SC Podiatrist Discusses Bunions
 
According to Michelle Butterworth, DPM, "Bunions are a very common medical condition that affects approximately 67 million Americans. Bunions are typically seen rather noticeably at the base of the big toe. To fully evaluate the deformity, you should see a foot and ankle surgeon for a full examination including x-rays. This will allow a full assessment of the severity of the deformity and help select the best treatment plan."

"There are basically two types of treatments: non-surgical and surgical. Non-surgical treatments include changes in shoe wear, padding, orthotic devices, activity modifications, oral and topical medications, and possible injection therapy. The goal of non-surgical treatment is to reduce pain, delay the progression of the deformity, and reduce the chance of damage to the big toe joint. It is important to know that these treatments do not get rid of the bunion or correct the deformity. The only way to get rid of the bump and straighten the toe is with a surgical correction," says Dr. Butterworth.

Runners with Flat Feet Need Appropriate Footwear: NY Podiatrist 
 
Flat feet can range from “low arch to no arch,” says Bradley Schaeffer, DPM, podiatrist and foot surgeon at Central Park SOLE in New York City, who describes the condition as “when your ankle and your heel collapse in, which in turn collapses the arch and makes the toes go off to the side.” In the most extreme cases, having flat feet means the soles of your feet flatten out completely when you stand up, and it’s common for people with flat feet to also experience Achilles pain and plantar fasciitis.

Regardless of where you fall on the spectrum of flat-footedness, the best thing you can do is invest in appropriate footwear, Dr. Schaeffer says. And the sooner, the better — especially if you’re a runner. “Insoles are everything for people with flat feet,” says Dr. Schaeffer.  “I think [rocker bottom] are great for someone with Achilles tendonitis, plantar fasciitis, or flat feet,” says Dr. Schaeffer. “It just kind of propels you forward, and doesn't put too much pressure on the ball of the foot and heel.”

Re-classification of Phenol a Scandalous Waste: New Zealand Podiatrist
 
A legal intricacy around a commonly used solution to treat ingrown toenails has likely led to about $1.8 million annually in extra costs for patients, Podiatry NZ says. Podiatrist Mark Smith says the confirmation of phenol as a "medicine" rather than a medical device has caused increased delays and costs for patients, and more work for health professionals.

For years, phenol was available as a “medical device” in New Zealand, meaning it could be supplied directly to podiatrists. However, in 2023, a query prompted Medsafe to confirm it as a “medicine”. This meant it now had to be subscribed by a medical doctor. “To have to explain this situation to patients every time I treat an ingrown toenail is just embarrassing,” Christchurch podiatrist Mark Smith said. Not only was it an inconvenience to patients, he said, it added wait-time and cost to getting treated, and was “a scandalous waste of general practitioners’ time”.

RE: Celebrating 100 Years of the Akin Osteotomy

The Akin osteotomy was first introduced by Dr. Akin in 1925 as a straightforward yet effective procedure to correct hallux valgus deformities. This technique involves removing a wedge of bone from the big toe to achieve better alignment. Initially, the procedure did not attract much attention, as other intra-articular procedures, like the Keller osteotomy, were more commonly utilized.
 
However, in 1967, Colloff and Weitz highlighted the advantages of the Akin procedure, particularly its effectiveness in correcting mild to moderate deformities without causing disruption to the joint. This renewed interest led to further refinements of the procedure, including adjustments for correcting both...

RE: The Reality of Podiatry

When I read the current discussion about one board vs. two, I take two steps back. When I sold my office, the buyer was sure she would be doing ankle procedures all day long. She was trained to do all of it and presumed it would follow through in the office. About six months later, I looked at her website and it offered nail treatments, shoes, and all the routines that we know.
 
From an economic standpoint, think about 6 nail procedures taking 15 minutes vs. going to the O.R. for that ankle, spending a few hours and being paid a meager fee. Of the 20,000 or so ankle fracture procedures done over last year, more than 19,000 were done by Orthopods. When I go to my Derm. He gets the exam fee plus a-zap of liquid nitrogen for a total of $200. He spends 15 to 20 minutes with me. I am just dealing with the reality of our situation.

I have said for many years that forcing all podiatrists to take a 3-year surgical residency is a mistake. All the numbers I have seen show that the average podiatrist generates less than 30 percent income from surgery. Also, as an attending podiatrist in our residency, we have had to deal with two bright young people who simply were not suited to be surgeons. This poses a horrible dilemma for our program…do we fail these young people and doom them to be unable to practice since they will not be able to get a license, or do we pass them, ascertaining they are competent surgeons when they are not. This should not happen!

I read with interest the many opinions on the relative lack of interest in podiatry school, as evidenced by the low number of applicants compared to other medical programs. I think that all the opinions expressed are valid. I'm sure there is no one reason for this lack of interest. It is true that some pre-med students don't know about podiatry, but I believe it is also true that many pre-med students DO know about podiatry, and don't want to spend a large portion of their day simply cutting toenails.
 
We all know that nail care is important, especially in certain patient populations. But we also know that cutting/trimming/debriding nails doesn't require an extensive, rigorous, and expensive 4-4-3 training regimen. Understanding and recognizing pathologies as manifested in nail deformities, and knowing how to treat those deformities is absolutely important and demands the well-trained...

While I was a student at the Illinois College of Podiatric Medicine, I was provided with a unique exposure to treating professional ballet dancers. The school was located across the street from the Chicago Ballet. These professional people had some of the most stressed feet I remember ever seeing. Being professionals, they were also demanding and wanted to get back to being able to perform. Of course, we could not blame them for having such an attitude and we were happy with the challenge. It was a great learning experience and opportunity.
