Dehydration Can Worsen Plantar Fasciitis: NY Podiatrist
A lack of H2O affects just about every system and part of your body, and your plantar fascia is no different. "Dehydration can lead to diminishing the lubricant fluid around tendons and ligaments that prevent overuse injuries," Nelya Lobkova, DPM says. This translates to potential micro-tearing and scarring of the plantar fascia, she says.

Fix it: "Drinking six to eight ounces of water every few hours is a good trend to follow to prevent dehydration," Dr. Lobkova says. But you may need more water if you're active and perspiring a lot. "Sweating creates a loss of water in the body that needs to be replenished," she says.

Gary M. Lepow, DPM, MS, has been appointed as the first podiatrist to the medical staff at the University of Houston College of Medicine (UHCOM). Dr. Lepow will join the faculty as a Voluntary Clinical Professor in the Department of Clinical Sciences.

Query: Denial For a Separate and Distinct Evaluation and Management
I saw a patient for follow-up of a neuroma, 3rd intermetatarsal space left foot. During that visit, she received a cortisone injection for G57.82 and M79.672 (CPT 64455 with J1100 x 1 unit and J3301 x 1 unit). During the same visit, she had a new complaint. The patient had a painful 4th left toe with callus.
I performed an evaluation and management for the hammertoe and callus for which I billed Aetna CPT 99213-25 with diagnosis of M20.42 and L84. I received a notice that the claim was partially denied, stating CPT 99213 is not eligible for payment because it is inclusive to CPT 64455. Medical records were sent and they continued to deny CPT 99213, stating it is inclusive to CPT 64455. I have asked for a higher level of appeal or to have a peer to peer, but have been informed that the appeal rights have been exhausted. Any thoughts on this and how I should proceed?

Response: Having practiced for over 30 years in that exact town, I know the dilemma well. Dr. P described the actions to take quite well. I would love to tell you that the insurance commissioner will help you, but good luck with that. I would call Aetna and “demand” a peer-to-peer review and to speak with a supervisor. I would make that call WITH the patient present with you and have them advocate for you and them at the same time. When encountering such resistance, especially in your demographic, I found the only recourse was to get the patient involved. Often, it worked.

RE: Major Changes in Undergraduate Medical Education Over the Past Several Decades

Medical education in the U.S. and Canada has changed considerably in the last several decades. Among the major changes are the following:
-
Reducing medical school programs leading to the medical degree to three years. Since graduate medical education (i.e., residency) is many years in duration and includes virtually all the information, that would be part of the typical fourth year..

Introducing clinical medicine early in the curriculum.
-
Including medical information and activities into the basic science component of the curriculum.
-
De-emphasizing inactive learning by markedly reducing the number of lectures and employing problem-based learning (PBL) which typically takes place in small groups (e.g., 6-8 students led by a single faculty member).
-
Employing objective structured clinical examinations (OSCE) in which students are asked to solve a problem in which they are faced with a simulated patient and are asked to solve a clinical problem. Students are evaluated as to how well they communicate/interact with patients, take a medical history, arrive at a clinical diagnosis, and come up with a treatment plan. The simulated patients are trained to act as if they were actual patients. The OSCE includes individual students interacting with a single patient, emulating a real patient-doctor interaction. How well the student performs is evaluated by a faculty member observing the activity via video and by the simulated patient who evaluates the student doctor for such activities as his/her communication skills.
