Cold Weather Exacerbates Overuse Injuries: MN Podiatrist
 
"Some people find they run better in cold weather conditions," says Dr. Paul Langer, a podiatrist with Minneapolis' Twin Cities Orthopedics. "I have set most of my personal records in spring races...The only thing that keeps me from running outdoors in winter is ice," Langer says. "I have seen too many walkers and runners with ankle fractures when it is icy out."

It's okay to run in those conditions, Langer says, but caution should be taken before running in freezing weather. Many runners report lower body injuries, especially injuries to the foot, ankle, or knee. Those runners need to take heed. "The cold can exacerbate overuse injuries," Langer says. "I advise patients with knee or ankle issues to keep their lower extremities insulated in winter." Extra socks and more layers are the best way to warm up down below, he says.

Olson earned his medical degree from Des Moines (Iowa) University College of Podiatric Medicine and Surgery. He completed a three-year residency in comprehensive foot and ankle surgery at Regions Hospital in St. Paul.

EHR Adoption Rate Slows - Physicians Facing Big Hurdles for Meeting Stage 2 
 
The pace of adoption of electronic health-record systems has begun to slow, and the physicians who have adopted systems have a long way to go to meet the government's Stage 2 criteria for meaningful use of the technology, according to an authoritative survey of practices by the National Center for Health Statistics at HHS. In 2013, the third year of the federal EHR incentive payment program, nearly 8 in 10 office-based physicians had adopted some form of an electronic health-record system, and nearly half of them had a “basic” system with key EHR functions defined.
 
The latest installment of the annual National Ambulatory Medical Care Survey, conducted between February and June last year, also showed that most physicians were far from meeting the incentive payment program's Stage 2 “meaningful use” criteria. For physicians who had met at least two years of Stage 1 requirements, Stage 2 got underway Jan. 1.

QUERIES (EMR)
Query: Meaningful Use for 2014
 
I achieved MU for the years 2011, 2012, and (attested for) 2013. When my office manager reviewed requirements for MU in 2014, she told me that it will probably take a lot more time/work to achieve MU in 2014 than previously. Here is my assessment, and I would be interested in yours: 
 
The amount of MU payment that I would be eligible for in 2014 would be $4,000 (probably reduced to $3,920 due to sequestration). The additional hours of staff time and my time required would cost me far more than the MU payment. If I felt that the additional requirements in 2014 would lead to a “meaningful” improvement in the quality of my care, I would suffer the loss. However, scanning the additional requirements, I feel the additional time expenditure would detract from time spent on patient care. I question whether it is wise for me to work to meet the 2014 requirements. I am in solo practice, and will probably only practice for an additional year or two.

I could not agree more with the letter written by Dr. Arden Smith. There were many erroneous statements in the article. For example, they are talking about how much was charged for certain procedures, but we all know that the amount paid by most carriers is about 30% of the charges. The amount of time we spend on chart writing, pre-authorizations, dealing with denials, and numerous other issues is growing exponentially! In the Queens and Long Island areas, North Shore, NYU, and Mount Sinai networks are buying any profitable medical offices, and the physicians are becoming salaried workers. The whole paradigm of medicine is changing drastically and not for the better. 
 
In recent years, I had the chance to speak to many physicians; most of them are very successful, yet about 70 -80 % were very unhappy about the general status of medicine and would NOT recommend their own children to follow them. There are always exceptions, but I am referring to the average practitioner. The NY Times, not surprisingly, is becoming more and more like the old Pravda in the Soviet Union, serving the complete incompetent socialist Obama administration. This might be an offensive statement for some, but if you speak to anyone who emigrated from the old Soviet Union, most will concur.

Dr. Silhanek writes of her thoughts regarding the support for Dr. Lee Rogers in his bid for Congress.  She has some intelligent concerns. While we all may not be in his district, we are all governed by Congressional legislation. I vote and support candidates who hold the same values and goals that I possess. I hope my colleagues do the same.

I read the comments of Dr. Silhanek regarding Dr. Rogers and Dr. Wenstrup in their pursuit of political office and feel she made some very good points. I have had the pleasure of hearing  Dr. Wenstrup speak at meetings, and have talked to him personally. I have the utmost respect for him personally. I do not agree completely with his political views, but he did get great exposure in podiatric publications such as this one, with it either stated or implied that he would be good for healthcare, good for podiatry! 
 
I realize that he has only been in office a short time, but what has he done? I haven't seen a thing regarding actions on his part that would forward our cause. Has he been out bashing "Obamacare", a popular thing to do? If, like so many other politicians, only critical words being leveled without constructive ideas to fix a broken system, it is all meaningless hypocrisy, in my opinion. If, on the other hand, he has been doing productive things, generating new ideas to remedy that which is broken, I'd sure like to hear about them.

In response to Dr. Alison D. Silhanek’s question about whether anyone has asked candidate Dr. Rogers about his position on the issues, the answer is yes. I specifically e-mailed Dr. Roger’s campaign for input on his positions regarding income tax reform, illegal immigration, the national debt, and other issues not specifically connected to podiatry. There was never any response to that e-mail inquiry of over 2 months ago, and accordingly I never provided any donations to Dr. Rogers.  
 
Yes, our profession does need adequate representation in the political system, but an unresponsive candidate to a fellow colleague is a warning sign, not only for podiatry, but for the overall national interest of who is qualified to be in elected office.

Oh my yes, documentation is CRITICAL. It will be vitally important that our documentation matches the ICD-10 coding scenarios we bill. I am in no way suggesting to bill to a documentation level but the opposite. One must document to the ICD-10 level used, be accurate and complete.
 
The APMA coding committee has a series of webinars that take place every other Thursday night and are free to members. It so happens that this week’s webinar is on documentation for ICD-10. We, as a profession are light years ahead of many other specialties and professions, and we will be prepared.

I offer a standing ovation to Dr. Sasiene and Dr. Garoufalis with respect to their letters and their 'campaign'.  Reading the NY Times article this weekend about how "egregious it is" for  doctors to make money from ancillary services turned my stomach. Oh those RICH doctors. 
 
The article described how a dermatologist doing Moh's surgery had to take the patient to the OR for a plastic closure and graft of their face. Not a word about...

RE: The Passing Of Martin, Mussman, DPM 
 
The death of Marty Mussman is the end of a bit of important history for the profession. He fought to have our profession included in Title 38, and that changed the way we were viewed - not just in the Department of Veterans Affairs, but it had a major impact on hospital affiliations and insurance recognition.
 
Public Health and Podiatric Medicine - Principles and Practice - Second Edition, published by APHA in 2006 contained a chapter prepared by Dr. Mussman. Chapter - "Podiatric Medicine - The Historical Development of Programs in The Department of Veterans Affairs," pages 93 - 99 was in truth a synopsis of the "early days." The information  should be read by everyone, especially our students. The current and future programs were included in the next chapter and authored by Dr. Jeffrey M. Robbins and demonstrated a seamless transition from the past to the future. 
 
Marty was a World War II veteran and the first full-time DPM with the VA and VHA. This chapter is perhaps the best eulogy and memorial that could be offered. He changed the system, fought a courageous battle, and demonstrated the integrity of podiatric medicine. I had the rare privilege of working with Marty for some fifty-five years. He was a role model for so many. He was what we all hope we can be.
