Modern Lifestyles a Major Contributor to Cancer: NY Podiatrist

Board certified podiatrist Kordai DeCoteau, DPM — founder and CEO of Sisters on the Vineyard, a health and wellness advocacy group — spoke  from the perspective of a cancer survivor. According to her, our modern lifestyle isn’t helping at all. “I was tested for genes out of the wazoo, and was negative,” Dr. Kordai said of her stage 2 cancer diagnosis. “I did not have a child before 35, I was overweight, I was stressed out — as most women are who are working and taking care of the households. In addition to that, I was exposed to x-rays,” she added.

Dr. Kordai DeCoteau

“When you put everything together… diet, lack of sleep, stress, work — what kind of work could you be doing that’s exposing you to this risk?” Dr. DeCoteau questioned. “Once you understand breast cancer more, hopefully you can navigate it better,” she said, adding that folks should “get tested earlier.”

SC Podiatrist Helps Monks During Nationwide 'Walk for Peace'

North Charleston podiatrist Jamelah Lemon Youmans, DPM and fellow Lowcountry physicians played a key role caring for Buddhist monks on a 2,300-mile 'Walk for Peace,' coordinating intensive medical support during their Columbia stop. The volunteer team treated blisters, dehydration, and fatigue, turning the visit into a powerful example of Charleston-area medical professionals rallying around compassion and community service.

Dr. Jamelah Lemon Youmans

For two days, Drs.  Boston, Sellers, Bryant, and Lemon Youmans were all part of the team administering IV fluids to the monks, checking their vitals, caring for their blisters, calluses, and other injuries. “It was a lot of kind of pandemonium before the monks arrived, but it was like once they came into the room, there was kind of this sense of calm and peace and we all just kind of sprang into action,” Dr. Lemon Youmans said.

DIABETES AND PODIATRISTS
Podiatrist-Led Initiative Launches to Help Prevent Diabetes-Related Amputations

Diabetes Guardians LLC, a physician-led public education organization has announced the national launch of DG2001, a free, multilingual patient-education initiative designed to prevent diabetes-related foot complications and lower-extremity amputations through early recognition, patient education, and risk-stratified podiatric care. DG2001 is supported by five years of investigator-led research conducted by Paul Y. Han, DPM, Clinical Professor in the Department of Diabetes at City of Hope, and published in peer-reviewed journals.

(L-R) Drs. Paul Han and John Giurini

Reported outcomes included a 2.8% diabetic foot ulcer incidence—representing a 32% reduction compared with national benchmarks—and a 0.43% major amputation rate, exceeding the Healthy People 2030 target established by the U.S. Department of Health and Human Services. The initiative is further informed by national clinical guidance, including the 2025 diabetic foot care guidelines of the American Diabetes Association (ADA) coauthored by John M. Giurini, DPM, Associate Professor of Surgery at Harvard Medical School.

RELEVANT RESEARCH - PART 1
Inflammation-to-HDL Ratios Linked to Diabetic Foot Ulcer Risk

Key Points:
  • People with higher levels of certain blood markers — including neutrophils, monocytes, and platelets compared to their HDL (“good”) cholesterol — were more likely to have diabetic foot ulcers.
  • One marker, the monocyte-to-HDL ratio, was especially telling: if it went above a certain level (0.29), the chance of having a foot ulcer was more than four times higher.
  • Low red blood cell levels may help explain part of the link between inflammation and foot ulcers, since red blood cells are important for wound healing.
  • These simple blood test ratios might help doctors spot patients who need extra foot care or monitoring.
  • The study looked at health data from over 1,200 U.S. adults with diabetes, using a national survey.
  • More research is needed to confirm the results, figure out ideal cutoff values, and learn how best to use these markers in daily care.
As noted in a commentary by diabetic foot expert David G. Armstrong, DPM, MD, PhD, research into inflammation and lipid-related blood markers offers a practical new way to think about identifying people at higher risk of foot ulcers — especially since more advanced tests aren’t widely available in everyday clinics.

PRACTICE MANAGEMENT TIP OF THE DAY
Tips for merging your medical practice without losing your people or your patients

Here are some practical tips to help physician owners and administrators plan, close, and integrate the right way.

1) Start with the awkward question: Why are we doing this?

Before you get pulled into valuation math and attorney calendars, slow down and make sure everyone is solving the same problem. Is the goal to spread overhead across more clinicians? Gain leverage with payers? Build a deeper bench for call coverage? Create a real succession path?

If the two sides don’t share the same “why,” you’ll feel it later, usually when you hit the first hard decision about compensation, staffing, or service lines. A helpful conversation-starter is Physicians Practice’s guidance in “Identifying the right reasons for a practice merger.”

CODING & BILLING Q&As FROM CODINGHELPLINE.COM
Query:  Use of CPT 97760/97763 for Custom Orthotics

Is it appropriate to use code 97760 (Orth Train/Mgt, initial encounter) for an orthotic pick-up appointment and code 97763 (Orth Train/Mgt, sub encounter) for 4-6 week orthotic check/re-eval? Are you aware of any payers paying these codes and do they all require the GP modifier?

Codinghelpline com Subscriber

Response from Codinghelpline.comWhile the term orthotic management and training is in the description of CPT codes 97760 and 07763, when dispensing custom functional foot orthotics and coding/billing for code L3000, it is not appropriate to use those codes. CPT codes 97760/97763 are used only when a provider needs to go beyond standard fitting of the orthotics.

FINANCIAL TIP OF THE DAY
What Is a Good Inheritance? Six Great Assets to Inherit

Leaving a good inheritance takes planning. If you want to help your heirs, these are the best assets to leave them.

5. Roth IRA

A Roth IRA is a retirement account funded with after-tax dollars. In exchange, your retirement withdrawals are income-tax-free, including your investment gains. This tax-free treatment continues when your heirs inherit the Roth IRA, making this another effective asset to leave behind. On the other hand, if you leave behind a 401(k) or traditional IRA, your heirs would owe taxes for taking the money out. Anyone receiving an inherited retirement plan (except a spouse) must also withdraw everything within 10 years of their death, whether they need the money or not, which can push them into a higher tax bracket.

If you have both types of retirement plans, consider spending down your taxable accounts or using them for charitable donations while keeping your Roth IRA balance for a future inheritance. Another option: if you have money in taxable retirement plans, consider a conversion to a Roth IRA. You pay income tax upfront on the amount you convert, but after that, the IRA assets and future growth will not be subject to income tax when withdrawn.

SOLE PURPOSE BY SAVANNAH SANTIAGO, DPM
10 Things You Need to Practice Medicine That Money Can't Buy

Here are some valuable tips that have aided Savannah Santiago, DPM in navigating her experiences as a student and a resident:

1. A Place I Feel at Peace

Growing up in San Diego, it’s no surprise that I love the beach and the ocean. There’s nothing more peaceful to me than floating in the water or lying on a surfboard, feeling the gentle lull of the waves—or diving beneath the surface and watching the seagrass sway with the tide. Since moving to Indiana, I’ve lost that beloved 20-minute drive to the shore, but I’ve found new places that bring me peace.

These days, it’s the paved trail next to my apartment. I cherish my sunset walks, when I can pause, take a deep breath, and remember how lucky I am to be alive—to feel the air in my lungs and the stillness of that moment. In the winter, my peaceful place shifts to a cozy chair by the window, wrapped in a heated blanket with a warm cup of tea in hand, watching the world go by outside.

Wherever you are, find a place that brings you peace—it doesn’t have to be the ocean. Sometimes, it’s just a quiet corner where you can breathe and be present.

RESPONSES/COMMENTS (OBITUARIES)
RE: The Passing of John Bergmann, DPM

I am deeply saddened to have learned of the passing of Dr. John Bergmann. He was the second-generation owner of Bergmann Orthotic Laboratory. John was not just a colleague but a friend for close to 40 years. He spent countless hours helping so many of us with difficult biomechanical cases. He fully gave all of us his time and expertise so that we could do the best for our patients.

Dr. John Bergmann

John was a real mensch. Whenever I had medical or even personal issues, he was there for me.  He never  would say, “ I am too busy to be with you in your hour of need.” He was a real people person. In the forty years of our friendship, never did I hear a bad word about anyone come out of his mouth. Even if he had professional disagreements with other biomechanical gurus, he might have disagreed, but would never ever make it personal.

During the last few months of his illness, many of us tried to reach out to him but he did not want his dire problems to be a burden to any of his friends and colleagues. He only accepted our prayers. May his family be comforted, and may his memory be a blessing for all of us.
