Managing Cash-Handling at Your Medical Practice - Part 3
When it comes to money handling, you can never have too many fail-safe protocols in place. The most important thing to remember is to never allow cash to be too accessible to staff. Here are some ways you can protect your cash by creating solid accounting processes:
3. Set $500 limit if petty cash must be kept in the office.
Get the rest of the cash collections into the bank immediately. This reduces the amount of cash loss you could suffer if someone decides to walk out with it. Do not let it add up! If your usual procedure states that once a month you make an actual bank deposit, but you're now sitting on a few thousand dollars, change your procedure.

Query: Class B Findings Clarification
It was my understanding that each pedal artery, dorsalis pedis and posterior tibial, are independent of each other as they relate to the opposite extremity in determining class B findings. In other words, having an absent dorsalis pedis (DP) artery on each foot would amount to two class B findings. I was advised recently that this is not the case. Having an absent DP on each foot would amount to one class B finding and a second finding would be needed to meet the class B criteria. How can that be when with the same named artery is an independent variable so to speak?
Additionally, in the CMS guidelines, it does not state for class B findings BOTH absent posterior tibial or dorsalis pedis pulse equals one class B finding. It simply states absent posterior tibial pulse or absent dorsalis pedis pulse. Clarification would be greatly appreciated.

Response: The short answer is that the Class Findings are PER EXTREMITY. So, if you have Class Findings on one extremity, then that single limb will qualify for an exception to the routine foot care exclusion. You cannot use the findings of one extremity and apply those findings to the other extremity. I do understand that usually if one limb has vascular disease, then the other limb probably has similar findings. But, CMS had to utilize some type of objective clinical findings to quantify arterial vascular disease without the need for advanced testing. In the Medicare Benefit Policy Manual, for “Presumption of Coverage”, it states: “In evaluating whether the routine services can be reimbursed, a presumption of coverage may be made where the evidence available discloses certain physical and/or clinical findings consistent with the diagnosis and indicative of severe peripheral involvement.”
So, if you have DP pulse 0/4 right and 1/4 left, PT pulse 0/4 right and 2/4 left, this indicates 2 Class B findings on the right limb, but not on the left limb. In this case, you can bill for routine foot care services on the right limb only. I realize that these qualifications for the exceptions to routine foot care probably leave a lot to be desired, but these are the rules that we currently have to deal with.

Query: Prescribing Protocol for Oral Antifungals
Has anyone changed their prescribing protocol for oral antifungals? I read a recent study where the authors did not see appreciable increases in liver enzymes throughout a 3-month course of Lamisil therapy. These patients did have higher than normal liver enzymes prior to starting the medication. If that is true and oral antifungals do not cause an increase in liver enzymes, then is there really a need to order a LFT prior to prescribing these medications?
Is anyone prescribing Lamisil (or any other oral antifungal) without first requiring a liver function test? Or, for that matter, prescribing oral antifungals to patients with already high liver enzymes (out of normal range)? This potentially creates a huge shift in how most of us practice. Are you aware of other studies with similar findings?

RE: Faxed Ciox Requests Wind up In Spam Folder

Lately, a good number of faxed chart requests from Ciox have been going in my spam folder. I am very reluctant to answer Spam alert phone calls or open emails in my Spam folder. Because of this, I am contemplating drafting a letter to Ciox health explaining our office policy moving forward is to only acknowledge requests from them that are sent via certified mail. This procedure will make certain all requests will be received and handled in the proper way. If our profession thinks this strategy is in order, maybe we as a group should adopt this protocol. I would appreciate any input from other podiatrists.

I do respect the comments of Allen Jacobs, DPM, but DPMs do need to be careful about what they say. Whether we administer vaccines or any of the many other drugs we may employ (and my understanding is that we are indeed allowed and hopefully capable of safely administering other drugs by mouth and parenterally), we also need to be able to be prepared for adverse reactions that patients may experience. If we are not, then it may be questioned by others as to why we are so permitted to administer such agents.
A large percent of COVID-19 vaccinations are given by pharmacists and many other healthcare workers who are not specialty trained physicians. Furthermore, we are experiencing what is perhaps the most serious public health emergency in the history of the world and almost a quarter of all COVID-19 deaths have occurred in the U.S. The capabilities of the podiatric physician should not be under-estimated.

In this case, both Drs. Levy and Jacobs are correct. Podiatrists are as well trained as any other medical professional in the early management of a severe allergic reaction to any injected medication. As a severe allergy sufferer, I've often asked the professionals who give me my allergy shots how many times they have encountered anaphylaxis. Most said never, but they have guidelines as referenced by Dr. Jacobs hanging in the office, "just in case."
In our office, as with most of my colleagues, we stock epi and injectible antihistamines, and pulmonary support devices and have the protocol hanging in the office. In over thirty years, I have never had to treat a single case of anaphylaxis. On the other hand, I have had to perform CPR three times and was successful two out of the three times and was assured by the ER docs that my only failure had nothing to do with my technique but rather an issue of arriving at the scene way too late.
