AT THE COLLEGES
NYCPM Professor and Department Chair Retires

Simon M. Nzuzi, DPM, who graduated from the New York College of Podiatric Medicine in 1979 and had been Professor of Community Health and Medicine there since 1981, retired at the end of the year. He was born in the Republic of Zaire, now known as the Democratic Republic of the Congo.
Dr. Simon Nzuzi

In addition to teaching, his research interests have been laser therapy for onychomycosis, and nail diseases. He has also been very active in the community, including service as Chairman of the Board of Christ Church United Methodist in New York City. After a big retirement party at the College in December, Dr. Nzuzi reflected on his career in a thoughtful, gracious poem sent to the entire NYCPM community.

APMA STATE COMPONENT NEWS
OPMA Launches Drive to Collect Shoes

Dr. Kim Gauntt of Oregon Foot Care Centers, and the Oregon Podiatric Medical Association (OPMA) have launched a drive to collect 25,000 pairs of shoes that will be delivered to the anti-poverty organization Soles4Souls. “OPMA is supporting Soles4Souls because it is the definition of a social enterprise where solid business practices are used to create positive change in people’s lives,” said Dr. Gauntt.

Dr. Kim Gauntt

The shoes that OPMA receives will go to Soles4Souls and will be distributed to small business programs that create sustainable jobs in Haiti and other nations. “The simple truth is that almost anyone with a closet has shoes they don’t wear, or an old pair that will just end up in a landfill,” said Dr. Gauntt.  “Give those to us, and know that you are taking a step to making the world a better place for all of us.”

PRACTICE MANAGEMENT TIP OF OF THE DAY

7 Ways Docs Can Attract Patients on Facebook - Part 2

2. Establish specific goals before starting.

Before you create a Facebook Ad, choose from several advertising goals and Facebook will display the actions you care about more prominently in your campaign summary page. Do you want more page likes, website visits, or new patient appointments?

A leading diabetes educator hired an agency to get more local patients to register for his workshop in Philadelphia. The agency created one targeted Facebook ad two weeks before the event, and received way more registrations -- 136 -- than he could fit in the room.  But it only worked because he really knew his audience (step 1), and structured the goals to get event signups (step 2), and not page likes or something irrelevant to his business goals. Have your end-goal in mind, and work from that foundation. Anything else will be a waste of time and money.

HEALTHCARE NEWS
New Test Identifies Early-Stage Melanoma With Metastatic Risk

Because two-thirds of melanoma patients who die or experience metastatic disease are initially diagnosed with early-stage disease, it is important to determine risk for metastasis in patients with early-stage melanoma. About 75% of patients with melanoma have early disease (stage I or II) at diagnosis.

A prognostic 28-gene signature (DecisionDx-Melanoma, Castle Biosciences) could predict which patients with stage I or II melanoma are at high risk for metastasis, and could alter the clinical management of these patients, according to a confirmatory study published in the January 1 issue of Clinical Cancer Research.

QUERIES (CLINICAL)
Query: Source for Neurological Testing Protocol/Equipment

Can anyone direct me to a good source for the format for a simple, yet complete, lower extremity neurological exam to perform in the office?  Other than a monofilament, tuning fork, neuro hammer with pin, is there any other equipment that should be considered essential for basic testing?

QUERIES (MEDICAL-LEGAL)
Query: Do Associates Get EHR Stimulus Money?

I moved to a new state in 2011 and began working for a group. The group attested under my name for the Meaningful Use incentives every year I was there. I was given a "bonus" of $1,800 the first year and nothing since. I went on my own a few months ago and, in looking at the incentive program for my new practice, I see that my former group is listed as a surrogate under my name. I also found out recently that when the EHR checks arrived, the front office staff was told not to tell anyone, especially the employed podiatrists that it had arrived.

If incentive money was collected by the group, do I have a legal right to recoup it? If the group has already attested under my name, am I eligible to do it for my new practice? Do groups typically pocket the EHR incentive money for their employees? This seems to be another case of podiatry "eating its young."

Editor's note: PM News does not supply legal advice. We suggest that you contact CMS and get the definitive answer to these questions. In general, the checks belong to the person or entitiy it is endorsed to, unless otherwise agreed to by the endorsee. Additionally, CMS will issue a 1099 form for taxes which names the endorsee. If the money is ultimately yours, you may need to consult with a healthcare attorney.

QUERIES (DME)
Query: DME Signage Problem

My office is located inside a hospital-owned medical building and my office door, like many others, is a solid wooden door with a practice identifier and suite number on it.  There are lease prohibitions against adding any type of signage that is “non-standard” to the building. This would include a DME “hours of operation” sign. I had this same problem in my previous office as well, and was never able to resolve it. What has the readership done in this type of building/lease door scenario?

CODINGLINE CORNER
Query: Billing Bioguard Gauze

We are beginning to dispense Bioguard gauze rolls (http://www.dermasciences.com/bioguard) from our office for Medicare recipients. We are using the appropriate A6266 code for this product. We are, however, having a very difficult time locating information on how many units patients are eligible to receive monthly. When appropriate, I thought we could dispense up to 120 units, regardless of number of wounds we are treating. We are only getting reimbursed for 30 units when there is only one wound. Our LCD does not mention how many units are reimbursable. Can anyone provide guidance?

Daniel C Albertson, Office of Paul Krestik, DPM, London, KY

Response: This product appears to be an impregnated gauze (other than with saline, water zinc paste or hydrogel). It can be billed once per day per wound. On a monthly basis, you would be limited to no more than 30 per wound. This is clearly stated in the Medicare DME "Surgical Dressings" LCD.

If you are treating four non-contiguous wounds which required four separate sponges (where cutting the sponges would not allow for adequate coverage), then you could be paid for 120 units. Your claim form would indicate this by using an "A4" modifier. Your documentation would, of course, need to support this level of service.

RESPONSES/COMMENTS (DME)
From: Paul Kesselman, DPM

The inspectors are there to ensure the public and Medicare that you are a real live breathing provider of DME. That being said, you must provide them with proof that your provide DME and that you are complying with the Supplier Standards.

1) Show them your inventory of DME products (in a closet or a display);
2) Have a framed copy of the current Supplier Standards on display in your waiting room and have a...
