NY Podiatrist Joins Childhood Cancer Foundation Board of Directors
Al Musella, DPM is the newest member of the Bridge to a Cure Foundation Board of Directors. Musella is a retired podiatrist and technologist, founder and president of The Musella Foundation for Brain Tumor Research & Information, Inc., a 501(c)3 nonprofit public charity established in 1997 to help brain tumor patients through emotional and financial support, education, advocacy, and fundraising for brain tumor research.

In this role, Dr. Musella established the first clinical trial database for brain tumors, upon which the National Cancer Institute modeled Clinicaltrials.gov. He has issued more than $12.5 million in brain tumor research grants and co-payment assistance grants to help patients access needed treatments. He also helped secure Medicare Payment approval for Optune, Temodar, and Avastin, and advocated for FDA approval of many treatments.

OH Podiatrist Joins NOMS Ankle & Foot Care Centers
NOMS Ankle & Foot Care Centers, recently announced the addition of Brendan Barrett, DPM to its staff of physicians and surgeons. Dr. Barrett is accepting new patients in the group's Boardman, Columbiana, Youngstown and East Liverpool, OH offices. Before joining the practice, he worked as a podiatrist in Florida.

Dr. Barrett earned a Doctor of Podiatric Medicine degree from the Kent State University College of Podiatric Medicine in 2013 and completed his three-year podiatric residency at James A. Haley Veterans Hospital and Bay Pines VA Healthcare System in Tampa and Bay Pines. He is board certified by the American Board of Podiatric Medicine.

UK Podiatrist Overcomes Stereotypes About Muslim Women Who Run
Until two years ago, Uroosa Khan couldn’t imagine herself as a runner. “Muslim women are not represented in mainstream media in regard to sports and athletics,” she tells Runner’s World. “I had few role models to look up to.” The 24-year-old podiatrist from Leeds, UK, also worried about standing out as a novice: “Would I be too fast or too slow or not running properly?”

Khan needed an outlet to relieve stress. While working on her dissertation for graduate school, she was inspired by her friend Namrah Shahid, 26, who at the time was one of the few visibly Muslim women at local races. Her friend’s enthusiasm gave Khan the courage to finally run too. “If she’s doing it, so can I,” Khan remembers thinking. Now Khan is proud to be one of nearly 50 women in Hijabi Runners, a Leeds-based group for Muslim women founded by Shahid in 2019.

Managing Cash-Handling at Your Medical Practice - Part 4
When it comes to money handling, you can never have too many fail-safe protocols in place. The most important thing to remember is to never allow cash to be too accessible to staff. Here are some ways you can protect your cash by creating solid accounting processes:
4. Get a Receipt from the Bank.

Another obvious one, yes, but if you allow an employee the freedom to drop a bunch of cash in a deposit bag on the counter and walk away, you've opened yourself up to multiple opportunities for that cash to never make it into your bank account.

The employee can state, "I dropped it at the bank, I swear!," but perhaps the cash never really made it to the bank. A bank employee could have made the decision to leave his job and keep your money as severance pay.

A customer could have taken the deposit, when the bank employee turned his back. All of these can be avoided with a bank receipt that then gets attached as confirmation and documentation that money was deposited in the bank.

RE: The Passing of Livio Parolin, DPM
Livio Joseph Parolin, 70, of River Forest, lost his heroic fight against cancer on Jan. 1, 2021. As a child, he attended Our Lady of the Angels. On Dec. 1, 1958, Livio survived the historic fire that took place at the school, and it was a blessing to anyone who met him over the years that he did.

Livio went on to Gordon Tech High School, where his academic and athletic success on the football field led him to the University of Notre Dame. He went on to become a doctor of podiatric medicine, and practiced for the past 40 years, where he touched the lives, not just the feet, of so many patients.

Query: Post-vaccination Dystrophic Toenails
A middle-aged female presented with dystrophic toenails. She relates that she had COVID back in May with no cough but had a 103 degree fever for 2 days. She has the antibodies to COVID. She took the first dose of the Moderna vaccine about two weeks ago and got fever, chills, and pains in her leg that felt like they were "shooting out of her toes."

Her symptoms lasted for two days. She had no trauma to the toenails, and did not change footgear. She is healthy with no meds and no allergies. There is no pain associated with the nail beds, and the rest of the nail seems to be attached to the nail bed. She contacted the CDC before the nails had this deformity, and is going to contact them again. Has anyone else seen this?

Here is the link Dr. Williams was referring to about the study. Although it does provide good evidence, I'd caution to not change prescribing patterns based on one study at this time. If more studies repeat results or there are enough to produce a positive systematic review, then hopefully the guidelines/standard of care could change based on more recent results.

Many years ago, I was a part of the clinical study for Lamisil (terbinafine). We never found that it caused any hepatic pathology. At the time, the routine was to draw blood for a hepatic panel after 6 weeks. Following the study, that was changed to prior to the initiation of the medicine. It was shown that those who had an elevated hepatic panel at 6 weeks already had one prior to the medication. My protocol is to draw blood prior to the giving of the prescription. One side note: I had a patient who begged to be given Lamisil but she had a liver transplant 8 months prior and I refused. Finally, I agreed only if her liver doctors would okay it. They said, "sure, no problem." I use that example with my patients who question the safety.

The FDA removed its recommendations for LFTs attendant to terbinafine therapy several years ago. It remains up to the clinician as to whether or not they want to monitor taking the patient's medical history into consideration. The incidence of hepatotoxicity necessitating discontinuing treatment is very low. In fact, in 25 years, I have had only seen one incident of that. The liver is also a very resilient organ. I have several post-treatment Hep C patients whose hepatologists gave the green light to terbinafine therapy without a concern.
I always get LFTs prior to therapy if none exist for the prior six months. I do not monitor these during therapy. Having said all that, there are RARE reports of severe idiosyncratic hepatotoxic reactions to terbinafine. This is why starting terbinafine therapy without laboratory confirmation of dermatophyte infection, although all too common, is in my opinion, sub-standard.

RE: Volunteer to be a Vaccinator

As part of the NYC Medical Reserve Corp, I gave 220 Moderna vaccine injections over two days, Saturday and Sunday, at the Port Richmond High School on Staten Island. Even though the lines were long, every single person was extremely grateful, thanking us profusely. Our group vaccinated almost 1,500 people in two days.

There was a learning curve, as expected, in order to become more efficient as the days progressed. I taught the other vaccinators a more efficient way of injecting, while maintaining aseptic technique. I am volunteering again, as soon as more vaccines become available. We need more vaccinators!
