Latino Community Especially Hard Hit by Diabetes: TX Podiatrist
Diabetes has been on the rise around the world, and Latino communities in the United States have been especially hard hit. A lethal combination of genetics, poor access to healthcare, diets high in processed foods, and sedentary lifestyles have created a crisis in places like San Antonio, a majority Mexican-American city in Southern Texas, that is costing a growing number of people their feet and legs — and eventually, for some, their lives.

“It’s a huge issue in San Antonio and dare I say, it’s the diabetic foot capital of the world in terms of complications,” said Michael Sobolevsky, DPM, a podiatry doctor at the Texas Diabetes Institute, the facility run by University Health in the heavily Latino neighborhoods of western San Antonio. “We are constantly amputating.”

Moisturized Feet Help Keep Feet Warm in Winter: WA Podiatrist
Some days it seems like no matter how thick your socks are, how dry your feet are, or how many blankets you have covering your legs, your feet just can’t get warm. One possible cause? Drier winter air, which can lead to cracked heels that are less able to hold onto body heat, explains Rion Berg, DPM, a podiatrist with Foot and Ankle Center of Lake City in Seattle. “And the effects can be most problematic for older adults with poor circulation.”

Dry, cracked feet are less able to maintain body heat, says Dr. Berg. So, when moisturizing your feet, look for products labeled balms, ointments, or body butters, which contain a higher ratio of oil to water than products labeled creams or lotions, which are primarily water-based. Indeed, oils provide more moisture and a stronger skin barrier, which are needed to restore cracked, dry feet. The only potential pitfall? These products can feel greasier on your skin and leave residue on fabrics, so it’s best to apply them at night and then put socks on to lock in the moisture. This way you’ll wake up with warm, soft, moisturized skin, and no greasy mess!

When shopping for slippers, think function over fashion, says Michael Trepal, DPM, a podiatrist and professor at the New York College of Podiatric Medicine’s Department of Surgery. But you should also like how they look, so it’s best to strike a balance between the two. Some slippers come with an open heel, enclosed heel, or in boot form.

Wearing slippers at home is a good idea, so long as they fit properly, according to experts. This is because slippers support your feet and eliminate the stress of walking barefoot on hard surfaces. They can also protect you from picking up infections, bacteria or fungi that may be on the floor — as well as cutting your feet on splinters, glass, or other sharp objects — specifically if you’re walking around communal spaces and high-traffic areas, like the hallways in an apartment building or dorm, according to Dr. Trepal.

Query: Difficulty Obtaining Credentialing with Anthem
I am having difficulty obtaining credentialing with Anthem to become an in-network provider. The stumbling block is my lack of board certification. I have explained that as a non-surgical, general practitioner podiatrist, a large percent of DPMs my age have no board certification, and that this should not block me from being credentialed. Any ideas on how I can persuade Anthem?

I suspect the patient is a smoker. If so, his smoking is likely the etiology or cause of his lesions. The reason is because smoking/anxiety can cause these in areas of the body where there are an abundance of sweat glands, plantar foot & palms of hand. This increased moisture leads to some of the glands being over productive and can cause blockage of the surface of sweat gland. Farther production moisture is unable to escape from the gland, leading to increased hyperkeratotic build-up and pain from weight-bearing.
Stopping the smoking will reduce the hyperhydrosis, thus allowing the majority of the punctate lesions to most likely resolve over a 2-3 months period of time. The main point is to control the hyperhydrosis to minimize the re-occurrence of these painful lesions.

There is no question about what Dr. McLean said is correct. The problem in knee pathology is greater in women due to their greater Q angles. Unfortunately, we do not have these patients walking into our offices as well as the hip and low back pathology due to biomechanics. I worked for a back surgeon and because I was helping so many patients, he fired me.
The difficulty is we as podiatrists and our biomechanical skills are not appreciated by surgeons. Sad but true. I know because I tried to talk to various medical schools.
