years, and over 200 shows on Meet the Masters. At least 80% of the DPMs who have volunteered their time as guests are making more money than ever and are positioned to do even better this year. If you are anything but positive about podiatry, it's time to change your focus. 

Get a mentor, hire a coach, change your focus. Read each issue of Podiatry Management and listen to old Meet the Masters shows.  Whether 2013 is your best year or your worst year is up to you. The resources are available.

RE: CMS Reduces Value of Nail Trimming by 50 Percent

In the Medicare Physician Fee Schedule final rule issued by CMS, the value for CPT 11719, Trimming of nondystrophic nails, any number, has been reduced by 50 percent. This action was taken unilaterally by CMS without guidance and input from APMA or AMA’s Specialty Society Relative Value Scale Update Committee (RUC). An unanticipated change of this nature is a change in protocol from previous CMS actions, which generally involve the RUC process. APMA has submitted comments to CMS to express opposition to this change.
