Proper Warm-up and Cool-Down Prevent Foot Pain: NY Podiatrist

If you are among the 72 percent of Americans who say they cannot exercise because of foot pain, those good intentions will never become reality. Now is the time to take action and get help. The foot specialists of the New York State Podiatric Medical Association are using January as a springboard for educating the public about the importance of foot health and safety when beginning an exercise program.

Dr. Gary Stones

According to Dr. Gary Stones, President of the New York State Podiatric Medical Association, there are simple strengthening exercises that can help prevent common foot injuries. “Feet bear the brunt of impact in any workout,” said Dr. Stones. “By warming up properly and cooling down with some massage techniques, you can avoid some of the most common foot pain associated with exercise.”

TEXAS PODIATRIC MEDICAL FOUNDATION NEWS

Texas Foundation Donation Challenge Winners

15 offices participated in the November Donation Challenge by putting cans in their offices to collect "change to make change." The First place winner was Dr. Jason Miller's office of Kingwood - Houston, Texas which raised over $10,000. In second place was Dr. Scott McKinney's office from Pasadena, TX.

Dr. Jason Miller (Back Left) with Staff

Dr. Miller said he hopes this will motivate everyone for next year to reach out to vendors and patients, as many small donations can add up.  As the top fundraiser, Dr. Miller received "Saving Soles" scrub tops for his staff, plus a resident scholarship was named in his honor. Proceeds support the Texas Podiatric Medical Foundation for future medical missions, resident grants, indigent patient support and other charitable projects.

Dr. Charles Ross was recently elected to serve a 2-year term as a Physician-At-Large position at Berkshire Medical Center (BMC), a category-1 trauma center with almost 200 physicians on staff. BMC serves a community of 50,000 residents in Pittsfield and approximately 100,000 residents in the surrounding areas.

PHYSICIANS AND THE INTERNET

Docs Skeptical of Online Ratings: ACPE Survey

According to a survey of physician executives, doctors are still highly skeptical of online ratings from patients and don't think many patients consult them—but most acknowledge that they have checked out their own profiles. The American College of Physician Executives sent the survey to more than 5,600 of its almost 11,000 members, with about 730 responding in October and November last year.

Only 21% believed that more than half of U.S. patients consulted an online-rating site, with 55% believing only one-quarter of patients or fewer had done so. That said, 69% of survey respondents acknowledged seeing what had been said about them online. Of those, the most-viewed site was Healthgrades.com, which had been visited by 89% of respondents; Vitals.com was a distant second and was visited by 33% of the respondents. These were followed by AngiesList.com, visited by 19%; Yelp.com, 13%;  DoctorScorecard.com, and RateMDs.com, which were both visited by 9% of respondents.

RESPONSES/COMMENTS (CLINICAL)

RE: Elongated Halluces (Robert Chelin, DPM)
From: Dwight L. Bates, DPM

The proximal phalange of each hallux appears to be long. A modified Keller procedure might be useful. It also appears the patient has metatarsus adductus which would explain tripping.

Dwight L. Bates, DPM, Athens, TX, dlbates04@yahoo.com

Consider shortening the hallux with a "Z" type osteotomy (Giannestras), removing bone from either side and fixating with two 2.0 screws. Also performing a shortening metatarsal osteotomy, i.e. Youngswick would be appropriate due the length of the 1st met and the significant metatarsus adductus.

RE: HIPAA Security Rule and Windows XP (Robert Steinberg, DPM)
From: Al Musella, DPM

Dr. Steinberg is correct. I didn't have space in my letter to the editor to go into details, but the correct procedure for upgrading any operating system is to install it on a spare computer or laptop, and test that all of your programs and hardware (printers, scanners, x-rays machines, etc.) work with the new version. Having said that, I have not found any program or any of my hardware that has worked under Windows 7 which didn't work under Windows 8. I did have one very old multifunction device that worked under Windows XP which did not easily work under Windows 7, but there was a way to fix it, and it works under Windows 8.

As to web-based programs not working under Windows 8, if they worked under Windows 7, there are simple ways to make them work on....

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1

RE: A Disturbing Incident
From: Mark Stempler, DPM

I'd like to share a disturbing incident. I recently saw a new palliative patient. His nails had just been recently debrided with areas of dried blood and areas of an overly aggressive treatment. He had this treatment performed at a nail salon. The most disturbing part of the story is that this patient has AIDS!

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2

RE: Podiatrists and Flu Shots (Paul Kesselman, DPM)
From: Elliot Udell, DPM

I understand Dr. Kesselman's concern that if podiatric physicians were to be given the right to administer flu vaccinations, some of us would wind up with unused vaccine, and this could be an economic hardship. I used to stock tetanus toxoid in my refrigerator, so I could rapidly vaccinate a person who presented with a dirty puncture wound. There is a short clinical window of opportunity to safeguard these patients. I found that it was a losing proposition, so I arranged with an internist in my building to take care of these people within 24 hours.

The market for flu vaccinations would be different because everyone is a potential candidate. Not getting vaccinated creates a public health problem and we see patients who do not go to doctors regularly, and some might be uncomfortable with getting an injection at a drug store. I also checked how much we would have to buy from Moore Medical. The minimal order is one ten-dose vial costing 101 dollars.

As for an internist's concern that non-primary care doctors should not be giving vaccinations, this argument has been settled when New York State allowed pharmacists to give flu, pneumonia, and other vaccinations to customers. Podiatrists are far more trained in giving injections than pharmacists.

RESPONSES/COMMENTS (SUCCESS TIPS FROM THE MASTERS)

RE: Medical School Entrance (Brian Kiel, DPM)
From: Brian Kashan, DPM

I have known Dr. Mullen for 30 years, and the mere implication that he is a racist is ludicrous and completely off base. He has proven so in many aspects of his practice and personal life. While statistics can be used to justify any purpose or point, reality seems to support Dr. Mullen.

Walk into a hospital and sit in the lobby. Watch the ethnicity of the medical staff. There is no doubt that there has been, and still is, an increasing melting pot comprising our physician base. I am not stating that...
