NY Podiatrist Discusses What Drew Her to NYCPM
As a promising undergraduate at Fordham University, Jackie Prevete’ DPM's decision to attend the New York College of Podiatric Medicine (NYCPM) felt straightforward. “The experience is unmatched,” she said. “They’re in that area of the city where the pathology gives students really great exposure to everything that podiatry has to offer. The clinic is busy, even at the student level. You get used to evaluating and seeing patients, and that’s what you’re going to need to be good at for the rest of your life. NYCPM affords that from the get-go. New York City is unmatched.”

For her, that involvement provided perspective. “I wanted something besides academics,” she said. “It helps you understand the bigger picture. It’s easy to go in and see patients every day, but what affects the profession as a whole can affect what you do.” Her advice to students is simple: keep academics a priority but engage beyond them. Last year, Dr. Prevete joined Northwell Health as program director of the podiatry residency and director of podiatry for the Eastern Region, roles that blend clinical leadership, administration, and mentorship.

Aussie Podiatrist Stresses the Importance of Exercises to Keep Feet Strong
About one quarter of our bones are in our feet. How can you keep them strong and pain-free for longer? Dentists have trained us well to visit them on a regular basis for a check-up, rather than waiting until things start to hurt. Dr. Hylton Menz, a professor of podiatry at La Trobe University in Melbourne, wishes we’d do the same for our feet. “We probably ignore our feet relative to other parts of our body, because we stick them in shoes and they’re a long way away, so we tend to forget about feet,” he says. “It’s only really when they don’t actually function properly – so when they become painful, they don’t do what we want them to do – that we really think about them.”

Our entire body weight rests on the soles of our feet, so maintaining a healthy weight is an important part of looking after our feet. It’s not just for mechanical reasons but also metabolic, Menz says. “There’s really good evidence now that adipose tissue – fat tissue – is active stuff, so it tends to become activated on nerve endings, so there’s a really strong connection between foot pain and being overweight,” he says. “There’s lots of benefits to having a healthy body weight, and one of the benefits is you’re far less likely to get foot pain.” We spend so much time on our feet yet rarely think of them as a body part that needs exercising. But there are some foot exercises that can help maintain the big and small muscles in the foot that are critical for balance and movement.

4. Optimize Prior Authorization
Unsurprisingly, prior authorization is a common source of physician practice revenue issues. Today’s prior authorization process is more complex than ever due to the industry’s transition to value-based care and the growing number of services subject to advanced medical necessity and appropriateness verification. Additionally, the process itself is time-consuming and often involves dealing with denials and appeals, contributing to a heavy administrative burden and new challenges for staff who must understand the clinical documentation and office notes necessary to support the request.
To avoid the revenue cycle impacts of prior authorization issues:

Standardize processes by working closely with payers to understand their requirements.

Implement status tracking to ensure timely approvals and identify patterns that can be addressed internally.

What Is a Good Inheritance? Six Great Assets to Inherit
Leaving a good inheritance takes planning. If you want to help your heirs, these are the best assets to leave them.
3. Brokerage accounts
Investments made through a taxable brokerage account, like stocks, bonds, and mutual funds, also make for attractive inheritances. They are easy to divide and value because your heirs can see the market price of these publicly traded investments. In addition, publicly traded investments are easy to sell and convert to cash. On top of it all, your heirs could receive a significant tax benefit on these inheritances. “Perhaps Dad has been investing all his life and has held shares of Apple, IBM, and Microsoft for years. If he sold during his lifetime, he’d owe considerable capital gains taxes,” says Michael Romero, President of Argent Trust Oklahoma at Argent Financial Group.
For example, someone who bought IBM Stock 20 years ago would have a cost basis of roughly $28 per share. If they sold at today’s price of about $304, that would be a taxable gain of $276 per share. However, if you hold these investments until you die, you receive something called a step-up in basis, which means the investment basis goes up to the market value on the day you die. “If your heirs sell soon after you pass away, they could sell these appreciated assets owing little to no taxes,” said Romero.

Ask for help when needed and remember the importance of timing and assessing who is willing to provide assistance. People notice when you want to learn and that you are paying attention!
13. Acknowledge Your Learning Stage - Remember that making mistakes is part of the learning process. Everyone makes errors, but it's essential to take ownership of them and avoid repeating them. Carelessness is very different than an honest mistake. Everyone has dropped instruments, cut sutures too long or too short, grabbed the wrong gloves, contaminated things, etc. OWN your mistakes. But try to not repeat them.

Query: Better Definition of "Limited or Minor Issue" in E/M Medical Decision Making
My billing software does not correctly classify issues as "acute uncomplicated" for many diagnoses and leaves them as "self-limited or minor". What should doctors learn about pre-assigned diagnosis classifications that appear wrong? Should the doctor update the diagnosis classification when this is not properly assigned? The concern is the software is automatically down coding E&M service. Is this a concern?

The words doctors document matters, and so does intent. Once treatment is part of the clinical decision-making process, the problem shifts in complexity. This requires the doctor to pay more attention to the software assigned diagnosis type. By no means should many of the diagnoses you treat be considered self limited or minor. A self-limited or minor problem is defined as a condition that runs its course typically without physician intervention. Think about mild rash, a bruised toe or even a cut area on skin. These are problems patients commonly manage at home without the need for medical care. They are minimal risk by nature, and they typically show up under codes like 99202 or 99212 with straightforward medical decision-making (MDM).
Here’s the way each encounter should be looked at: if the patient didn’t come in for it, would the condition have resolved on its own? If yes, we’re looking at self-limited or minor, and level 2 E/M. When the problem escalates to acute, uncomplicated, but still low risk, it becomes apparent that your treatment is needed as it will not resolve on its own. Now, let’s say that "cut" is becoming red, warm, and swollen. The patient tries an over-the-counter remedy at home, no change. At this point, they schedule a visit with the doctor. The physician now evaluates the condition and considers treatment, such as a topical or oral antibiotic. Same cut but an entirely different classification. Why? Because it’s no longer expected to resolve without intervention. Per the AMA, “a problem that is normally self-limited or minor but is not resolving” is now considered an acute, uncomplicated illness or injury, now level 3 E/M or higher.
