Smelly Feet Could Indicate Something More Serious: OH Podiatrist
 
The common courtesy of kicking off shoes at the front door of a friend’s house is a smelly feet sufferer’s worst nightmare. What causes it to become rank is usually overactive sweat glands and their secretions mixing with bacteria on the skin, adds Nicholas R. Butler, DPM, assistant professor at Kent State University’s College of Podiatric Medicine.

The biggest risk smelly feet present is that of infection, which can open the door to a slew of other health issues, depending on how long it’s left untreated. He stresses that the risk of infection is higher in those who have diabetes, skin diseases, or those who are obese. If you notice any irritation, swelling, or other signs of infection on or around your feet, or persistent foot odor after following the above tips, Dr. Butler says it’s worth seeking medical attention.

NY24 Clinical Conference Another "Broadway Hit"
 
The (New York) Foundation for Podiatric Medicine and New York State Podiatric Medical Association welcomed nearly 2,000 people from across the U.S. and several countries this past weekend to NY24, its annual clinical conference in Times Square. With nearly 60 faculty from around the country and 130 exhibitors it offered premier podiatry educational programming, lectures, workshops, networking, and interactive learning. Northwell Health, a leading NY healthcare system held their “Shining Light on Wound and Diabetic Foot Care Symposium.” Rodica Pop-Busui, MD, a featured NY24 speaker, serves as President of the American Diabetes Association.

The conference also offered an APMA Coding Seminar, a Radiography Program, a Medicare Update for 2024 from NGS, a Young Members Track including a Complications Seminar for residents, and podiatry career awareness activities organized in collaboration with the NY College for Podiatric Medicine. For years, scanning via mobile phone for orthotics purposes can only be done on an I-phone, We know that Android phones are far more than seventy percent of total mobile phone sales. When will scanning technology be available on them? This is getting to be a little strange considering how many thousands of apps are now available for Android users.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: Decreases in Reimbursement by Medicare Were Not Approved by Congress

CMS cuts that had been rescinded previously have not been covered in the recent additional funding approved by Congress. Late on Thursday, January 18, a Continuing Resolution (CR) to sustain governmental funding passed the Senate and the House. The CR funding is now extended to March 1 and March 8. This is the third approval for short-term spending for 2024.
 
The fix for a 3.37% cut in the conversion factor for Medicare in 2024 is not incorporated into the additional funding. Failure to address this cut could lead to significant reductions in reimbursement. APWH is monitoring this closely with PM News updates.

The widespread practice of digitizing slipper casts in the production of custom foot orthoses should not be a reason to abandon slipper casting. Instead, at least for podiatrists, the focus should be on selecting a lab that specializes in positive casting techniques. There are still a few remaining. Positive casting retains the intricate details and contours captured by slipper casting, ensuring higher accuracy and customization in the orthoses. (I don’t think a scan can accurately capture the subtle positioning of a fully pronated MTJ and manually delineating the first ray or elevating the second MTP while maintaining a neutral STJ).
 
Using a proper lab leverages the strengths of traditional slipper casting while avoiding the potential shortcomings of digital scanning, such as loss of subtle anatomical details when conforming to the closest foot contour in the lab digital library. By choosing a lab that commits to positive casting, podiatrists can ensure that the full benefits of their meticulous slipper casting efforts are realized in the final orthotic product. I have tried both methods and various labs and found there is still no substitute unless your priority is cost, ease, and convenience.

I welcome Jeff Root's response, but the point he misses is my STJ neutral casts, with mild plantar flexion of the 1st Ray, if scanned by a quality lab, in my experience, are superior to the scans being done in DC's offices, and some DPM's offices. I have had multiple patients from both. Maybe a larger problem is forgetting biomechanics are complex and using assistants to cast or scan is not good for the profession.

I must take issue with my friend Jeff Root when he states, "the end product depends on the same technology whether the doctor uses slipper casting or in-office scanning." When my colleague and I first developed the STS casting system, it was with the intent of providing a method and material to simplify and make more efficient casting for functional foot orthotics by maintaining the standard of casting established by Merton Root, DPM that relied on factors designed to mimic mid-stance foot positions by attempting to achieve STJ "neutral position" and "stabilizing the mid tarsal joint".
 
I do not dispute that CAD/CAM is used by many labs to scan casts or use scans provided by practitioners. However, it has become very obvious that practitioners often neglect accurate positioning of the foot using techniques that were developed by Drs....
