Six Canadian senior podiatric medical students from the Université du Québec à Trois-Riviéres recently completed a 15-week clinical rotation at the New York College of Podiatric Medicine as well as at Metropolitan Hospital, with which NYCPM is affiliated. They spent five weeks each in Orthopedics/Pediatrics, Surgery and Medicine.

ASFP Awards Grant to FL Podiatry Resident
The American Society of Forensic Podiatry (ASFP) has awarded Dr. Jessica Popelka, chief podiatry resident at University of Florida (UF) Health Shands Hospital, to present her poster presentation on forensic bare footprint analysis at the 2016 Forensic Podiatry Seminar & Workshop in Phoenix Arizona, May 19-21.

The ASFP board voted unanimously to bestow Dr. Popelka with a $500 grant to attend the conference and present her poster. The American Society of Forensic Podiatry offers grants for education and research related to the forensic analysis of foot-related evidence.

Proposed Medicaid Expansion Would Include Hard-to-collect Co-Pays: KY Podiatrist
Newly elected Republican Gov. Matt Bevin announced plans to remake Kentucky’s Medicaid expansion look more like nearby Indiana’s program, a more limited version that calls for patients to share more costs so they have a bigger stake in their care. Bevin is moving forward with plans to dismantle the state’s popular “kynect” health insurance exchange where residents sign up for both Medicaid and private coverage. If the governor succeeds and Medicaid changes begin next year, Kentucky would join six other states with specially-designed Medicaid expansion programs approved by the federal government. It also would be one of the first states to replace the traditional Medicaid expansion envisioned by the ACA with one that includes co-pays and premiums.

“It’s almost to the point where you’re paying to see the patient,” said Dr. Robert Miller, a local podiatrist. If Kentucky adopts an Indiana-style Medicaid expansion, Miller added, collecting co-pays will prove a challenge, based on his experience with the one Medicaid company he deals with that charges them. Doctors must document that they tried to collect, and the cost of sending out letters to those who can’t afford a $2 co-pay can be $5 or $10.

OH Podiatrist Recognized by American College of Sports Medicine
Dr. Eugene Kubitz was recently recognized for 25 years of membership in the American College of Sports Medicine.These 25 years include training and research as an exercise physiologist, foot and ankle medical and surgical specialist, and associate professor of biomechanics/orthopedic/sports medicine at Kent State University College of Podiatric Medicine.

He also recently attended the advanced sports medicine course titled “Sports Injury Clinic,” sponsored by the Ohio Foot and Ankle Medical Association. Kubitz has served as the volunteer team physician for the Perkins High School athletic teams since 1998.

AZ Podiatrist Disappointed By Flop of Inhaled Insulin
Last week, drugmaker Sanofi pulled out of its agreement with MannKind Corporation to sell the inhaled insulin product called Afrezza. The device, cleared by the FDA in June of 2014 on its third try, apparently flopped when it hit the market, earning only a few million dollars in 9 months on the market.

This is a drug with wonderful promise that will now not likely be realized. As a surgeon who specializes in preventing some of the end stage complications of this disease, I do not have occasion to prescribe it, but I do see the devastating results of diabetes, and therefore am something of a customer myself!"

In the Shift to Value-based Care, Providers Confront IT Challenge
A recent report in Health Data Management concluded that 2016 is set to be another year in which both healthcare providers and industry regulators continue to navigate the intricacies of transitioning to value-based care. Although this Affordable Care Act mandate will not be fully implemented until 2019, when providers will have to elect to use either a Merit-Based Incentive Payment System or an Alternate Payment Model, 30% of payments processed this year by the Centers for Medicare and Medicaid Services will adhere to the new standards, which account for both the cost and quality of care
As a value-based care system is only viable through data analysis, this shift presents providers with a considerable technological challenge. While the transition from paper began through the implementation of electronic health records (EHR), facilitated by the Meaningful Use program, providers will now have to develop a multifaceted tech plan that not only accounts for patient records, but one that also incorporates medical data with sophisticated accounting practices, manages administrative tasks, and coordinates with the platforms of other providers.

I have to disagree with Ray Posa's statement that all EPs are required to meet Meaningful Use. CMS has instituted a program that has a penalty for those who do not elect to meet Meaningful Use. You as a business owner should look at this program and the time and expense involved, then make a business decision as to whether or not it is in your best interest to implement the program or to take the penalty.
Once we elect to enter into a contract with CMS to accept Medicare, we are entering into a contract that sets our rates. If we do not like the terms of the contract each and every year...

RE: Empirical Terbinafine for Nail Fungus Is Most Cost-effective (Bryan C. Markinson, DPM)

I used to be a spokesperson for Novartis labs, back in the day, way before it became generic. There was an absolute protocol that Novartis set up which was to identify the fungus on the nails via a biopsy. Then the patient would need to take a liver enzyme test before starting the pill every 28 days for the next two months. If that was not done, the "Lamisil" was not covered. For the people who were also on "statins", there was greater than a 20% chance for increased liver enzymes. That immediately stopped the drug. I truly feel that nail biopsies should be done before the "new topicals" are prescribed. There is no difference. That goes for lasered nails as well.
