OH Podiatrist Cites 4 Reasons Why Feet Get Bigger With Age
You've always worn the same shoe size since, well, forever. Then one day you wake up and your tennis shoes feel a tad tight, or your sandals are suddenly a little snug. No, you're not imagining it. "Everybody's feet get bigger as we get older," says board-certified podiatrist and foot and ankle surgeon, Mark J. Mendeszoon, DPM.

"Typically, as we enter our fourth decade, it's not unusual for our feet to grow by one full size," Dr. Mendeszoon says. He cites four reasons: 1) Overuse 2) Weight gain 3) Hormonal changes and 4) Underlying medical conditions.

MI Podiatrist Changed Career Path to Study Hyponatremia
Marathoners know better than most to stay hydrated. Your body does need lots of fluid before, during, and after the race, but it's possible to overdo it. That can lead to a condition called hyponatremia. The Chevron Houston Marathon was one of the first to report cases of hyponatremia in 1999. American College of Sports Medicine fellow Tamara Hew-Butler, DPM was working at a marathon medical tent at the time. "It was hot that year," she said. "That was the first year that four runners were sent to the emergency room. They were in a coma for a week.

That's when Dr. Hew-Butler, also an associate professor of exercise physiology at Wayne State University, left her profession as a podiatrist and focused her research on hyponatremia - much of that research done in Houston. "We did a series of research that showed that some of these runners that were moving a little slower were drinking 100 cups of fluid during the race," she said.

OH Podiatrist Inducted into Rosecrans Hall of Fame
Zack Hill, DPM is the newest edition to the Rosecrans Hall of Fame. The 2012 graduate was an outstanding all around athlete, earning 12 varsity letters, four each in baseball, basketball, and football. Dr. Hill is currently a second year podiatric resident at Grant Medical Center.

Hill captained the 2012 basketball Bishops, which finished the regular season ranked Number 1 in the Associated Press Poll. He also was named to the All East Central Ohio League and East District teams and was selected to play in the Eastern District All Star Game.

Why You Should Consider a Private Equity Offer For Your Practice
3) How to scrutinize the offer - Even if the numbers work, practice owners should go into any transaction with their eyes open. Probably first and foremost, they need to evaluate a potential buyer to ensure that it will be a good cultural fit for the practice. Do they have the same commitment quality and service as you do?
By their nature, private equity firms make investments in businesses they believe they can make more profitable by improving efficiencies. That could take the form of investing in technology that will at once improve care and make the practice more efficient. It could also mean that they expect to increase doctors' patient loads to a level you're not comfortable with. These discussions need to happen ahead of any sale.
Practice owners should also take a close look at the employment agreement that a private equity buyer is offering. Since they’ll be merging into a larger organization and potentially working alongside other doctors, they should secure provisions that protect them from getting squeezed out or marginalized within the organization — in essence, ensuring that their new employer won't impinge on their productivity levels.

Query: Cuboidectomy
How would you code a cuboidectomy? I have tried to look up codes and asked around but have not come up with a good option.

Response: I was unable to find a specific code for a complete excision of the cuboid bone. There is a code for partial excision:
CPT 28122 Musculoskeletal partial excision (craterization, saucerization, or diaphysectomy) of bone (e.g., for osteomyelitis or tarsal bossing), tarsal or metatarsal bone, except talus or calcaneus.
If there is no code available, the only other option is to use the NOC [not otherwise coded] CPT 28999 and submit claim with an operative report, and request peer review for reimbursement consideration.

I was saddened to read in PM News that Dr. Stanley Bogusz passed away. Sometimes you know in one conversation that you are speaking with an exceedingly nice person. Dr. Bogusz was one of those people. My condolences to his family and friends.

As has been noted here in the past, there is no doubt a double standard and an uneven playing field when it comes to prosecuting physicians vs. insurance companies. Why is that? Because physicians and hospitals are the low-lying fruit, while the insurance companies can afford to fund lobbyists and legal teams with an endless stream of finances. They also have politicians in their back pockets.
CMS also wants to get out of the risk-taking business; thus, the insurance companies will get plenty of leeway from CMS and the OIG and get away with these financial crimes against society with no more than a slap on the wrist. Unless or until it can be shown that a Medicare Advantage Plan has caused undue harm to a patient, they will continue to get away with impunity.
There is still plenty that physicians can do about this. It starts with the novel idea that politicians, attorneys and the MBAs, actuaries, and others with advanced degrees in business are limited in their abilities. When was the last time you saw one of them examine or treat a patient, admit a patient to hospital, treat infections, perform surgical procedures, prescribe medications, write orders, along with all the other daily tasks only physicians can execute? And let’s not forget that politicians serve on our behalf. They should be properly remembered on Election Day!
